Previous issue date: 2002 / Resumo: Este estudo comparou o valor calórico da terapia nutricional prescrita com a infundida e ambas com o gasto energético total calculado para 107 pacientes. Na análise estatística foi utilizado o teste não-paramétrico de Wilcoxon e o teste exato de Fisher (o nível de significância adotado foi de 5 %). A idade mediana dos indivíduos foi de 45 anos, 77 do sexo masculino e 30 do feminino. Cinqüenta pacientes (46,7%) receberam nutrição enteral, 28 (26,2%) nutrição parenteral e 29 (27,1%) ambos os produtos. A duração da nutrição enteral foi em média 16,5 dias (:t 9,54) e mediana de 14 dias e da nutrição parentera! foi de 14,5 dias (:t 9,76) e mediana de 11 dias. A intercorrência mais fieqüente que levou à suspensão da terapia nutricional foi o jejum, em 78,5 %. O déficit entre a terapia nutricional enteral prescrita (média de 24.975 Kcal:t 19932 Kcal e mediana de 18.820 Kcal) e a infundida (média de 16.138 Kcal:t 14.562 Kcal e mediana de 10.397) foi estatisticamente significante (p = 0,0001). O déficit entre a terapia nutricional parenteral prescrita (média de 25.254 Kcal :t 22394 Kcal e mediana de 20.790 Kcal) e a infundida (média de 19.763 Kcal:t 17758 Kcal e mediana de 13.673 Kcal) também foi significante (p = 0,0001). O custo da perda da terapia total com a utilização da nutrição enteral foi de R$ 5.584,88 (79 pacientes) e com a parenteral foi de R$ 16.150,13 (57 pacientes). Os resultados sugerem que o déficit da terapia nutricional administrada onera a instituição além de trazer conseqüências clínicas para o paciente / Abstract: This study compares the caloric value of the nutritional support prescribed with that implemented and the total energy spent by 107 patients in both cases. The non-parametric Wilcoxon and exact Fisher tests were used (the level of significance adopted was 5%). The patients was monitored in accordance with a standardized daily evolution file, that contained information about the patient's identify, history of the disease, intercurrents, anthropometrical measurements, calculation of nutritional needs, laboratory assessment, quantity of nutritional support prescribed and instituted and others. The average age of the male patients was 46 :t 18,10 years. There were 77 male patients (72 %) and 30 females (28 %). Enteral nutritional support was received by 50 patients (46.7%), 28 patients (26.2%) received parenteral nutritional support and 29 patients were given both nutritional products. The average duration of the enteral nutritional support was 16.5 :t 9.54 days and the parenteral nutrition was 14.5 :t 9.76 days. The most common intercurrent that caused the nutritional support to be suspended was fasting (78.5%). The average difference between prescribed enteral nutritional support (24.975 kcal :t 19932 Kcal) and that established (16.138 kcal:t 14562 Kcal) was statistically significant (p = 0,0001). The cost of the total enteral support lost was U$ 5.584,88 (79 patients) and the parenteral support was U$ 16.150,13 (57 patients). The results indicate that while this deficit is an onus to the institution, it has clinical consequences for the patients. A responsible nutritional support team is fundamental for reducing the costs and complications of this therapy / Mestrado / Ciencias Basicas / Mestre em Clinica Medica
