Previous issue date: 2001 / Resumo: O presente trabalho objetivou analisar, entender e discutir os pontos de vista de uma amostra de psiquiatras e o significado atribuído por eles aos fatores de influência sobre suas indicações de tratamento nos serviços de saúde pública da cidade de Campinas. Os objetivos específicos foram: 1) Identificar e descrever as indicações de tratamento feitas pelos psiquiatras nas Unidades Básicas; 2) Identificar e descrever a visão e significados que os psiquiatras atribuíram acerca dos múltiplos fatores influentes sobre estas indicações. Este estudo pressupôs que a visão dos psiquiatras recairia sobre a grande influência dos fatores institucionais nos tratamentos, mas que as características próprias de sua pessoa seriam fatores fundamentais de influência sobre algumas indicações. As razões para esta pesquisa fundamentaram-se na importância da qualidade da relação médico-paciente nas Unidades Básicas; em conhecer a visão do psiquiatra de maneira a contribuir para nortear aspectos organizacionais do serviço; relevância médico-social da prevenção e abordagem terapêutica na saúde mental na Rede Básica e a possibilidade de que os resultados possam contribuir
para reflexões sobre o tema. O Método utilizado foi o Clínico-qualitativo, operacionalizado através da técnica de Entrevista Semidirigidade Questões Abertas. A amostra foi de 08 psiquiatras da Rede definidos segundo os critérios de inclusão: concordar em participar da entrevista mediante seu tema; aceitar a impossibilidade da delimitação de tempo para a entrevista; aceitar a gravação em fita para posterior transcrição literal; concordar com um segundo encontro caso necessário. Os resultados indicaram que os tratamentos psiquiátricos nas Unidades seguem desde um padrão da clínica clássica até a associação desse padrão a outras alternativas de tratamento possíveis na instituição ou mesmo em outros serviços da Rede. Os fatores que influenciam as indicações de tratamento convergem entre os fatores relativos à pessoa do paciente, fatores institucionais e fatores próprios da pessoa do psiquiatra, mas, em geral estão principalmente baseados em fatores próprios da pessoa do psiquiatra do que a outros por ele argumentados. A pessoa do paciente torna-se um fator entre fatores / Abstract: This current work aimed to analyze, understand and discuss the viewpoints of a sample of psychiatrists and the meaning attributed by them to the mtluencing factors about their treatments prescriptions in the health city service of the city of Campinas. The specific aims were: 1) identify and describe the prescriptions of treatments given by psychiatrists in the basic units of the city health service; 2) identify and describe the conceptions and meanings psychiatrists had concerning the multiple factors influencing on
these prescriptions. This study assumed that the psychiatrists views would confirm the great influence of the institutional factors in the treatments but it also considered that the psychiatrists personal traits would be the most important factors that could influence on some prescriptions given. The reasons for this research indicate the importance of to qualify the doctor-patient relation in the basic health care; to know the psychiatrist's view in order to contnDute for the organization of the service; the social medical importance of prevention and therapeutic mental health approach in the basic care and the possibility that results may contribute to reflections on these issues. The method used was clinical qualitative, using the technique of semi-guided open-ended interviews. The sample was of 08 psychiatrist of the basic care service defined according to inclusion criteria: to agree on participate of the interview about this topic; to accept the prior unknown extent of the interview; to accept the tape recording for later transcription; to agree about a second meeting if necessary. The results indicate that psychiatric treatments in the units follow patterns ranging from the standard clinic to the combination of this pattem with others alternatives of treatment able to be offered in the institution or even in others services of care system. The influencing factors for referals of treatment are those of the patient, institution and personal traits of the psychiatrist but, in general, they are mainly based on the personal traits of the psychistrit than the others they indicated.
