Previous issue date: 2011-10-26 / Objetivo: Avaliar em pacientes obesos mórbidos submetidos à derivação gástrica em Y de Roux com anel de contenção há mais de cinco anos: 1) o efeito a longo prazo da cirurgia nas comorbidades; 2) a relação deste efeito com o reganho de massa. Métodos: Foram estudados 140 pacientes (122 mulheres, média de idade de 41,4±10,6 (variação 19 a 62) anos, média de massa de 138,7±23,1 (variação 98 a 220) kg e média de IMC de 52,3±7,9 (variação 37 a 82) kg/m2 submetidos à derivação gástrica em Y de Roux com anel de contenção há cinco anos ou mais. Foram analisadas as seguintes comorbidades: diabetes tipo 2, hipertensão arterial, doença cardiovascular, dislipidemia, apnéia do sono, artropatia e infertilidade, e classificadas em Resolvida, Melhorada, Inalterada ou Piorada no momento de menor massa e no último seguimento. Resultados: Houve diferença significativa na variação da massa nos três momentos do estudo (inicial x mínimo, p<0.001; inicial x final, p<0.001; mínimo x final, p<0.001). Não houve variação do status obtido no momento de menor massa para diabetes tipo 2, doença cardiovascular e infertilidade. Hipertensão arterial mostrou discreta piora dos resultados, porém, sem significância estatística (p=0,8). Houve piora dos resultados para dislipidemia (p=0,012), apnéia do sono (p=0,046) e artropatia (p=0,007). Não há relação entre a variação do IMC e a evolução da comorbidade para infertilidade (p=0,71, massa mínima e p=0,42, massa final). Há relação direta entre a variação do IMC e a evolução das comorbidades para diabetes tipo 2 (p=0,013, massa mínima e p=0,014, massa final), hipertensão arterial (p=0,015, massa mínima e p=0,001, massa final) e doença cardiovascular (p=0,015, massa mínima e p=0,043, massa final). Há relação direta entre a variação do IMC e a evolução das comorbidades apenas no último seguimento para artropatia (p=0,096, massa mínima e p=0,0012, massa final), dislipidemia (p=0,054, massa mínima e p=0,015, massa final) e apnéia do sono (p=0,11, massa mínima e p=0,011, massa final). Conclusões: Em pacientes obesos mórbidos submetidos à derivação gástrica em Y de Roux com anel de contenção há mais de cinco anos: (1) Há manutenção do status obtido para o momento de menor massa para as comorbidades diabetes tipo 2, hipertensão arterial, doença cardiovascular e infertilidade, mas não para dislipidemia, apnéia do sono e artropatia; (2) A melhoria de todas as comorbidades avaliadas, com exceção da infertilidade, associou-se diretamente ao percentual de perda de massa corpórea. / BACKGROUND: Roux-en-Y gastric bypass (RYGB) is considered an effective therapy for weight loss although weight regain may be observed in a long-term follow-up. Obesity-related comorbidities are also well treated by RYGB due to weight loss and intestinal hormone changes. Few studies reported long-term status of comorbidities especially if weight regain is present. This study aims to analyze: (1) the resolution of obesity-related comorbidities after RYGB in a long-term follow-up and (2) its relationship to weight regain. METHODS: 140 patients (mean age 41, 18 male, mean body mass index (BMI) before operation 52 (range 39-82) kg/m2, mean BMI at nadir weight 29 (range 18-46) kg/m2, mean BMI at last follow-up 33 (range 19-49) kg/m2 were followed-up after RYGB for morbid obesity for at least 5 years (mean follow-up 90, range 60-155 months). Diabetes, cardiovascular disease, arterial hypertension, dyslipidemia, sleep apnea, arthropathy, and infertility were present when submitted to surgery in 26 (18%), 14 (10%), 88 (62%), 18 (13%), 83 (59%), 90 (64%), 9 (16%) patients; and in 6 (4%), 8 (5%), 38 (27%), 6 (4%), 45 (32%), 75 (53%), 7 (12%) patients at the Nadir weight, respectively. RESULTS: BMI was significantly different in the 3 periods. Comorbidities resolution was sustained in a long-term follow-up for diabetes, hypertension, cardiopathy and infertility. Comorbidities status was direct related to the % of weight loss for all comorbidities except infertility. CONCLUSION: Our results show that comorbidities remission after RYGB is sustained in the majority of patients in a long-term follow-up. Mudanças no hábito de fumar e ganho de peso gestacional : um estudo de coorte em capitais brasileiras / Smoking cessation and prenatal weight gainFavaretto, Ana Lenise Ferreira January 2001 (has links)
Objetivo: Avaliar a influência de mudanças no hábito de fumar sobre o ganho de peso gravídico materno. Métodos: Foram entrevistadas 5.564 gestantes com 20 anos ou mais, sem diabetes mellitus prévio em serviços de pré-natal geral de seis capitais brasileiras, entre 1991 e 1995, e acompanhamos, através de revisão de prontuários, as gestações até o parto, identificando 4.000 gestantes com peso pré-gravídico relatado, peso medido no terceiro trimestre, hábito de fumar e época de sua eventual modificação, quando disponíveis. Resultados: Entre as gestantes ex-fumantes (915, 23% do total), 240 (26%) pararam de fumar durante a gravidez. A mediana de cigarros/dia das que continuaram fumantes (717, 18%) foi reduzida de 10 para 5 após o início da gravidez. Após ajustar para idade, escolaridade, cor da pele, IMC pré-gravídico, paridade e centro clínico, as ex-fumantes ganharam 1.030 g (IC95% 590 a 1.460) a mais que as nunca fumantes, sendo maior a diferença (1.540, IC95% 780 a 2.300 g) nas que pararam após a concepção. O ganho do peso na gravidez se correlacionou, tanto em fumantes quanto em ex-fumantes, com o número de cigarros diminuídos na gravidez. Conclusão: Diminuir ou parar de fumar na gravidez, embora importante para uma gestação saudável, é fator de risco para ganho de peso materno. / Objective: Evaluate the association of changes in smoking habit with maternal weight gain. Methods: We interviewed 5564 pregnant women > 20 years, without prior diabetes mellitus, during a second trimester pre-natal visit in general prenatal care clinics in 6 Brazilian cities, from 1991 to 1995, and followed them, through chart review, to term. We now report associations in the 4000 women who had complete information concerning pre-pregnancy and 3rd trimester weight, smoking status and its eventual changes during pregnancy. Results: Of women who stopped smoking (915, 23% do total), 240 (26,2%) stopped during pregnancy. The median number of cigarettes smoked/day among those who continued (717, 18%) decreased from 10 to 5 with pregnancy. In linear regression models adjusting for age, educational level, ethnicity, pre-pregnancy body mass index, parity and clinical center, exsmokers gained 1030 (95%CI 590 – 1460) grams more than never smokers, this difference being greater – 1540 (95%CI 780 – 2300) grams – in those who quit while pregnant. The size of weight gain in both smokers and ex-smokers was proportional to the quantitative reduction in daily number of cigarettes smoked during pregnancy (p=0.007). Conclusion: Stopping to smoke or decreasing the quantity of cigarettes smoked in pregnancy, although important for maternal and child health, is a risk factor for maternal weight gain.

A population perspective on obesity prevention : lessons learned from Sweden and the U.S.Nafziger, Anne January 2006 (has links)
Obesity prevalences are increasing in Sweden and the US. Obesity has many health consequences and health risks are associated with small increases in weight and marked obesity. Cross-sectional and panel surveys from northern Sweden and upstate NY provide the basis for furthering understanding of body mass index (BMI) development. BMI and weight change (+/-3%) were used to evaluate obesity and weight loss, maintenance, or gain. The 1989 prevalences of obesity were 9.6% and 21.3% in Sweden and the US; 1999 prevalences were 18.4% and 32.3%. Ten-year incidences (1989-1999) of overweight and obesity were 337/1000 and 120/1000 for Sweden and 336/1000 and 173/1000 for the US. Cross-sectional data suggest obesity is a problem of older age while panel data show that the young are gaining weight most rapidly. Individual changes in BMI have similar trends for Sweden and the US; the majority of adults are gaining weight. Older age, being a woman, higher BMI, and type 2 diabetes were associated with higher odds of weight non-gain. Younger age, being a man, being married and using snuff (snus) increased the odds of weight gain. The obese were 2-7 times more likely to report self-rated poor health. Healthy behaviours explain more of the person-to-person variability in BMI than do unhealthy behaviours or chronic diseases. Encouraging trends were found among Västerbotten Intervention Programme participants: a higher proportion of adults maintained weight in more recent survey years. The proportion of weight-gaining adults with identified health risk factors is smaller than those without risk factors. Frequently weight maintenance is felt to be important only for those identified as having a problem with weight or an obesity-related health condition. The largest proportion of those gaining weight are those with a normal BMI. Obesity prevention should target those usually considered low-risk (young, without cardiovascular risk factors, normal BMI).
