"Acts of disclosing" : an enthnographic investigation of HIV/AIDS disclosure grounded in the experiences of those living with HIV/AIDS accessing Paarl Hospice House seeking treatmentLe Roux, Rhonddie 10 1900 (has links)

antiretroviral treatment (ARVs) would be made available to people living with
HIV/AIDS. Paarl Hospice initiated a support group for people to deal with this
disease in 2003.
Since February 2004 Paarl Hospice has been recruiting people from the surrounding
informal settlements for ARVs. By means of participant observation I explored how
HIV/AIDS-related disclosure experiences unfolded in places, spaces and events
associated with the support group in the context of factors enabling and preventing
people from accessing Hospice House. I did this by considering the insights drawn
from an anthropological approach. I found the meanings of disclosure in the majority
of studies to be limited and restricted. Available studies approached disclosure in a
top-down fashion by regarding the definition of disclosure as the announcement of
HIV-positivity at the time of diagnosis only. These studies have not considered social
differences relating to disclosure neither did they focus on the actual process of
disclosure.
By means of a constructivist approach to grounded theory I seek to broaden the
definition of disclosure to account for the range of ways in which disclosure practices
take place. I found that disclosure could not be separated from the situational context
in which it occurs and that it can only be understood in relation to the circumstances
and relationships in which it takes place. In this study, disclosure was an ongoing
process, situated somewhere between active, public announcement of an HIV-status
and complete secrecy and somewhere between voluntary and involuntary revealing of
the disease. / AFRIKAANSE OPSOMMING: Paarl in die Wes-Kaap is geïdentifiseer as een van die 15 nasionale areas waar
antiretrovirale medikasie beskikbaar gestel sou word aan mense wat leef met
MIV/VIGS. Paarl Hospice het gedurende 2003 ʼn ondersteuningsgroep geїnisieer om
aan MIV/VIGS aandag te gee.
Sedert Februarie 2004 is Paarl Hospice in die proses om mense te werf uit die
omliggende informele behuisingsgebiede vir antiretrovirale behandeling. Met behulp
van antropologiese insigte en deelnemende waarneming kon ek nagaan hoe
verskillende maniere van MIV/VIGS-verwante bekendmaking ontvou in plekke,
ruimtes en gebeurtenisse wat verband hou met die ondersteuningsgroep. MIV/VIGSverwante
bekendmaking is ondersoek te midde van inhiberende en fasiliterende

ART : the views of counsellors about skills needed in counselling HIV/AIDS patientsFrans, Nocawe R. 12 1900 (has links)
Thesis (M Social Work (Social Work))--Stellenbosch University, 2008. / South Africa is experiencing a serious HIV/AIDS (Human Immunodeficiency Virus/Acquired
Immune Deficiency Syndrome) epidemic, with millions of its people living with the disease and
dying from related diseases. As there is no cure as yet, counselling as a form of intervention is one
of the most powerful ways to address the psychosocial aspects of HIV/AIDS.
The motivation for this study was the lack of research concerning skills needed by counsellors in
counselling HIV/AIDS patients who are on antiretroviral treatment (ART) or are about to
commence antiretroviral treatment. To add to the knowledge that is lacking, the study was
approached by means of both quantitative and qualitative research methods. The purpose of the
research was explorative and descriptive. The discussion on the literature that was studied provides
an overview of the implications of HIV/AIDS for the individual and the family, and of the
psychosocial implications, in addition to describing antiretroviral treatment and the nature of HIV
and adherence counselling processes and skills.
A sample of 16 adherence counsellors who were between the ages of 27 and 57 years was
interviewed. These adherence counsellors were employed by Sothemba Aids Action, placed at the
different ART sites, trained at the Aids, Training, Information and Counselling Centre (ATTIC) and
have gained one or more years’ experience in HIV/AIDS counselling. A semi-structured
questionnaire was used as research instrument. It contained both open- and closed-ended questions.
All the interviews took place at the clinics where the counsellors were employed or worked.
The results of the study showed that an equal number of respondents from two different ethnic
groups were interviewed; all the participants had received high school education; and a few
obtained tertiary level qualifications. They all received training in basic counselling skills and
adherence counselling skills. A few indicated additional training in Voluntary Counselling and
Testing (VCT), as well as Prevention of Mother-to-child Transmission Counselling (PMTC). A
minority of counsellors indicated making use of a translator and that their experience was that the
message was always misinterpreted. All the adherence counsellors indicated a need for further
training involving social problems and counselling of children at different ages and stages of
development. It was also found that the adherence counsellors lack skills in counselling intervention
processes and in intervention. All the adherence counsellors raised concerns regarding their
working conditions and salaries.
In light of the above it is recommended that patients be counselled in their preferred language, that
all counsellors receive the same training in counselling and in the additional areas where they
experience a shortcoming.
Data on HIV/AIDS and adherence counselling, including the views of counsellors and patients are
limited. It is recommended that more research be done on HIV/AIDS and adherence counselling,
including the different disciplines that are involved and the views of patients about services rendered to them by those in the different disciplines, and counsellors.

So ’n ondersoek kan lig werp op toekomstige demografiese arbeidsmagtendense en verwante koste, byvoorbeeld verlies aan produktiwiteit en dienslewering weens afwesigheid, ’n toename in aftredes, en stygende sieke- en pensioenfondseise wat waarskynlik ingedien sal word. Daarbenewens kan so ’n ondersoek die werkgewer in staat stel om noukeuriger vir die toekoms te beplan, vanuit die oogpunt van finansies sowel as menslike hulpbronne.
Daar is egter ’n neiging by werkgewers om MIV/VIGS steeds as ’n maatskaplike of samelewingsprobleem eerder as ’n besigheidspesifieke risiko te beskou. Onkunde is meestal die rede hiervoor. Werkgewers neig om weg te skram van direkte risikobestuur, dikwels met die argument dat dit die regering se plig is om MIV/VIGS-opleiding en gesondheidsorg te voorsien. MIV/VIGS moet soos ander groot geïdentifiseerde sakerisiko’s beskou, gemeet en proaktief bestuur word, net soos met wisselkoers wisselvalligheid, politieke en infrastrukturele risiko’s, en persoonlike en batesekuriteit.
Hoewel hierdie risikofaktore dwarsoor die wêreld bestaan, en sommiges in ander wêrelddele groter is, het hul gekombineerde uitwerking veral in Suider-Afrika ernstige implikasies vir investering en die koste om hier sake te doen.
Soos alle ander sakerisiko’s moet die hantering daarvan multidimensioneel wees:
• Identifiseer, meet en bestuur die risiko; plaas MIV/VIGS eerste op die direksie se agenda.
• Stel senior beamptes aan om die risiko te bestuur.

• verhoogde koste vir personeelwerwing, arbeidsomset en opleiding weens die verlies van ervare personeel. / ENGLISH ABSTRACT: Human Immunodeficiency Virus infection and the Acquired Immune Deficiency Syndrome (HIV/AIDS) is one of the greatest challenges facing employers today, and should provide the impetus for a thorough investigation among employees in order to arrive at an estimate of HIV prevalence within the workplace.
Such an investigation could shed light on future demographic workforce trends as well as related costs, e.g. loss in productivity and service delivery due to absenteeism, increased retirement and a rise in medical aid and pension fund claims that the employer is likely to encounter. Furthermore, this investigation could enable the employer to plan better for the future – both from a financial and human resources viewpoint.
In the ‘real’ world though, employers still perceive HIV/AIDS as a social or community problem rather than a business specific risk. Employers, largely through ignorance, tend to shy away from direct risk management – often using the argument that it is the government’s responsibility to provide HIV/AIDS education and healthcare. In this way they nullify the existing ‘equilibrium convergence’ relationship between the state, business and labour.
HIV/AIDS must be measured and proactively managed and should be regarded in the same light as other major identified business risks, e.g. personal and asset security, exchange rate volatility, and political and infrastructure risks.
While it is true that all of these particular risk factors exist across the globe, and may be greater in other parts of the world, nowhere else do they seem to combine with such severe implications to deter investment and raise the cost of doing business than in Southern Africa.
As for any other business risk, the response should be multi-dimensional:
• Identify, measure and manage; place HIV/AIDS at the top of board agendas.
• Appoint senior executives to manage the risk.
• Regularly evaluate management structures and interventions.
• HIV/AIDS education is key to both management and employees.
Risk management should be holistic. For example, providing healthcare without proactive patient management is pointless. Similarly, treatment plans without funding are futile, and well-funded benefit plans without practical access to treatment are a waste of time.
The aim of this study is to explore the process Old Mutual (SA) followed in estimating the HIV prevalence among its 13 000 permanent employees nationally. The resultant statistics would then be used to project whether or not Old Mutual (SA) will be able to achieve and sustain its employment equity targets, given the HIV/AIDS pandemic. Predictions on the implications of HIV/AIDS for Old Mutual (SA) should be approached with due caution, although this study suggests that it could probably seriously impact on shortages in the supply of labour in future, given the legislative requirements for the various designated groupings.
HIV/AIDS not only causes illness, disability and death among Old Mutual (SA) employees, coupled with severe economic and emotional disruption for their families, it also increases the cost of doing business in South Africa. These costs include:
• increased healthcare expenses;
• increased retirement, pension and death benefit claims;
• decreased productivity as worker absenteeism rises owing to personal illness, or absence from work to care for sick relatives; and
• increased recruitment, labour turnover and training costs due to loss of experienced workers.

Assessing the micro-economic impact of HIV/AIDS on a South African pharmaceutical manufacturer as well as evaluating their policy on HIV/AIDSLudick, Christopher Vernon 03 1900 (has links)
Thesis (MBA)--Stellenbosch University, 2004. / ENGLISH ABSTRACT: HIV infection has increased sharply in SA over the past decade, from almost zero to a
level where between 4-6 million citizens are estimated to be HIV positive (i.e. around Il
percent of the total population). Given the considerable lag and link between the HIV and
AIDS epidemic, the mortality consequences of this exponential increase in HIV infection
over the 1990s are more or less matter-of-fact over the coming decade; even drastic
interventions can do little to avoid this reality, albeit possibly impactingfurther beyond.
The health care industry, and more specifically the pharmaceutical industry, is the only
industry that can have a direct impact on the outcome of the epidemic in terms of
provision of antiretroviral drugs. More importantly, the decision by multinational
companies to provide voluntary licensing to local SA pharmaceutical manufacturers for
the manufacturing of generic ARVs has gone a long way into achieving the World Health
Organisations' objective of providing an ARV cocktail for less than $1,00 per day.
The mam aim of the study is to establish and study the micro-economic effect of
HIV/AIDS on a South African pharmaceutical manufacturer and to evaluate their
HIV/AIDS Policy with the framework of the mV/AIDS & SID Strategie Plan for South
Africa 2000-2005.
Both qualitative and quantitative methods were used to obtain data from various key
informants, manufacturers and market survey companies. The analysis of quantitative
data was done using Excel software and a descriptive analysis method was used to
interpret the data.
The key findings from the study are that Aspen Pharmacare will experience a 20,8 %
HIV prevalence rate in 2005, which will progressively increase to a 25,6 % level in 2015.
This prevalence level will be severely experienced in the skilled, semi-skilled and
unskilled employment of the company during the 2010 period and will start to stabilise in
the latter part of 2015. The AIDS prevalence in the company will increase from a 2,0 %
level in 2005 to a 4,4 % level in 2015. This increase is largely due to the increase in the
prevalence rates in the semi-skilled and unskilled employees. At a senior management level the forecasted number of employees that will have clinical AIDS after 2010 is
between 6 and 8. This clearly indicates that mv/AIDS prevalence at this level is
independent of race and is lifestyle dependent.
If the company were to have the full responsibility for the provision of benefits, based on
the current expected employee benefit structures, the direct cost to company would add
10 % to salary and wages by 2005 and around 20 % by 2010. Indirect costs to company,
such as recruitment and training, increased labour turnover, lost skills and intellectual
property, etc. are estimated to be 2,5 % by 2005 and 5 % by 2010.
With the high HIV/AIDS prevalence rates, especially amongst the unemployed,
companies will have to carry the costs of their mv/AIDS patients for longer and register
then with Aid for AIDS when it becomes too costly. More importantly employers will
have to investigate the cost implication of assisting employee dependents, as this will
have a direct impact on the morale of the employees.
Aspen Pharmacares' mv/AIDS Policy goes beyond the requirements of the mv/AIDS
Strategic Plan for SA in terms of the legal and social requirements. The company also has
a Corporate Social Investment division that assists many NGOs, clinics, hospitals and
communities.
Based on the intellectual property, the pharmaceutical competencies and the continuous
dialogue that exists between the pharmaceutical industry and the department of health,
the researcher concludes, that pharmaceutical companies have an advantage over nonpharmaceutical
companies in dealing with the mv/AIDS issues.
The paper concludes by suggesting recommendations that companies can adopt to ensure
that their mv/AIDS policy can form a significant component of their skills retention
strategy. / AFRIKAANSE OPSOMMING: MIV infeksie het skerp gestyg in SA oor die laaste dekade, vanaf amper geen tot 'n vlak

Assessment of the implementation of the HIV and AIDS policy in the Department of Labour, Western Cape DirectorateLevendal, Carol January 2004 (has links)
Increasing HIV infection rates affect government employees as much as workers in other places. While government has responded to the evolving crisis with a number of policy documents, little is known about the implementation of such policies in government departments. This study assessed the HIV/AIDS policy in the Department of Labour and identified weakness in the implementation. The results of the study may be used by the Dept. of Labour to improve its implementation if necessary.

An assessment of the quality and acceptance of a ready-to-use supplement, Sibusiso, by human immunodeficiency virus and human immunodeficiency virus/tuberculosis treated patients in KwaZulu-Natal.Mabaso, Prudence Bongekile. 29 November 2013 (has links)
Introduction: Malnutrition is a health issue directly and indirectly contributing towards high rates of morbidity and mortality globally, particularly in developing countries. South Africa (SA) is faced with a double burden of diseases with a high prevalence of both under and over nutrition. The high prevalence of Human Immunodeficiency Virus/Acquired Immune Deficiency Syndrome (HIV/AIDS) in SA worsens undernutrition. HIV/AIDS increases nutrient requirements and inadequate food intake results in malnutrition. Nutrition support through food supplementation is important to combat the high prevalence of malnutrition in sub-Saharan African countries including South Africa. Thus, a groundnut-soya based supplementary paste, Sibusiso, has been produced. However, its nutritional composition and acceptability have not been studied.
Objectives: (i) To determine the nutritional composition and physical properties of a ready-to-use supplement, Sibusiso, (ii) To determine the sensory acceptability of Sibusiso among healthy subjects; and sensory acceptability and perceptions of Sibusiso by subjects treated for HIV and HIV/TB.
Materials and methods: The nutritional composition, colour and texture of Sibusiso and a commercial peanut butter (control) were analysed following standard procedures. A cross-sectional consumer acceptability test was done using a 5-point facial hedonic scale (healthy control group, n = 68; HIV, n = 88 and HIV-TB co-infection treated, n = 51). A total of six focus group discussion sessions (HIV subjects = 4 sessions and HIV/TB co-infected subjects = 2 sessions) were also conducted.
Results and discussions: The protein content of Sibusiso (16 g/100 g) was almost half that of the commercial peanut butter (control), (25 g/100 g). However, Sibusiso contained 1.4 times more ash (4 g/100 g) and almost twice as much carbohydrate (40 g/100 g) compared to the commercial peanut butter (22 g/100 g). The fat (40 g/100 g) and energy (2 624 kJ/ 100 g) content of Sibusiso was not substantially different from that of the commercial peanut butter which was 43 g/100 g and 2 852 kJ/100 g, respectively. The lysine content of Sibusiso (58 mg/g) was about 1.7 times higher than that of the commercial peanut butter. The methionine (11 mg/g) and histidine (35 mg/g) content of Sibusiso was almost twice that of the commercial peanut butter, respectively. The nutrient content of Sibusiso was either similar or slightly more than that of other ready-to-use
supplements such as Plumpy’nut®. Sibusiso met the FAO/WHO/UNU recommendations for essential amino acids. The consumption of 50 g of Sibusiso per day may provide approximately 35% of the Estimated Energy Requirements (EER) and 30% of the Recommended Dietary Allowance (RDA) for protein for adults. Sibusiso was brown in colour, similar to the commercial peanut butter. Its textural attributes were found similar to that of the commercial peanut butter but harder and stickier.
The acceptability of Sibusiso was significantly associated (p ≤ 0.05) with the health status of consumers. Overall, Sibusiso was liked by 94% of HIV and HIV/TB individuals (mean score: 4) compared to 85% for the healthy group (control). More than 90% of the HIV/TB and HIV treated individuals liked the taste compared to the control group (86%, mean score: 4). The colour and mouthful were rated 'good' by more than 80% of the HIV and HIV/TB group, mean score: 3, with only 68% among the healthy group, mean score: 4.1.
Conclusion: Sibusiso is a good source of nutrients and was found to be acceptable to HIV and HIV/TB treated consumers. with a well-recognised brand, operating in a competitive environment. The
Metropolitan Doyle model that the company developed in the 1980s was the
first of its kind and is used to predict the course and impact of the HIV/AIDS
pandemic. Metropolitan has since been actively engaged in developing
insurance products, designed to take HIV/AIDS into account. It has,
moreover been active in advocating action on a wide range of issue relating to
the disease, not least through its RedRibbon website, the publication of the
respected journal, AIDS Analysis Africa, and through various community
initiatives, particularly involving HIV/AIDS education. The company’s external
work on the issue of HIV/AIDS is therefore well recognised. Although it has
had an internal programme for several years, it has only seriously addressed
the HIV/AIDS in the workplace since 2003 and in so doing, has not found the
necessity of using any existing models of best practice.
This case study examines the guidelines of a comprehensive HIV/AIDS
programme, as set out by the Global Business Coalition on HIV/AIDS, with a
view to finding out whether and to what extent a company such as
Metropolitan might have implemented its recommendations and what impact it
might have in the fight against HIV/AIDS in the workplace.
The case study further examines the likely impact of the King II
recommendations relating to the disclosure requirements on HIV/AIDS (in
conjunction with the Global Reporting Initiative Guidelines) with a view to
assessing whether these can enable a company such as Metropolitan, to
have regard to the impact of the disease on the sustainability of their business
and the steps that might be taken to mitigate the impact. / AFRIKAANSE OPSOMMING:
Metropolitan Holdings Beperk is ‘n lewensversekerings – en
beleggingsmaatskappy, met ‘n alombekende handelsmerk, wat in ‘n
kompeterende omgewing funksioneer. Die Metropolitan-Doyle model wat
deur die maatskappy in die 1980’s ontwikkel is, was die eerste in sy soort, en
word gebruik om die verloop en impak van die MIV/VIGS pandemie te
voorspel.
Metropolitan is sedertdien aktief betrokke in die ontwikkeling van
versekeringsprodukte, was spesifiek ook MIV/VIGS as oogmerk het. Die
maatskappy is boonop besig om betrokkenheid oor ‘n wye spektrum van
uitkomste verwant aan die siekte te propageer, veral deur die RedRibbonwebtuiste,
die publikasie van die hoogaangeskrewe joernaal, AIDS Analysis
Africa, en deur verskeie gemeenskapsinisiatiewe wat spesifiek die opvoeding
aangaande MIV/VIGS insluit. Die maatskappy se eksterne werk op die
