Previous issue date: 2008-02-25 / Objectives: This study compared two leprosy rapid tests detecting (1) IgM against PGL-I (ML-Flow test, KIT, Amsterdam, Netherlands) or (2) IgM, IgG and IgA against PGL-I (ML-ICA, Yonsei University, South Korea).
Methods: Whole blood and serum from untreated multibacillary (MB; n=50) and paucibacillary (PB; n=48) leprosy patients (Reference Center for Diagnosis and Treatment, Goiania, central Brazil), MB household contacts (HHC; n=35) and healthy endemic controls (EC; n=50) were tested. Leprosy patients were classified by dermato-neurological evaluation, bacillary index (BI) and histopathology. Median age was 29 years, 39.9% males. MB patients had BB-BL-LL leprosy (median BI=2.0), PB patients had TT-BT leprosy.
Results: (1) Using sera, ML Flow positivity of MB patients was comparable to ML-ICA positivity. (2) For ML-Flow tests, the positivity was comparable with whole blood and serum. (3) Among PB leprosy ML- flow positiviy was comparable for whole blood and serum but it was higher than the ML-ICA seropositivity in whole blood and serum. (4) Among MB leprosy patients positivity of ML Flow test in serum and whole blood was comparable to the positivity of ML-ICA in serum.
Conclusion: ML-Flow test is more field applicable than ML-ICA since high positivity was observed among MB patients using whole-bood. / Objetivos: Este estudo comparou dois testes rápidos para hanseníase que detectam (1) IgM anti-PGL-I (ML-Flow test, KIT, Amsterdam, Holoanda) ou (2) IgM, IgG e IgA anti-PGL-I (ML-ICA, Yonsei University, Coréia do Sul).
Materiais e Métodos: Foram testados sangue-total e soro de pacientes com hanseníase multibacilar (MB; n=50) e paucibacilar (PB; n=48), virgens de tratamento (Centro de Referência em Diagnóstica e Terapêutica, Goiânia, GO), contactantes intradomiciliares de MB (CD; n=35) e indivíduos saudáveis de área endêmica (CS; n=50). Pacientes com hanseníase foram classificados segundo avaliação dermato-neurológica, índice baciloscópico (IB) e exame histopatológico. A mediana da idade foi de 29 anos, 39.9% do sexo masculino. Pacientes MB apresentavam as formas BB-BL-LL (mediana do IB=2.0), Pacientes PB apresentavam as formas TT-BT.
Resultados: (1) Para o grupo MB não observamos diferença significante entre os resultados dos testes ML-Flow em amostras de sangue-total e soro e os resultados do ML-ICA em amostras de soro; (2) Em todos os grupos (MB, PB, CD e CS) que não observamos diferença estatisticamente significativa entres os resultados do ML-Flow em sangue-total ou soro; (3) Entre os pacientes PB o ML-Flow apresentou sensibilidade significativamente maior que o teste ML-ICA, em amostras de sangue-total e em soro; (4) O teste ML-ICA em soro discriminou melhor os pacientes MB e PB.
Conclusão: O teste ML-Flow mostrou-se melhor para utilização em situações de campo que o ML-ICA, pois apresentou alta reatividade em sangue-total em pacientes MB.
