[pt] A doença renal crônica representa a entrada definitiva no universo da doença e coloca o indivíduo diante de sua inerente condição de finitude. O trabalho com esse grupo de pacientes é permeado por perdas e questões sobre morte, sofrimento e medo. Lidar com a possibilidade de morte e de perdastraz sentimentos singulares para cada sujeito, e este se vê diante do vazio e mergulhado no desamparo. A exigência é de acolhimento, segurança e proteção contra o sentimento de ameaça e direcionada, principalmente, aos profissionais de saúde responsáveis pelo tratamento renal. As características dos pacientes, suas novas construções psíquicas pós-diagnóstico, as dificuldades dos profissionais que lidam com a doença renal crônica e a interação entre ambos são discutidos baseados na revisão bibliográfica e na experiência da autora em uma clínica destinada a esse grupo de pacientes. Objetivou-se ressaltar a importância da relação médico-paciente que privilegie um ambiente de acolhimento, comunicação, incentivo à autonomia e respeito aos diferentes discursos, para garantia de qualidade do tratamento e para uma melhor adaptação e uma nova perspectiva de vida antes da morte. / [en] Chronic renal failure is the definitive entry into the world of disease and puts the individual before his inherent condition of finitude. The work with this group of patients is permeated by loss and questions about death, suffering and fear. Dealing with the possibility of death and loss stirs very personal feelings in subjects, who suddenlyfeel empty and helpless. Their needfor refuge, safety and protection against those threatsis primarily directed towardsthe health professionals responsible for their kidney treatment. The patients characteristics, their new post-diagnosis psychic constructions, the difficulties of professionals who deal with chronic kidney disease and their interaction are discussed based on a literature review and the author s experience in a clinic that treats this group of patients. The objective of this study was to highlight the importance of the doctor-patient relationship insofar as it promotes refuge, communication, autonomy and respect towardsthe different speeches, which results in treatment quality assurance, better coping and a new life perspective before death.
