Genotypic and phenotypic characterisation of Staphylococcus epidermidis isolated from prosthetic joint infections

Staphylococcus epidermidis has emerged in recent years as an important nosocomial pathogen, especially in infections associated with implanted foreign body materials (e.g., prosthetic joints and heart valves) and in individuals with a compromised immune system (e.g., cancer patients and neonates). Although rare, implant infections are long lasting and cause severe suffering for the patient that includes pain and disability and even increased mortality. One aim of the present thesis was to develop and evaluate a genetic method for species identification and simultaneous detection of rifampicin resistance in staphylococci. A second aim was to examine S. epidermidis isolated from prosthetic joint infections (PJIs) and from wrists and nares of healthy individuals regarding their antibiotic susceptibility, biofilm production, virulence factors, and epidemiology. Comparison with phenotypic diagnostics revealed that 8 (16%) of 49 isolates differed in their species identification in favour of the genetic method. In addition, mutations associated with rifampicin resistance, including two not previously reported, were possible to detect in all isolates resistant to rifampicin. Antibiotic susceptibility testing of 61 PJI isolates showed multi-drug resistance in 91%. Furthermore, the results of the synergy testing revealed that no antibiotic combination was significantly better than the others. Hence, the effects that were possible to detect were isolate dependent. To find a method for discriminating between invasive (n=61) and commensal (n=24) isolates of S. epidermidis genotypic and phenotypic characterisations of biofilm production (including the ica and aap genes), antibiotic susceptibility, virulence-related genes (such as agr and ACME) and epidemiology were performed (using multilocus sequence typing [MLST], typing of the staphylococcal chromosome cassette mec [SCCmec] and PhenePlate). Significant differences were found in antibiotic susceptibility, i.e. there was more resistance among invasive isolates. MLST sequence types (ST) ST2 and ST215 dominated the invasive isolates.

Orientador: Vera Lúcia Mores Rall / Abstract: Salmonella spp. é uma bactéria entérica, responsável por graves doenças de origem alimentar e um dos principais agentes envolvidos em surtos no mundo todo. A contaminação ocorre, principalmente pelo consumo de carne de frango e ovos, uma vez que esses animais podem ser portadores de vários sorovares patogênicos para o homem. O objetivo do estudo foi avaliar o potencial patogênico de 40 cepas de Salmonella spp., isoladas de esteiras de um abatedouro de aves, por testes fenotípicos de adesão, invasão e produção de biofilme e análise genotípica da presença de vários genes relacionados com fatores de virulência, além de verificar sua persistência no ambiente e sua susceptibilidade a antimicrobianos por disco-difusão. Foi observada resistência à tetraciclina (17,5%) ampicilina (10%), cefotaxima (7,5%), cotrimoxazol (5%) e cloranfenicol (2,5%). Todas as cepas apresentaram os genes invA, sipB, sipD, ssaR, sifA, sitC, iroN, tolC, flgK, fljB e flgL. Os genes sopB e sipA estavam presentes em 92,5% dos isolados, enquanto sopD e spvB foram observados em 90% e 32,5% das cepas, respectivamente. Todos os isolados aderiram e invadiram células HeLa, com índice de invasão variando de 1,4 a 73,8%. Com relação à produção de biofilme, 31 (77,5%) cepas foram capazes de produzir biofilme em microplacas de poliestireno. Pela técnica de eletroforese em gel de campo pulsado (PFGE), detectou-se a persistência de clones no ambiente por até 18 semanas, entre as 20 amostradas. Não foi possível o estabelec... (Complete abstract click electronic access below) / Resumo: Salmonella spp. is an enteric bacterium responsible for serious foodborne disease, being one of the main agents involved in outbreaks worldwide. Contamination occurs mainly by poultry and egg consumption once these animals carry some pathogenic serotypes for the human being. Our aim was to evaluate the pathogenic potential of 40 strains of Salmonella spp., isolated from poultry slaughterhouse mats, by adhesion and invasion phenotypic tests, and biofilm production, and genotypic analysis to identify genes related to virulence factors, besides we verified its environment persistence and its antimicrobial susceptibility by disc-diffusion. We observed resistance to tetracycline (17.5%), ampicillin (10%), cefotaxime (7.5%), cotrimoxazole (5%) and chloramphenicol (2.5%). All strains possessed invA, sipB, sipD, ssaR, sifA, sitC, iroN, tolC, flgK, fljB and flgL genes. The genes sopB and sipA was present in 92.5% of the isolates, while sopD and spvB were observed in 90% and 32.5% of the strains, respectively. Estudo de polimorfismos genéticos na susceptibilidade e na resposta à sepse / Study of genetic polymorphisms in the susceptibility and in the response to sepsis

Polimorfismos em genes codificando proteínas envolvidas no reconhecimento de microrganismos e na resposta imune contra patógenos podem influenciar na quantidade ou na função da proteína produzida em resposta ao estímulo bacteriano. Existem evidências de que alguns destes polimorfismos genéticos modificam a resposta do paciente à sepse. No presente estudo, foram estudados polimorfismos presentes em genes que, sabidamente ou supostamente, apresentam um efeito biológico importante na sepse com o objetivo de identificar marcadores associados com a sepse e com a evolução clínica favorável ou com a morte. Entre esses polimorfismos estão aqueles presentes nos genes TNF_, TNFß, IL1RN, HSP70, IL6, IL10, CD14, TLR4, TLR2. As técnicas utilizadas compreenderam PCR em tempo real usando sondas com marcação fluorescente (VIC e FAM), PCR usual, digestão com enzimas de restrição e eletroforese. Os 97 pacientes com sepse, sepse grave e choque séptico estudados foram tratados na UTI do Hospital de Base de São José do Rio Preto. Também foram estudadas amostras de 207 indivíduos saudáveis coletadas no Hemocentro do mesmo hospital. Os polimorfismos IL6-597, HSP70-2 e o haplótipo do gene IL6 apresentaram valores estatisticamente significativos. Os dados obtidos sugerem que os polimorfismos IL6-597 e o haplótipo IL6 -174/-1753/- 2954/-597 estão associados com a evolução clínica da sepse, enquanto o polimorfismo do gene HSP70 está associado com bacteremia. A associação de polimorfismos em pacientes com sepse deverá ser avaliada em um maior número de pacientes no Brasil para ampliar nosso conhecimento sobre variantes genéticas e seus efeitos no curso clínico da sepse. Associações de polimorfismos com sepse e susceptibilidade a infecção podem ser no futuro utilizadas como marcadores moleculares para prognóstico. / Polymorphisms in genes codifying proteins involved in the recognition of microorganisms and in the answer of pathogens can influence the amount or the function of the protein produced in response to bacterial incentive. There are evidences that some of these genetic polymorphisms modify the patient\'s answer to sepsis. In the present study, we studied polymorphisms present in genes that, knowingly or supposedly, has an important biological effect in sepsis with the objective of identifying markers associated with sepsis and with the favorable clinical evolution or with the death. Among those polymorphisms they are those presents in the genes TNF, TNF, IL1RN, HSP70, IL6, IL10, CD14, TLR4, TLR2. The used techniques were Real time PCR using fluorescent probes (VIC and FAM), PCR, digestion with restriction enzymes and electrophoresis. The 97 patients with sepsis, severe sepsis and septic shock studied were those treated in INTENSIVE CARE UNIT of the Base´s Hospital of Sao Jose do Rio Preto. And 207 healthy individuals\' samples were collected in Hemocentro of the same hospital. The polymorphisms of the genes IL6-597, HSP70-2 and the haplotype of the gene IL6 presented estatistical significant values. The polymorphism IL6-597 and the haplotype IL6 - 174/-1753/-2954/-597 can be associated with the clinical evolution of sepsis, while the polymorphism of the gene HSP 70 can be associated with bacteremia. The association of polymorphisms in patients with sepsis will have to be evaluated in bigger samples of patients in Brazil to extend our knowledge on genetic variants and its effect in the clinical course of sepsis. Associations of polymorphisms with sepsis and susceptibility to infection can be in the future used as molecular marker for prognostic.

Perfil de resistência a agentes antimicrobianos de bactérias anaeróbias isoladas de infecções endodônticas agudas

A presente tese teve como objetivos analisar o padrão de resistência a antimicrobianos de bactérias isoladas de infecções endodônticas agudas por meio de uma revisão sistemática e meta-análise (capítulo 1); identificar e determinar a diversidade genotípica, a sensibilidade bacteriana e os fatores de virulência de anaeróbios facultativos isolados em casos de abscesso apical agudo (capítulo 2); e realizar, por meio de uma proposta de informativo, a difusão de informações geradas com orientações sobre o uso de agentes antibióticos em infecções endodônticas agudas para dentistas e pacientes (capítulo 3). A revisão sistemática foi realizada por meio de pesquisa em base de dados na internet e na literatura cinza até maio de 2015. As normas do PRISMA foram seguidas. Estudos clínicos em humanos que avaliaram o perfil de resistência a antimicrobianos de isolados de infecções endodônticas agudas primárias por meio de métodos laboratoriais foram incluídos. O efeito randômico da Meta-análise foi empregado, e o desfecho foi descrito reunindo as taxas de resistência para cada antibiótico testado. Os dados de sete estudos foram extraídos. A taxa de resistência para 15 diferentes agentes antibióticos foram avaliadas, variando entre 3,5% a 40%. Baixas taxas de resistência foram observadas para amoxicilina + clavulanato e amoxicilina, e altas taxas foram observadas para tetraciclina. No capítulo 2, amostras de canais radiculares foram coletadas de sete dentes com diagnóstico de abscesso apical agudo. Bactérias anaeróbias facultativas foram identificadas com o auxílio de métodos fenotípicos e MALDI-TOF MS. A sensibilidade antimicrobiana das cepas isoladas foi determinada para benzilpenicilina, eritromicina e clindamicina por meio da difusão em disco. Bactérias anaeróbias facultativas foram isoladas de 3 dentes. Streptococcus spp., Staphylococcus aureus, Enterococcus faecalis, e Actinomyces viscosus foram identificados. Streptococcus spp. e S. aureus foram sensíveis à benzilpenicilina. E. faecalis (24 cepas) isolados de um mesmo paciente tiveram a sensibilidade antimicrobiana determinada por meio da concentração inibitória mínima para benzilpenicilina, amoxicilina e amoxicilina + clavulanato utilizando-se o E-test. A diversidade genotípica e a presença de fatores de virulência (ace, asa, gelE, efaA, cylA, esp) nas cepas de E. faecalis foi analisada por meio do PFGE e PCR, respectivamente. A expressão da gelatinase e da hemolisina foi testada, e a produção de biofilme quantificada. E. faecalis foram sensíveis aos antibióticos beta-lactâmicos. O mesmo perfil cromossonal de DNA foi revelado para cepas de E. faecalis isoladas. Os genes gelE, ace e efaA foram detectados em 18 cepas. A expressão da gelatinase e a produção de biofilme foram observadas. Cepas de E. faecalis tiveram o mesmo perfil de DNA cromossomal, porém parecem apresentar diferentes perfis de virulência. No capítulo 3 foram apresentadas duas propostas de textos informativos. A primeira com o objetivo de orientar o dentista sobre o uso correto dos antibióticos sistêmicos em endodontia. A segunda visa informar os pacientes sobre a utilização dos antibióticos e esclarecer possíveis dúvidas. / The present thesis aimed to analyze the antimicrobial resistance profile of bacteria isolated from acute endodontic infections through a systematic review and meta-analysis (Chapter 1); to identify and determine the genotypic diversity, antimicrobial susceptibility and virulence factors of isolated facultative anaerobes in isolates from acute apical abscess (Chapter 2); and to provide information for dentist and patients generated from guidelines on the use of antibiotic agents in acute endodontic infections (Chapter 3). The electronic databases and the gray literature were searched up to May 2015 for systematic review. PRISMA guidelines were followed. The clinical studies in of humans that have evaluated the antimicrobial resistance of the isolates of primary acute endodontic infections through laboratorial methods were included. A random effect meta-analysis was employed, and the outcome was described as being the pooled resistance rates for each antimicrobial agent. The data from 7 studies were extracted. The resistance rates for 15 different antimicrobial agents were evaluated, ranging from 3.5% to 40.0%. Lower resistance rates were observed for amoxicillin+clavulanate and amoxicillin, and higher resistant rates were detected for tetracycline. In Chapter 2, root canal samples were collected from seven teeth. Facultative anaerobic bacteria were identified by phenotypic methods and MALDI-TOF MS. Antimicrobial susceptibility of strains was determined to benzylpenicillin, erythromycin and clindamycin by disk-diffusion. Facultative anaerobic bacteria were isolated from 3 teeth. Streptococcus spp., Staphylococcus aureus, Enterococcus faecalis, and Actinomyces viscosus were found. Streptococcus spp. and S. aureus were susceptible to benzylpenicillin. E. faecalis strains (n=24) isolated from the same patients had their susceptibility determined by minimum inhibitory concentration of benzylpenicillin, amoxicillin and amoxicillin + clavulanate using the E-test. The genotypic diversity and virulence factors (ace, asa, gelE, efaA, cylA, esp) were analyzed in E. faecalis strains by PFGE and PCR, respectively. Phenotypic expression of gelatinase and cytolysin were tested. Biofilm production was quantified. All the E. faecalis strains were susceptible to β-lactam antibiotics. The same chromosomal DNA fragmentation profile was revealed to E. faecalis strains isolated. The gelE, ace and efaA genes were detected in 18 E. faecalis strains. Gelatinase expression and biofilm production were observed. E. faecalis strains had the same chromosomal DNA profile, but showed virulence profiles different. In Chapter 3, two sugestion for information texts were presented. The first aims to guide dentists on the proper use of systemic antibiotics in endodontics. The second aims to inform patients about the use of antibiotics and clarify possible doubts.

Excitace v magnetitu ve slabých polích / Low-field excitations in magnetite

We have performed detailed measurements of magnetic and dielectric properties of high quality magnetite (Fe3O4) single crystals in weak magnetic and electric fields. These measurements can reveal details of phase transitions and other features that are not yet fully understood. We focused not only at the Verwey transition - a well known phase transition taking place at about 120 K in stoichiometric samples - but we also explored and described new relaxation effects in magnetite at low temperatures. The low-temperature properties were also found to be exceptionally sensitive to sample quality, stoichiometry and homogeneity. The results presented in this thesis were acquired on sensitive non-commercial SQUID magnetometer complemented by four-probe dielectric spectroscopy and dc conductivity measurements.
