Previous issue date: 2009 / O Instituto Nacional de Traumatologia e Ortopedia (INTO) foi instituído com a finalidade de assessorar o Ministério da Saúde na formulação da política nacional de promoção da saúde, tratamento e reabilitação de pacientes com doenças do sistema músculo esquelético, qualificar profissionais, desenvolver pesquisas, além de continuar prestando assistência através de sua unidade hospitalar. O Instituto atende, exclusivamente, pacientes do Sistema Único de Saúde (SUS) e se destaca como um centro de excelência no tratamento de doenças e traumas ortopédicos, de média e alta complexidade. Devido ao aumento da expectativa de vida da população, acidentes domésticos e do crescimento da violência urbana (acidentes trânsito, vítimas por arma de fogo), existe atualmente grande demanda em traumatologia nos serviços de emergência. Sendo assim, torna-se necessária a proposição de uma política de saúde, de forma a dar resposta às necessidades da sociedade, diminuindo o número de seqüelas produzidas pelo atendimento inadequado, assim como a sobrecarga do Sistema Previdenciário. Esse projeto tem o objetivo de melhorar o processo da rede de serviços de traumatologia e ortopedia, com o apoio do INTO, como formulador de políticas públicas relacionadas às suas especialidades. Foi utilizado o modelo de avaliação em saúde de Contandriopoulos (1997) para diagnosticar os estrangulamentos existentes no atendimento ao trauma ortopédico no Estado do Rio de Janeiro, no Hospital Estadual Getúlio Vargas(HEGV), no período de 17 de dezembro de 2008 a 30 de junho de 2009. Foi evidenciado um aumento significativo no número de cirurgias realizadas após a implantação das mudanças propostas nesse trabalho, que constam de melhorias no processo de trabalho no HEGV, tais como organização do fluxo de assistência, contratação de profissionais capacitados e envolvidos com o cuidado ao usuário e otimização de materiais e equipamentos. Com os resultados encontrados, observamos que a gestão do processo de assistência ao trauma ortopédico associada a uma central de regulação atuante e com acordos de cooperação entre as três esferas de governo, que até então está incipiente, poderá mudar o rumo e o futuro desses pacientes e da rede de atenção à saúde pública no Estado do Rio de Janeiro. / The National Institute of Traumatology and Orthopedics’ (INTO) was instituted with the purpose to assist the Health Ministry in the formularization of a national politics of health promotion, treatment and rehabilitation of patients with diseases of the musculoskeletal system, to qualify professionals, to develop research, beyond continuing giving assistance through its hospital unit. The Institute takes care of, exclusively, patients of the Public System of Health (SUS) and it stands out as an excellency center in the treatment of average and high complexity orthopedics’ illnesses and traumas. As a result of the increasing life expectancy of the population and the growth of the urban violence, there is currently a great demand in traumatology in the emergency units. Thus, it is essential to recommend health politics guidelines in order to give reply to the needs of the society in addition to diminish the number of sequels produced by inadequate medical care, and the overload of the Public Health System. Therefore, we suggest a pilot project, with the intention to legalize the net of traumatology and orthopedics services, with the support of INTO, in the role of formulator of public politics related to its specialties. We used as a model of assistance to the orthopedics trauma a pilot project developed in the State of Rio de Janeiro at the State Hospital Getúlio Vargas (HEGV) in collaboration with INTO, with the intention to extend it for all domestic territory, and a proposal of analysis model aiming adequacies for the improvement of the assistance. The evaluation was carried out from December 17, 2008 through June 30, 2009. It was evidenced a significant increase in the number of surgeries carried through after the implantation of the project in function of the new model of
management. The effective increase of the productivity is justified by the change in the
process of work management, organization of the assistance flow, admission of skilled
professionals committed with the welfare of the system user and with the optimization of
materials and equipments. Based on the current results, we observed that the management of
the process of assistance to the orthopedics trauma, in association with a central office of
operating regulation and with agreements of cooperation between the three governmental
spheres, we will be able to change the course and the future of these patients and the net of
attention to the public health in the State of Rio de Janeiro.
