Enhancing Comfort Care Techniques in the Pediatric Inpatient SettingMansky, Nicole Rae 01 January 2019 (has links)
The benefits of pharmacological and nonpharmacological interventions for children undergoing painful medical procedures are well documented in extant nursing literature; however, such techniques continue to be underused in practice. Improving comfort during medical procedures is necessary to enhance the patient and family experience. The purpose of this project was to provide education to nursing staff about pediatric pain theories and evidence-based practice recommendations that support the use comfort interventions for pediatric patients of all ages. The practice-focused question addressed the development of an educational program designed to enhance staff knowledge of comfort intervention options and the benefits provided to patients. The Kirkpatrick model of evaluation was used to guide the educational design and evaluation process. Data analysis demonstrated statistically significant improvement in test scores following the educational intervention. A total of 32 staff members of a unit at the project site participated in the project. Participant pretest scores averaged 61.6% and posttest scores averaged 97.8%, with all but 6 participants having scored 100% on the posttest. The nonparametric test Wilcoxon signed-ranks test was used, and statistical significance was seen (z = -4.969, p = .000). Implications of this project to promote social change include opportunities to expand the use of the educational module to other departments within the organization to promote a culture of comfort through a fact-based understanding of the theories and evidence that support the consistent implementation of comfort techniques.

The work presented in this thesis is devoted to further understanding heat transfer through clothing under different circumstances, in order to provide guidelines for the design and construction of clothing with regard to thermal comfort. In one part of this work, studies were concentrated on the clothing thermal insulation in windy conditions. In this part, a newly designed cylindrical togmeter and a theoretical model have been developed. The numerical solution derived from the theoretical model agrees well with the experimental findings from the cylindrical togmeter in a wind tunnel. The heat transfer mechanisms involved in the wind induced reduction of clothing thermal insulation have been better understood by examining the experimental and theoretical results. The effects of wind velocity, air permeability and stiffness of the outer fabrics, air permeability and thickness of the inner fibrous battings, and the dimensions of the human body on the clothing thermal insulation have also been examined and discussed. Furthermore, based on the understanding of the mechanism of air penetration into permeable clothing assemblies, methods have been proposed for the design and construction of wind resistant protective clothing by using permeable outer fabrics. These methods were evaluated on the cylindrical togmeter and are believed to have important practical values. The other part of this work was focused on the development and laboratory use of a fabric manikin. The "skin" of the manikin was made of coated water-proof fabric, and heated water was circulated inside the "body". The arms and legs of the manikin could be moved to simulate walking. The manikin was very cheap to construct when compared with that of a copper manikin and can be widely applied for routine tests for outdoor and military garments subject to some modifications in its design. With this fabric manikin, a series of experiments have been conducted to investigate the effects of body motion, clothing design and environmental conditions on the thermal insulation of clothing. Some useful information for the design of functional clothing and for the prediction of the thermal stress of a clothed person in different environmental conditions has been provided through this investigation.

Experimental and numerical investigation of the dynamic seat comfort in aircrafts.Ciloglu, Hakan 01 December 2013 (has links)
This research focuses on the dynamic seat comfort in aircrafts specifically during takeoff, landing and cruise through turbulence flight conditions. The experiments are performed using a multi axis shaker table in the Automotive Centre of Excellence (ACE) at the University of Ontario Institute of Technology subjected to sample takeoff, landing and cruise vibration recordings obtained onboard of an actual flight. The input vibrations introduced to the aircraft seats during actual flight conditions and during the experiments in the ACE are compared and it is concluded that the given flight conditions were successfully replicated for the interest of this thesis.
The experiments are conducted with two different aircraft seats, economy class and business class. Furthermore, to investigate the importance of seat cushion characteristics in addition to economy and business class seat cushions, three laboratory made cushions were included in the investigation as well. Moreover, the effect of passenger weight is also discussed by conducting the experiments with 1 and 2 identical dummies.
It is concluded that static seat properties play a significant role in the comfort perception level as well as flight conditions. Among the three flight condition, landing appeared to be the most uncomfortable case comparing to takeoff and cruise.
In addition to experimental work, a numerical study to simulate the flight conditions is undertaken with the initial work of CAD modelling. The simulated responses of the seat is partially matching with experimental results due to unknown parameters of the cushion and the connections of the aircraft seat that cannot be created in the CAD model due to unknown manufacturing processes.
