Avaliação nutricional pré-cirúrgica de crianças com lesão labial e/ou palatal /

Orientador: Flávio Luís Moreira / Resumo: A lesão lábio-palatal é uma malformação congênita com incidência, no Brasil, de 1:600 nascimentos. Quando a fissura acomete o palato, os problemas alimentares mais observados são sucção insuficiente, perdas alimentares, cansaço ao sugar com gasto energético aumentado, deglutição excessiva de ar, ingestão inadequada, ganho de peso insuficiente e comprometimento do estado nutricional. São crianças que dificilmente aleitam ao seio e ainda são mais expostas a infecções respiratórias e otites médias. A correção das malformações implica em vários processos cirúrgicos iniciados em torno do terceiro mês de idade, com o fechamento do lábio e o palato sendo fechado entre 12 a 18 meses. Após esta idade são comuns cirurgias de revisão de palato, secundária de lábio, de columela, etc. Todos estes processos acabam comprometendo mais ainda o estado nutricional dos pacientes. Tendo em vista os aspectos acima, foi proposto no presente estudo avaliar o estado nutricional pré-cirúrgico de crianças de 2 a 7 anos, portadoras de fissura lábio e/ou palatal, segundo os indicadores dietéticos, antropométricos e laboratoriais e segundo o tipo de fissura. A população estudada foi constituída por quatrocentos e oitenta e nove pacientes de ambos os sexos, não sindrômicos, após aceite dado por escrito pelo responsável legal de cada criança. Os pacientes de cada faixa etária foram subdivididos em 3 grupos de acordo com o tipo de fissura e submetidos às avaliações dietéticas, antropométricas e laboratoriais estabelecidas. Os dados obtidos foram comparados com os valores padronizados por estudos internacionalmente reconhecidos e também receberam tratamento estatístico pertinente. (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Cleft lip and palate is a congenital malformation affecting one out of every 600 births in Brazil. The most frequent feeding problems associated with cleft palate are sucking inability, food losses, tiredness upon sucking with increased energetic demand, excessive intake of air, improper ingestion, insufficient weight gain and disturbances of the nutritional status. These children are seldom breastfed and are more prone to respiratory infections and otitis media. Correction of the malformation comprises several surgical procedures initiated around three months of age, when lip surgery is performed, followed by palatal closure between 12 and 18 months old. After this age range there may be secondary palate or lip surgeries, columella lengthening, etc. All these interventions may pose even more damage to the nutritional status of these patients. Considering all these aspects, the present study was proposed in order to evaluate the pre-surgical nutritional status of children aged 2 to 7 years old presenting with cleft lip and/or cleft palate, according to dietary, anthropometric and laboratory indexes and according to the type of cleft. The study population comprised four hundred and eighty-nine patients of both genders, without syndromes, after achievement of written consent from the caretaker of each child. Patients in each age range were subdivided in three groups according to the type of cleft and submitted to the established dietary, anthropometric and laboratory evaluations. The obtained information was compared to standardized values proposed by worldwide recognized studies and proper statistical analyses were also performed. Development of the palate morphology and shape of the dental arch in patients with cleft Objective: The aim of this study was to compare the differences in development of the palate morphology and in the relationship and shape of the dental arches in two groups of individuals with different types of orofacial cleft defects. Introduction: Orofacial clefts are the cause of the insufficient growth of the face. Range of the resulting deformation depends on the type of cleft, individual growth disposition of the patient and comprehensive interdisciplinary treatment of the cleft. Material and Methods: The sample consisted of 18 patients with unilateral cleft lip and palate and 18 patients with isolated cleft palate. Measurements were performed on study casts of patients aged 9 and 13 years in average. Following measurements were performed: the frontal and distal width of the upper and lower dental arch, the length of both halves of the upper dental arches, the frontal length and depth of the upper dental arch and the overjet. According to the sagittal relationship of the dental arches, patients were classified into one of five categories of the GOSLON yardstick score. Para tanto, foram usadas um total de 13 ratas do tipo Holtzman. G1) 3 ratas receberam 3mg/Kg Dex. Intramuscular (i.m.) do 14o ao 17o dia de prenhez. G2) 4 ratas receberam 3mg/Kg Dexa (i.m.) do 14o ao 17o dia de prenhez, e uma dose de 5mg/kg de ácido fólico (via oral) do dia 14o até 17o de prenhez. G3) 3 ratas receberam 3mg/Kg Dexa (i.m.) do 14o ao 17o dia de gravidez e uma dose de 5mg/kg de ácido fólico (via oral) do início até o final da prenhez. G4) 3 ratas receberam 3mg/Kg Dex (i.m.) do 14o ao 17o dia de prenhez e uma dose de 5mg/kg de ácido fólico (via oral) desde 5 dias antes da concepção até o final da gravidez. As cabeças dos fetos foram removidas no 19o dia de prenhez e estudadas. Foi utilizado o teste não paramétrico de Kruskal-Wallis. O resultados mostraram o seguinte: G1) dos 27 fetos, (96,30%) apresentaram fenda total, e apenas 1 apresentou fenda parcial. G2) Dos 23 fetos apenas (8,7%) com fenda total, (43,35%) com ausência de fenda e (47,80%) apresentaram fenda parcial. G3) Observou-se 88,24% apresentaram fenda incompleta, 5,88% apresentaram fenda completa e 5,88% ausência de fenda. G4) dos 31 animais, (54,4%) apresentaram fenda incompleta, e 32,26% apresentaram fenda completa e (12,9%) sem fenda. / The aim of the present study is to determine whether (FA) prevented cortisone-induced cleft palate (CP) in the Holtzman mouse. The animals were divided in 4 groups: G1- 3 pregnant mice had 3mg/Kg of Dex intramuscularly administrated (IA) from the 14th to 17th day of gestation; G2- 4 pregnant mice had 3mg/Kg of Dex IA from the 14th to 17th day of gestation, with a daily dose (dd) of 5mg/kg FA oral administrated (OA) from 14th to 17th day of gestation; G3- 3 pregnant mice had 3mg/Kg of Dex IA from the 14th to 17th day of gestation, with a (dd) of 5mg/kg FA OA from day 0 until 17th day of gestation; G4- 3 pregnant mice had 3mg/Kg of Dex IA from 14th to 17th day of gestation, with a (dd) of 5mg/kg FA OA 5 days before the fertilization until the 17th day of gestation. The fetuses were examined on 19th day of gestation. The data was analyzed by not paired Kruskal-Wallis test, and showed statistical difference among average score values. The results demonstrated that (96,3%) (G1) had total CP, and only 1 partial CP. In the (G2), (8,7%) showed total CP, (47,8%) partial CP and (43,35%) showed no CP. The (G3) showed (5,88%) had total CP, (88,24%) partial CP and (5,88%) had no CP. In the (G4), (54,4%) showed partial CP and (32,26%) total CP. Only (12,9%) had no CP. These results indicated that FA reduces the incidence of Dex-induced cleft palate independently on the period of the administration.

A avaliação subjetiva foi realizada por 20 avaliadores provenientes de quatro grupos constituídos por: fonoaudiólogas; pais e pacientes portadores de fissura de lábio e/ou palato; cirurgiões dentistas, fisiologistas e geneticista; cirurgiões plásticos do Hospital de Reabilitação de Anomalias Craniofaciais, da Universidade de São Paulo. A avaliação objetiva consistiu de mensurações no lábio superior dos lados fissurado e não fissurado realizadas pelo autor. Os resultados mostraram que as mensurações dos lados fissurado e não fissurado apresentaram-se diferentes e 70% das avaliações dos examinadores foram satisfatórias. Não houve correlação significante entre o julgamento dos avaliadores e as diferenças mensuradas entre os lados fissurado e não fissurado. / The aim of the present study was to subjectivelly and objectivelly evaluate the aesthetics of the lip of 38 cleft lip patients, 21 males and 17 females, aging 4 to 14 years (mean eight years and eight months) who have undergone lip surgery by the SPINA’S technique, and to correlate both kinds of evalutions. Subjective evaluation was performed by 20 individuals among speech/language pathologistis, cleft lip and palate patients, cleft lip and palate patients parents, dentists, physiologists, geneticist and plastic surgeons from the Hospital of Rehabilitation of Craniofacial Anomalies, University of São Paulo. Objective evaluation consisted of measurements in the cleft and noncleft sides of the upper lip, which were performed by the author. The results have shown that there were differences in the measurements in both sides of the lip, and 70% of the subjective evaluations were considered satisfactoriy. No significant correlation was found between the subjective evaluation and the measurements obtained in the cleft and noncleft sides of the patients lips.
