Previous issue date: 2005-01-28 / The mouth cancer is a disease loaded of stigmata and treated with emphasis in the biomedical view, which doesn't allow the perception of the singularity and the social and cultural context in which the patients are inserted. This study aims at to understand the world and the way of living of men bearers of this illness.There were 6 participants in this research, all of them had a mouth cancer surgery in Santa Casa de Misericórdia Hospital in the city of Fortaleza and they were also assisted at the speech therapy clinic in the same institution. As scenery, there were the homes of the informers, located in the suburban area of Fortaleza, Ceará, Brazil. The data collection was accomplished during the period of April to July of 2004 and for that collection, the ethnographic interview was used as method, composed of 07 (seven) leading themes: knowledge about the disease; lifestyle and the evolution of the disease; impact in the quality of life; strategies and abilities on facing the disease; taken measures; accomplished treatments and aesthetics before and after the surgery. The participant observation was also part of that research. All of the interviews were recorded, transcribed and coded and its contents were analyzed and interpreted based on the theory reference of the medical anthropology, speech therapy and promotion of health. The results evidence that the mouth cancer means the bad disease and dangerous and its therapeutic course needs the use and the combination of different resources such as: the self care, the popular medicine and the bio medicine. The surgery which is a very common technique, generates outstanding and significant impacts in these dimensions: physical-biological, emotional-psychological, social and spiritual. It is represented by mutilation; alimentary restriction, which will be done either by probe or with the modification of the consistence for the pasty and also the total life dependence (affectionate, financial). The overcoming of those impacts is also marked by creative strategies in all those dimensions, that impel them to go on. evidencing then, the Northeastern man's force and his capacity to struggle through a lot of adversities promoting his well being. / O câncer de boca é uma doença carregada de estigmas e tratada com ênfase no enfoque biomédico, o que não permite perceber a singularidade e o contexto social e cultural em que os pacientes estão inseridos. Esse é um estudo qualitativo, com abordagem antropológica, que objetiva compreender o mundo e o modo de viver de homens portadores dessa doença.Os participantes dessa pesquisa foram 06 (seis) pacientes cirurgiados de câncer de boca na Santa Casa da Misericórdia de Fortaleza e atendidos no Ambulatório de Fonoaudiologia da referida instituição. Como cenário, tiveram os domicílios dos informantes, localizados na periferia de Fortaleza, Ceará, Brasil. A coleta de dados foi realizada no período de abril a julho de 2004 e, para tanto, foi utilizada como método a entrevista etnográfica, composta de 07 (sete) temáticas norteadoras: conhecimento da doença; estilo de vida e a evolução da doença; impacto na qualidade de vida; estratégias e habilidades de enfrentamento; providências tomadas; tratamentos realizados e estética antes e depois da cirurgia. A observação participante também fez parte dessa pesquisa. Todas as entrevistas foram gravadas, transcritas e codificadas e seus conteúdos analisados e interpretados com base no referencial teórico da Antropologia Médica, Fonoaudiologia e Promoção da Saúde. Os resultados evidenciam que o câncer de boca significa a doença ruim e perigosa e o seu percurso terapêutico necessita da utilização e combinação de recursos diferentes como: o auto cuidar, a Medicina popular e a Biomedicina. A cirurgia como uma técnica quase sempre empregada, produz impactos marcantes e significativos nas dimensões: físico biológica, emocional psicológica, social e espiritual. É representada pela mutilação; restrição alimentar, em que passa a ser realizada ou por sonda ou com a modificação da consistência para a pastosa e ainda a dependência total diante da vida (afetiva, financeira). A superação desses impactos é marcada também por estratégias criativas em todas essas dimensões, que os impulsionam a ir em frente. Evidencia-se, então, a força do homem nordestino e a capacidade de lutar diante de muitas adversidades e assim promover o seu bem viver. É a significação da necessidade de ter a educação em saúde como estratégia para melhoria da qualidade de vida dessas pessoas.
