Previous issue date: 2013-09-19 / Conselho Nacional de Pesquisa e Desenvolvimento Científico e Tecnológico - CNPq / Fatigue is a common complaint among elders and little is known about its prevalence and associated factors. Besides its significant impact on health conditions and functional incapacity that persist throughout the years, it results in negative impact on the development of activities of daily living, mood changes and increased risk of disability, impaired quality of life and mortality of elderly people. OBJECTIVE: To assess the prevalence and factors associated to fatigue in non-institutionalized elders. METHODOLOGY: This is a population-based cross-sectional study, conducted during the year of 2010. The sample consisted of 635 elders with age of 60 years or older, living in the urban area of Goiânia, Goiás (Brazil). Exclusion criteria were: obtaining a score lower than 20 in the Mini-Mental State Exame (MMSE) and any other reason that impeded the participants to answer the interview by themselves. The dependent variable was fatigue, characterized by two empirical factors: the verbalization of both "fatigue" and "reduced ability to perform usual activities." The independent variables were gender, age, education, marital status, paid work, household income, self-reported need of help in the tasks of everyday life, if the elder had someone to help, body mass index, self-rated health, regular exercise, pain, referred comorbidity, fall during last year, history of alcohol consumption and smoking history, activities of daily living and instrumental activities of daily living. The chi-square test or Fisher test, when necessary, were used to estimate the association between the independent variables and fatigue. Poisson regression with robust variance was performed for multivariate analysis. RESULTS: The prevalence of fatigue was 22.5%, and significantly associated with female sex (p = 0.007), higher education (p = 0.046), self-reported need of help (p = 0.003), self-perceived poor health (p = 0.000), absence of regular exercise (p = 0.001), pain complaints in the past 30 days (p = 0.000), higher number of comorbidities (p = 0.001), and dependence in instrumental activities of daily living (p = 0.000). In multivariate analysis, the variables considered independently associated with fatigue were pain in the last 30 days (PR = 1.46, 95% CI: 1.03 - 2.08, p <0.035) and self-rated poor health (PR = 1.82, 95% CI: 1.29 to 2.57, p <0.001). CONCLUSION: The present study is the first nationwide and estimated fatigue as prevalent in one-fifth of non-institutionalized elderly. Fatigue was independently associated with pain and self-rated poor health which indicates that elders with these two characteristics should receive special attention since they are at greater risk for fatigue. Health professional need training on how to comprehensively evaluate the elderly in order to identify factors associated with fatigue so they can act mainly against those modifiable factors. / A fadiga é uma queixa comum em idosos e pouco se sabe sobre a sua prevalência e fatores associados. Além do impacto significante nas condições de saúde e déficits funcionais que persistem ao longo dos anos, acarreta em prejuízos no desenvolvimento das atividades de vida diária, alterações de humor e aumento do risco para incapacidade, prejuízo à qualidade de vida e mortalidade dos idosos. OBJETIVO: Analisar a prevalência e fatores associados à fadiga em idosos não institucionalizados. METODOLOGIA: Este é um estudo transversal de base populacional, realizado durante o ano de 2010. A amostra foi constituída por 635 idosos maiores de 60 anos residentes em domicílios particulares da zona urbana de Goiânia (Goiás, Brasil). Os critérios de exclusão utilizados foram: obtenção de escore menor que 20 pelo Mini-Exame do Estado Mental (MEEM) e qualquer outra razão que impedisse que o próprio idoso respondesse as questões. A variável dependente deste estudo foi fadiga, caracterizada pelos fatores empíricos: a verbalização de “cansaço” e a “diminuição da capacidade de realizar atividades habituais”. As variáveis independentes estudadas foram sexo, idade, escolaridade, situação conjugal, trabalho remunerado, renda familiar, autorrelato da necessidade de ajuda nas tarefas do dia a dia, se tem alguém para ajudá-lo, Índice de Massa Corporal, autopercepção de saúde, atividade física regular, presença de dor, comorbidade referida, queda no último ano, hábito de consumir bebida alcoólica, história de tabagismo, capacidade funcional atividades básicas de vida diária e atividades instrumentais de vida diária. O teste qui-quadrado ou Fisher, quando necessário, foi utilizado para estimar a associação entre as variáveis independentes e a fadiga. A regressão de Poisson com variância robusta foi realizada para análise multivariada. RESULTADOS: A prevalência de fadiga foi de 22,5%, e significativamente associada com o sexo feminino (p=0,007), maior escolaridade (p=0,046), autorrelato da necessidade de ajuda (p=0,003), autopercepção da saúde ruim (p=0,000), ausência de atividade física regular (p=0,001), queixa de dor nos últimos 30 dias (p=0,000), maior número de comorbidades (p=0,001), e dependência nas atividades instrumentais de vida diária (p=0,000). Na analise multivariada, foram consideradas variáveis independentemente associadas à fadiga a queixa de dor nos últimos 30 dias (RP=1,46, IC95%: 1,03 – 2,08; p<0,035) e autopercepção de saúde ruim (RP=1,82, IC95%:1,29 – 2,57; p< 0,001). CONCLUSÃO: O presente estudo é o primeiro em âmbito nacional e estimou a fadiga prevalente em um quinto dos idosos não institucionalizados. A fadiga mostrou-se independentemente associada à dor e à autopercepção de saúde ruim o que sinaliza que idosos com essas duas características devem receber atenção especial uma vez que estão em maior risco para fadiga. É necessário maior conhecimento por parte dos profissionais de saúde para uma avaliação abrangente aos idosos, e como alternativa para abordar sintomas com a dimensão da fadiga, é recomendada a identificação dos fatores associados a fadiga para que se possa atuar, principalmente, frente aqueles modificáveis.
