The diversity of ethnic and cultural groups and the effects of language in the therapeutic relationship are timely professional issues of concern to occupational therapy practitioners. The tri-ethnic, tri-cultural South Florida area offers a natural environment where one can study how patient-therapist interactions are influenced by language barriers in a diverse society. This study examines the effects of language on the adequacy of occupational therapy services, specifically how language affects the length of the treatment program. The nature of diagnosis therapists' ethnicity, and how they impact treatment outcomes are also addressed. A sample was drawn from the occupational therapy outpatient department of a large county hospital. Data taken from patients' charts examined race, sex, age, diagnosis, and language. Number of treatment sessions and length of treatment were viewed as proxy measures for adequacy. Findings indicate that the effect of language cannot be understood aside from ethnicity. Implications for occupational therapy practice are discussed.

Personer med förvärvad hjärnskada (FHS) kan ha svårigheter att använda vardagsteknik (VT),som mobiltelefoner, biljettautomater, datorer, vilka är nödvändiga för och förekommer iflertalet av de aktiviteter vi dagligen utför. Kunskapen om vad som karaktäriserarsvårigheterna i användning av VT samt hur svårigheter hanteras är begränsad. Det övergripande syftet med denna licentiatuppsats var att öka kunskapen om svårigheter ianvändandet av VT, och om vilka strategier som används för att övervinna svårigheter, för attstärka möjligheterna till aktivitet och delaktighet hos personer med kognitiva funktionsnedsättningar efter förvärvad hjärnskada.Licentiatuppsatsen omfattar två delstudier, som fokuserar på deltagarnas användning av VT. I båda studierna enomfördes kvalitativa intervjuer och icke deltagande observationer av 13 personer med en förvärvad hjärnskada i vuxen ålder. Vid analysen av data användes konstant komparativ metod. Delstudie I identifierade och beskrev vad som karaktäriserade svårigheterna i användning av VT i dagliga aktiviteter samt hur svårigheterna påverkade deltagarnas liv. För att få kunskap om hur svårigheter hanterades genomfördes en andra studie, vilken identifierade och beskrev hur personerna hanterade svårigheter med att använda VT och hur detta påverkade utförandet av uppgifter i dagligt liv.Resultatet från delstudie I visade att svårigheterna som identifierades, inte enbart var relaterade till VT, utan även till interaktionen mellan VT, uppgiften som utfördes, personen och miljön. Det var en kombination av svårigheter utifrån varje persons unika livssituation. Svårigheterna i användandet av VT medförde att deltagarna upplevde aktiviteter mer krävande, en förändrad identitet och begränsad delaktighet i hem och samhälle. Resultatet från delstudie II, visade att deltagarna, för att hantera svårigheterna, använde flera typer av strategier. Dessa karaktäriserades av att de varierade i effektivitet beroende på om personen var medveten om svårigheterna, om aktiviteterna med VT var högt prioriterade och av den sociala miljön. När personer med FHS använde pålitliga strategier kunde de övervinna svårigheterna och fortsätta sitt engagemang i aktiviteter med VT.Resultaten visar att svårigheter i användning av VT och de strategier som används för att hantera svårigheterna varierar mellan person och situation. Resultatet visar även att inte enbart VT som sådan bör beaktas för att minska svårigheterna i dagliga aktiviteter, utan att komplexiteten av flertalet samverkande faktorer bör uppmärksammas. The purposes of this study were to identify both the role development of the occupational therapist and the factors which have influenced and directed the role changes. It was assumed that these identifications would assist both the profession and educators of occupational therapists to evaluate the trends as to how and why the profession is developing and to plan for appropriate professional and educational changes for the future. The role of the occupational therapist, an allied health professional, has changed from that of a craft-oriented medical technician to that of a professional clinician, researcher, educator and consultant. This contemporary therapist was found to be practicing in a number of areas both within and outside of the medical model of practice. The development of these new roles has resulted in considerable concern, both outside of and within the profession, as to whether or not the traditional craft-oriented medical model of practice is still valid. As well, occupational therapists in both the United States and Canada have expressed the need to re-evaluate their roles, their responsibilities, and their education in order to meet the needs of changing patterns in the organization and delivery of contemporary health services. The study of the role development and identification of factors influencing the role changes was carried out through a review of literature published between 1922 and 1972 plus a minimal number of unpublished papers related to the topic. A brief review of the 1970-1971 Canadian occupational therapy curricula was included in an attempt to indicate the influences educational programs had on the role development of the occupational therapist. It was found that new roles developed in response to present and proposed government legislation, changing needs and demands of society in general, and changing emphases in medical education and health care delivery. It was also found that the educational influences of occupational therapy programs were minimal, if not inhibitory, to the role development of the occupational therapist. Conclusions drawn from the review of the literature resulted in the establishing of eighteen recommendations concerned with the role development, research needs, and educational needs of future occupational therapists in Canada. It was felt that these recommendations represented guidelines for change which must be implemented by the individual therapists, professional associations, and educators if occupational therapy is to remain a viable health profession. / Education, Faculty of / Educational Studies (EDST), Department of / Graduate
