Previous issue date: 2000 / Resumo: A proposta deste estudo foi a de verificar possíveis mudanças estruturais na posição do disco articular em relação ao côndilo e da fossa mandibular com o côndilo, nos portadores de maloc1usão Classe II divisão Ia. de Angle, durante fases diferentes com terapia pelo uso do aparelho funcional Twin-Block, através de imagens obtidas por cortes sagitais de ressonância magnética. As articulações temporomandibulares foram analisadas em cinco ocasiões; TO - início de tratamento com boca fechada e aberta; TI - início de tratamento imediatamente após a colocação do aparelho Twin-Block e em T2 - ao final do tratamento com boca fechada e aberta. Houve alteração do côndilo- fossa e disco-côndilo do tempo TO para TI, no início do tratamento quando a mandíbula foi avançada para frente em tomo de 4 mm. O aparelho Twin-Block produziu modificações nas estruturas mandibulares estudadas e ao término do tratamento (T2) praticamente todos os discos tinham retomado para suas posições de pré-tratamento. Houve preservação do espaço côndilo-fossa mandibular e das estruturas da ATM. O tratamento no geral não resultou em qualquer mudança adversa ou patológica na posição do disco articular, ao contrário, a aplicação do Twin-Block foi útil, devolvendo o equilíbrio das estruturas da A TM e oc1usais, dentro dos padrões faciais. O objetivo foi o de analisar se o aparelho Twin-Block, produziu alteração no posicionamento do disco-côndilo e da fossa mandibular-côndilo, utilizando IRM / Abstract: The purpose of this study was to verify the possible structural changes in the position of the articular disk in relation to the condyle and the fossa with the condyle, on patients who had malocclusion Class II 1 st. Division of Angle, during different phases with therapy using the functional appliance Twin-Block, utilizing images obtained from sagittal slice of magnetic resonance imaging. The TMJ were analyzed in tive occasions: TO - Closed and opened mouth at the beginning of the treatment, TI - At the beginning ofthe treatment immediately after the placement of the Twin-Block, and T2 - at the end of the treatment with the mouth closed and opened. There was an alteration of the condyle-fossa and of the disk-condyle at the TO period to the TI period, at the beginning of the treatment when the jaw was forwarded around 4 mm. The appliance Twin-Block produced jaw growth, at the end of the treatment (T2) all of the disks had retumed to its pre-treatment position. There was a preservation of the space jaw condyle-fossa and of the TMJ structures. The treatment, in general, did not alter any adverse or pathological in the position of the articular disk, on the contrary, the application of the Twin-Block can be useful, giving back the equilibrium of the TMJ structures and oclusal, within the facial standards.
