Abstract
The most threatening outcome of interstitial lung diseases is death caused by progressive pulmonary fibrosis characterised by increased collagen deposition, although the clinical course is highly variable. The aim of this study was to evaluate the role of procollagen I and III propeptides in estimating collagen metabolism and its relationship to disease activity and prognosis in patients with sarcoidosis, fibrosing alveolitis and asbestos-related lung diseases.
The study included 160 patients. The levels of procollagen I carboxyterminal propeptide (PICP) and procollagen III aminoterminal propeptide (PIIINP) in serum, bronchoalveolar lavage fluid (BALF) and epithelial lining fluid (ELF) were assessed from 137 patients employing human antigens. There were 60 patients with sarcoidosis, 18 with fibrosing alveolitis and 5 with asbestosis and 17 controls. Thirty-seven patients had been exposed to asbestos, but did not show parenchymal involvement. Twenty-five of them had pleural plaques, while 12 had normal chest radiographs. Immunohistochemical stainings for procollagen I aminoterminal (PINP) and III aminoterminal propeptide were carried out on open lung biopsies of the remaining 23 of the 160 patients, of whom 13 had sarcoidosis and 10 fibrosing alveolitis. Antibodies to these procollagen peptides react with the aminoterminal domains of the corresponding propeptides intracellularly and with the respective pN-collagen in collagen fibres in the extracellular space.
Procollagen III aminoterminal propeptide was elevated in the sera of the patients with sarcoidosis and fibrosing alveolitis, but not in the asbestosis or asbestos-exposed patients as compared to the controls. The level of PIIINP in BALF was highest in sarcoidosis and second highest in fibrosing alveolitis, but hardly detectable in the other groups. BALF-PICP was higher in the patients with fibrosing alveolitis, sarcoidosis and asbestosis than in the controls. PIIINP in BALF correlated with BALF-PICP, serum angiotensin-converting enzyme (S-ACE), interleukin 2-receptor, BALF-albumin and BALF-lymphocytes and BALF-PICP had a significant correlation with BALF-albumin and BALF-lymphocytes in sarcoidosis. BALF/ELF-PICP had an inverse correlation with the specific diffusion coefficient (DLCO/VA) in fibrosing alveolitis. Both PIIINP and PICP were higher in ELF than in serum in sarcoidosis and fibrosing alveolitis and PICP was higher in ELF compared to serum in asbestosis, suggesting active local synthesis in the lower respiratory tract. The levels of PIIINP in BALF were significantly elevated in sarcoidosis patients with parenchymal involvement compared to those without. Detectable PIIINP in BALF also predicted a poor outcome in fibrosing alveolitis. BALF-PIIINP reflected the disease activity based on chest radiographs in sarcoidosis and a poor prognosis in fibrosing alveolitis, whereas BALF-PICP marked the development of fibrosis.
In lung biopsy specimens, type I and III pN-collagens were increased in fibrosing alveolitis and sarcoidosis. Type I pN-collagen was expressed in areas with damaged or deficient alveolar epithelium. Type III pN-collagen was present underneath regenerative, metaplastic alveolar and bronchiolar type epithelium and was accumulated both in the loose, newly formed fibrosis and in the denser old fibrosis. Type I procollagen was present in intracellular spots in newly formed fibrosis. In sarcoidosis, type I procollagen was present intracellularly in granulomas, whereas type III pN-collagen was expressed extracellularly around granulomas.

Estudos recentes sobre disfunção ventricular demonstram o potencial terapêutico da modulação da matriz extracelular. Isso se dá pela influência que a referida matriz tem sobre a função sistólica e a diastólica do coração. Outros estudos demonstram a influência do digital sobre os sistemas neurohormonais desbalanceados no cenário de disfunção ventricular levantando uma questão acerca do potencial do digital como modulador da deposição do colágeno intersticial e perivascular miocárdico. Sabendo-se da importância prognóstica que a concentração do colágeno no referido cenário tem, a literatura apresenta uma lacuna de conhecimento. Objetivo: Avaliar o papel do digital no remodelamento miocárdico em um modelo experimental. Material e Métodos: 60 ratos Wistar foram separados em 3 grupos de 20. Um grupo controle (GC); outro grupo submetido ao modelo experimental de uninefrectomia, administração de água de beber com 1% de NaCl e de aldosterona subcutânea (GA); e outro grupo submetido ao mesmo modelo experimental, mas também recebendo digitoxina na ração de comer na dose de 100 g/Kg/dia (GAD). Resultados: A fração de volume de colágeno intersticial e perivascular mostrou-se maior no GA comparado aos outros dois grupos (GC e GAD). O índice de desempenho miocárdico mostrou diferença estatisticamente significativa entre o GA (0,49 ± 0,08) e o GC (0,32 ± 0,06) e o GAD (0,4 ± 0,13) (p=0,001). Os níveis séricos de BNP mostraram diferença estatisticamente significativa entre o GA (1,07 ± 0,32 ng/ml) e o GC (0,75 ± 0,19 ng/ml) e o GAD (0,84 ± 0,21 ng/ml) (p=0,01). Os níveis de metaloproteinases não diferiram entre os grupos. Houve uma correlação positiva entre uma maior fração de encurtamento e menores níveis séricos de BNP no GAD. Conclusões: Esses dados demonstram que a digitoxina teve efeito reduzindo a deposição de colágeno intersticial e perivascular e melhorando a função cardíaca avaliada pelo BNP e IDM nesse modelo experimental / Recent studies on myocardial dysfunction are highlighting the therapeutic potential of the myocardial extracellular matrix management. Its interesting to highlight the importance of the dynamics of the cardiac extracellular matrix, because even the systolic and diastolic functions are implicated on it. Other studies showed that digital compounds may regulates the neuroendocrin misbalance due to myocardial impairment and influencing these systems the digital compounds may regulates the interstitial collagen deposition. Objective: To evaluate the role of the digital on a myocardial fibrosis in an experimental model, examining if the digital is able to prevent the collagen deposition. Methods: The sample was divided in 20 rats from the control group (CG); 20 rats submitted to a fibrosis experimental model in which the rats are uninefrectomized, drink water with 1% NaCl during the protocol and receive aldosterone through an osmotic minipump (AG); and 20 rats submitted to the same experimental model treated with digitoxin in a daily dose of 100 g/Kg (DAG). Results: The interstitial and perivascular collagen volume fraction showed a significant difference between the AG and the other 2 groups (CG and DAG). The myocardial performance index showed a significant difference between the AG (0.49 ± 0.08) and the CG (0.32 ± 0.06) and the DAG (0.40 ± 0.13) (p=0.001). The BNP levels showed a significant difference between the AG (1.07 ± 0.32 ng/ml) and the CG (0.75 ± 0.19 ng/ml) and the DAG (0.84 ± 0.21 ng/ml) (p=0.01). The metalloproteinases levels did not differ among the groups and there was a positive correlation between the shortening fraction and the BNP levels among the GAD animals. Conclusion: These data demonstrate the digitoxin positive effect on the myocardial collagen deposition in this experimental model of interstitial fibrosis and could have a new therapeutic target previously unexplored

Type I and type III collagen metabolites and peritoneal cells in predicting the clinical outcome of epithelial ovarian cancer patientsSimojoki, M. (Marja) 21 January 2003 (has links)
Abstract
Malignant tissue growth induces marked biochemical and structural changes in the extracellular matrix of the tumour and its surrounding tissues. In the present study, we evaluated the prognostic value of the serum concentration of the markers of synthesis of type I collagen (PICP, PINP) and type III collagen (PIIINP) as well as the marker of type I collagen degradation (ICTP) and compared them with the conventional indicators of prognosis (clinical stage, grade of differentiation, histological subtype, residual tumour load and the age of the patient). The prognostic value of peritoneal cytological findings at operation was an additional object in our studies.
High preoperative serum ICTP (>5.6μg/L) and PIIINP (>3.2μg/L) concentrations and a low PICP:PINP ratio (>2) correlated with poor prognosis in ovarian carcinoma in univariate analysis and in multivariate analysis when each variable was analyzed separately with the conventional factors. However, ICTP concentration was the only prognostic variable in multivariate analysis including PIIINP, PINP, ICTP and CA125. When analyzed with conventional prognostic factors (clinical stage, grade, residual tumour, presence of ascites, histology), clinical stage and ICTP were independent indicators of prognosis. In addition, malignant cells in the peritoneal fluid aspirate at primary operation, grade and the age of the patient predicted poor prognosis in multivariate analysis.
Postoperative serum ICTP concentration 9-months after the operation was the strongest prognostic factor as compared to the preoperative ICTP and CA125 values and clinical variables.
These results indicate that serum collagen metabolites, especially ICTP, are indicators of prognosis in epithelial ovarian cancer. The present ICTP-test does not detect the degradation products of immature type I collagen, the dominating form in ovarian cancer tissue. Therefore, the excess ICTP in invasive ovarian cancer might originate through the degradation of trivalently matured collagens in non-malignant tissues surrounding the malignancy. ICTP may thus be an indicator of invasive properties of the tumor and its determination opens up new perspective to predict the clinical outcome of ovarian cancer.

Effects of tobacco on human gingival fibroblastsZhang, Weiping January 2011 (has links)
Indiana University-Purdue University Indianapolis (IUPUI) / The negative heath consequences of smoking are widely recognized, but there are still about 20% of the people in United States using tobacco products. Cigarette smoke condensate (CSC), the particulate matter of cigarette smoke, is comprised of thousands of chemicals (e.g., nicotine). Secondary only to bacterial plaque, cigarette smoking is a major risk factor for periodontal disease. Human gingival fibroblasts (HGFs) are the main cellular component of periodontal connective tissues. During the development of periodontal disease, collagen degradation occurs. Collagen is the major extracellular matrix component of the gingiva. The major extracellular matrix degrading enzymes produced by the HGFs are the matrix metalloproteinases (MMPs). The MMPs are mainly modulated by the tissue inhibitors of metalloproteinases (TIMPs). In this dissertation, three studies aimed at understanding the effects of tobacco on human gingival fibroblasts and their mechanisms have been conducted: the effects of CSC on HGF-mediated collagen degradation; comparison of the effects of CSC on HGFs with that of nicotine; and the combined effects of CSC and bacteria on HGFs.
The cell proliferation of HGFs decreased and cytotoxicity increased in HGFs treated with increasing concentrations of CSC. CSC increased the collagen degrading ability of the HGFs by altering the production and localization of MMPs and TIMPs.
Nicotine is one of the major components and the most pharmacologically active agent in tobacco. The percentage of nicotine in the CSC was 2.4%. CSC (100 µg/ml) increased the collagen degrading ability of the HGFs by affecting membrane associated MMP-2, MMP-14, and TIMP-2, but the level of nicotine in the CSC may only play a limited role in this process.
Porphyromonas gingivalis (P. gingivalis) is an opportunistic pathogen involved in periodontal disease. The combined effects of CSC and P. gingivalis supernatant increased HGF-mediated collagen degradation by destroying the balance between the MMPs and TIMPs at the protein and mRNA levels.
This project demonstrated that tobacco (with or without P. gingivalis) increased HGF mediated collagen degradation, as seen in the periodontal disease, through altering the MMPs and TIMPs.
