A group analysis evaluation of the selected members of the acidum family of homoeopathic remedies in terms of known materia medica

Submitted in partial compliance with the requirements of the Master’s Degree in Technology: Homoeopathy, Department of Homeopathy, Durban University of Technology, Durban, South Africa, 2015. / Homoeopathy has advanced as an empirical art and has become a modern age science backed by the kind of massive research data and critical reviews that helps scientific knowledge become acceptable within the global knowledge space. Two centuries ago, the practice of homoeopathy was possible with a few hundred proven remedies of that time but as this science developed through both research and collection of empirical data the number of remedies available increased exponentially. As both the study of remedies and the remedy selection process became more challenging, group analysis by famous homoeopaths such as Farrington (1992), Sankaran (2003), Scholten (1993), Mangliavori (in Vidal 2005) emerged. Currently, with a database of over 3000 remedies available for prescription, group analysis, though a contested approach has proven to be an adequate tool that helps consolidate mass homoeopathic data into meaningful groupings that makes both the study of remedies and their application in practice easier. Classifying homoeopathic remedies into groups by means of group analysis and allowing such methods and results thereof to go through rigorous critiquing refines homoeopathic knowledge and improves its ability to sustain itself as a competent science. Homoeopathy has emerged as a technologically inclined science, utilising various software programs enabling more thorough correlation of symptoms and remedies and so improving the prescription process. Software programs have proven to be very useful tools for the development of group analysis. The aim of this study was to extract the common characteristic symptomatology of five selected homoeopathic remedies belonging to the acidum family as represented in the known materia medica and repertory. The acidum remedies appearing in Radar® 10 repertory (Archibel 2008) were analysed in terms of rubric representation (frequency) and the top five were selected for inclusion in the study. The top five remedies were: Nitricum acidum, Phosphoricum acidum, Muriatic acidum, Sulphuricum acidum and Fluoricum acidum. All rubrics in which the selected remedies appeared were extracted using the homoeopathic software package Radar® 10 (Archibel, 2008) and analysed for sensations and active, passive and compensation reactions of the selected remedies, as per Sankaran (2002). The common primary sensations identified were, burning, sore, swelling, pressing, cramping, dryness, weakness, tearing, and coldness. There were also sensations of anxiety, restlessness, delirium, delusions, dullness, sadness and cheerfulness. Active reactions identified were: hot, heat, inflammation and sensitivity. Passive reactions identified were: coldness, numbness and weakness. Compensation reactions identified were: restlessness, hurriedness, intense reaction and passion. Miasmatic keywords as per Sankaran (2002) were used to determine the miasmatic tendencies of the selected top five acidum remedies. In general all acidum remedies appeared to have a dominant syphilitic miasmatic tendency but acidums were present through all the miasms. Their miasmatic tendencies are easily recognised when viewed in the light of their pathophysiological processes. The pathological tendencies of acidum remedies examined in this study include: haemorrhoids, general haemorrhage, syphilis, warts, neoplastic and non-plastic tumours, irritable bowel syndrome (reflux, colic, constipation), rheumatism, muco-cutaneous ulcerations (STIs, Xerostomia, diabetic ulcers) and bone diseases (osteoporosis, osteitis, and peri-osteitis). The results of this research were compared to the findings of Scholten (1993) and Mangialavori and Marotta (2005). It is the researcher’s view that Sankaran’s methodology of group analysis which he used to explore biological groups of remedies (plants and animals) is adequate for use with non-biological groups of remedies such as mineral acidums. The researcher found group analysis methodology worthy as a tool for classifying the mass data of acidum remedies into orderly sets of meaningful data. Group analysis is consistent with the laws and principles of homoeopathy and encourages the use of materia medica and repertory which are fundamental to the study and application of homoeopathic knowledge. / M

Adverse developmental programming of the adult phenotype by fetal glucocorticoid excess and its prevention by postnatal dietary Omega-3 fatty acids

[Truncated abstract] Increased incidence of hypertension, insulin resistance, obesity and dyslipidemia, collectively referred to as the metabolic syndrome, has been linked to low birth weight, an indicator of a poor fetal environment. This association reflects developmental programming, a process by which organ systems are affected during early development such that disease states are more likely to emerge in adult life. Fetal glucocorticoid overexposure is thought to be a key factor that mediates developmental programming. Accordingly, maternal treatment with the synthetic glucocorticoid dexamethasone retards fetal growth and leads to delayed puberty, hypertension, hyperinsulinemia, and hyperleptinemia, either with or without increased adiposity, in adult offspring. Importantly, the postnatal environment can either amplify or attenuate the long-term outcome of developmental programming. The focus of this thesis was whether adverse developmental programming outcomes can be attenuated by the postnatal environment and thus provide therapeutic potential. Specifically, the effects of a postnatal diet rich in omega-3 fatty acids on glucocorticoid-induced developmental programming outcomes was investigated. ... The adipocyte phenotype was examined in Study 6, with hyperleptinaemia evident in offspring at 6 and 12 months of age in dexamethasone-exposed animals on a standard omega-3 diet, but this effect was prevented by a high omega-3 diet. The pattern of plasma leptin was paralleled by changes in leptin mRNA in retroperitoneal fat. Similarly, plasma levels of the inflammatory markers IL-6 and IL-1β were upregulated by prenatal glucocorticoid exposure and these were attenuated by postnatal dietary omega-3 fatty acids. Overall, omega-3 ingestion reduced adiposity, as indicated by measures of body composition. In conclusion, the studies presented in this thesis demonstrate for the first time that many of the detrimental effects of excess glucocorticoid exposure in utero on the adult phenotype can be attenuated by a postnatal diet rich in omega-3 fatty acids. This beneficial effect of omega-3 fatty acids was associated with a reversal of some (e.g. adiposal leptin) but not all (e.g. renal GR) 'programmed' changes in gene expression. These findings raise the possibility that dietary supplementation with omega-3 fatty acids may provide a viable therapeutic option for preventing and/or reducing adverse programming outcomes in humans.

Measurement of fish consumption in population-based studies of cancer

[Truncated abstract] The role of fish consumption and omega-3 polyunsaturated fatty acids (PUFAs) in the prevention of disease has been the subject of much investigation in recent years. A clue that these factors might be of importance was the observation that populations consuming high levels of marine omega-3 PUFAs had lower rates of morbidity and mortality from cardiovascular disease. From there, research in this area has expanded to include a range of chronic diseases and their prevention. An area of particular interest is the role of fish consumption in the prevention of various cancers. In Australia, one in three men and one in four women are expected to be diagnosed with cancer by the age of 75. Identifying preventive factors that can be translated into constructive health promotional messages is of great importance in addressing this group of diseases that has such a large impact on the health and wellbeing of the population. ... Results and conclusions Analysis of the data from the population-based case-control study suggests a protective effect of preserved fish consumption, possibly due to the high oil content of these fish. The developed questionnaire is a valid and reliable tool for measuring fish and seafood consumption as determined by regression analysis with an independent biomarker and reliability analysis using intra-class correlation. Importantly, reliability can be maintained despite asking a high level of detail from participants. For ranking according to overall fish consumption, detailed questioning is probably not necessary, however inclusion of variables representing multiple categories of fish and seafood consumption in a regression model enables us to better account for variation in blood omega-3 PUFA levels than a single variable representing overall consumption. For the purpose of questionnaire validation, plasma phospholipid and erythrocyte membrane levels of EPA and DHA are equivalent biomarkers of fish and seafood consumption. The choice between them by future investigators will be based on more practical aspects such as convenience and the fasting state of subjects. The tangible product of this thesis is additional evidence to support a protective association between fish consumption and prostate and breast cancers, and a valid and reliable questionnaire v for measuring habitual consumption of fish and seafood in a West Australian population, that could also be applied to other populations after minor adjustment for local fish and seafood consumption patterns.

O processo de reparação tecidual evolui tardiamente com alteração das propriedades físicas da pele, cuja gravidade varia, entre outros fatores, em função da profundidade original da lesão. Clinicamente a pele é seca, descamativa, discrômica e com diminuição da elasticidade, especialmente em cicatrizes hipertróficas. As características do tegumento pós-queimadura podem ser melhoradas com agentes de uso tópico normalmente utilizados em patologias como acne e fotoenvelhecimento. Foram avaliados 77 pacientes portadores de seqüela de queimadura peri-oral, entre seis e 46 anos de idade. O tipo de tegumento foi classificado como restaurado, quando epitelizado espontaneamente, ou enxertado. O tratamento tópico consistiu de tretinoína (0,01-0,05%) e ácido glicólico (5-7%) durante três meses. Duas distâncias foram medidas com paquímetro digital em abertura bucal máxima, a inter-dentária (D) e a inter-labial (L). Houve aumento significativo (p£0,01) de D e L nos grupos tratados quando comparados aos respectivos controles, tanto nos pacientes com tegumento restaurado como nos enxertados. O aumento de L foi significativamente maior que D. Sugerimos a utilização da medida inter-labial como parâmetro complementar à medida inter-incisal como método de avaliação indireta da elasticidade cutânea no tegumento pós-queimadura. / Tissue repair processes evolve with alterations of the cutaneous physical properties, which are proportional, among other factors, to the original depth of the lesion. At examination, the skin is dry, dyschromic and less elastic, specially in hypertrophic scars. Post-burn skin characteristics can be improved by topical agents regularly used in pathologies like acne or photodamage. We evaluated 77 patients who had peri-oral burn sequela. The ages ranged from six to 46 years old. The tegument was classified as restored, when spontaneously healed, or skin grafted. The topical treatment consisted of tretinoin (0,01-0,05%) and glycolic acid (5-7%) for three months. Inter-incisal (D) and inter-labial (L) distances were measured at maximal mouth opening with a digital pachymeter. Both inter-incisal and inter-labial distances increased significantly in the groups treated when compared to respective controls (p£0,01). This fact was similar in both types of tegument. Inter-labial distance (L) increased significantly more than D. We suggest its use as a complementary measure to inter-incisal distance for indirect evaluation of skin elasticity in the post-burn tegument.
