The need for social work intervention for the elderly patients and their family members

Contends that poor discharge planning for elderly patients in American hospitals is the result of reduced lengths of stay which do not give medical social workers adequate time to assess patients' needs. A survey methodology was used to assess social service and community resource needs of hospitalized elderly patients and their family members at Riverside Community Hospital in California. Argues that the results of the survey can be used to improve discharge planning and lead to a more client-centered practice in hospitals.

Osamostatňování klientů zařízení náhradní výchovné péče v mezinárodním srovnání / Gaining Independence of Clients of Facilities of Alternative Education Care in International Comparison

Gaining independence of clients in the system of the alternative education care is the cardinal topic of the work. Within the framework of the international comparison, the author analyses and makes a survey of the problems accompanying this process. The work does not contain only the conclusions the author came to based on the research in chosen countries (Czech Republic and Poland) but also presents suitable recommendations how the problem of gaining independence of clients of the alternative education care can be solved. The introductory part is applied to theoretical solutions, the international comparisons and examples of a good practice abroad. The author follows opinions of different experts in causalities of the problem from the influence of the biological family through socialization of the clients of the education care to the very problems of the institutional care. He presents these opinions subsequently in the context. In the methodological and research parts the author tries to answer the cardinal question: "What is way the clients of the alternative education care are prepared in order to gain independence?" Background: The definition and concept of the rehabilitation differs across disciplines. However, the goals of comprehensive rehabilitation, as defined in the literature, could not be fully achieved without multidisciplinary cooperation, through which it is possible to apply a wider range of interventions, which is an important indicator in addiction treatment. The cooperation between individual fields has the potential not only in better quality care towards the target group, but it is also important to consider the possibility of better financing of care, which could lead to greater satisfaction of both users and service providers. Objectives: This diploma thesis seeks the interrelationships between theory and concept of rehabilitation in addictology, respectively in the aftercare program based on the Apolinar model of addiction treatment, and rehabilitation in physiotherapy - in terms of both theoretical anchoring and implementation of rehabilitation in practice. Penological investigation of the offender rehabilitation path

The thesis examine the process of rehabilitation offered by the Department of Correctional Service as from the time the offender is admitted (sentenced) until such time he is released back into society. Rehabilitation in the departmental context is based on four key delivery areas, which must be in place to ensure that offenders are rehabilitated. Faced with several challenges, e.g. demilitarization, structural defects of the facilities that are not conducive for rehabilitation or build for Unit management principles, career path developed for officials, and the transformation of the old penitentiary system to the new generation prison system, the department failed to achieve their objectives. The thesis will examine the current process of rehabilitation in the Department of Correctional Service, and identify areas of under performance to seek best practices to improve service delivery. The dissertation analyzes the personnel management system in healthcare facilities with after-care and long-term care in the Czech Republic, focusing on the implementation of certain personnel activities. The main focus is on the characteristics of health facilities and the subsequent long-term care in the Czech Republic, staffing and elements that influence the work of personnel in such organizations. The individual personnel activities in healthcare facilities with after-care and long-term care in the Czech Republic are put into a structure. The thesis includes a qualitative research survey of the level of personnel management in healthcare facilities with after-care and long-term care in the Czech Republic established by both public and private sector, having different legal structures. The research survey focuses on learning about the form of personnel management in healthcare facilities with after-care and long-term care in the Czech Republic, it tracks personnel management as a complex system of partial personnel activities.
