Objetivos: O objetivo deste ensaio clínico randomizado foi avaliar o comportamento de restaurações de resina composta bulk-fill em dentes posteriores (classes I e II). Metodologia: Dezessete pacientes que necessitavam de restaurações em pelo menos dois dentes posteriores foram selecionados para participar do estudo. As causas para indicação das restaurações foram: substituição de restauração deficiente de amálgama ou resina composta e/ou lesão de cárie. Em cada paciente foram realizadas duas restaurações. Randomicamente, uma cavidade foi restaurada com a resina nanohíbrida Esthet-X HD (grupo controle) e a outra restaurada com a resina Esthet-X HD associada à resina de baixa contração Surefil SDR Flow (grupo teste). Em ambos os grupos, um sistema adesivo convencional de dois passos (XP Bond) foi utilizado. Todos os pacientes receberam tratamento global para a atividade cariosa. Após um período de quatro anos, a qualidade das restaurações foi avaliada por 1 examinador previamente calibrado, através de uma modificação do método USPHS e através do método FDI. Os dados obtidos foram submetidos à análise estatística pelo teste não-paramétrico Mann-Whitney (p<0,05) e a concordância intra-observador avaliada através de teste Kappa. Resultados: No quarto ano de acompanhamento, 26 dentes (18 restaurações classe I e 8 classe II) foram avaliadas, e não foram observadas diferenças estatisticamente significantes entre as restaurações dos grupos controle e teste para os métodos de avaliação utilizados. Não houve falhas em nenhuma das restaurações ao longo do tempo. Conclusão: A qualidade das restaurações classe I e II com resina bulk-fill foi semelhante à das restaurações com resina nanohíbrida convencional. As resinas compostas são materiais usualmente utilizados em restaurações dentárias diretas de dentes posteriores. O objetivo desse trabalho foi determinar o valor total do custo dos materiais diretos e indiretos utilizados em restaurações de dentes posteriores em resina composta. O cálculo dos custos foi baseado no método de sistema de custeio variável. As repetições foram obtidas de preparos classes I e II em dentes pré-molares e molares artificiais. A lista dos materiais foi obtida por meio de consulta a uma banca de juízes e baseada nos padrões de excelência comprovados na literatura. Os valores dos materiais foram obtidos de uma média dos valores consultados no mercado fornecedor. Os materiais foram quantificados para cada tipo de preparo com uso de balança de precisão. Os dados foram avaliados por estatística descritiva e pelos testes de Mann-Whitney e Kruskal-Wallis. A análise foi feita para os materiais utilizados em biossegurança (Capítulo 1) e para a realização da restauração (Capítulo 2). O custo encontrado para a biossegurança foi de R$ 8,85. Para os demais materiais para restauração de Classe I foi de R$ 8,69 e para a de Classe II de R$ 8,86. Não houve diferença estatística no custo entre os tipos de preparos para restauração, sendo a média total dos materiais de R$ 17,63. Os valores encontrados podem ser utilizados no cálculo do valor final do procedimento restaurador, auxiliando na gestão de serviços odontológicos públicos ou privados / Abstract: The cost of the materials is part of the calculation of the value of dental fees. The composite resins are materials commonly used in direct posterior dental restorations. The aim of this study was to determine the total value of the cost of the direct and indirect materials used in composite resin posterior dental restorations. The calculation of costs was based on the method of variable costing system. The repetitions were obtained from classes I and II cavities in artificial premolars and molars teeth. A list of the materials was obtained by an experts panel and based on the excellence standards established in the literature. The values of the materials were obtained from an average of the values founded in the supplier market. The materials were quantified for each type of cavity with the use of precision balance. The data were assessed by descriptive statistics and by Mann-Whitney and Kruskal-Wallis tests. The analysis was performed for the materials used in biosafety (Chapter 1) and on the restoration (Chapter 2). The founded cost (Brazilian current coin) was R$8.85 for biosafety. The others materials for Class I restorations was R$8.69 and for Class II restorations was R$ 8.86. There was no statistically significant difference in cost between the types of cavities, with an average of total materials of R$ 17.63. Retention of resin restorations by means of enamel etching and by pins

Indiana University-Purdue University Indianapolis (IUPUI) / An investigation was conducted into the effect of acid etching of the enamel and the use of pins on the retention of direct filling resins when used for restoration of fractured incisor teeth. The retention secured by these techniques as related to the cavity design also was studied. Four different cavity preparations were used. The retention of the resin in all four was compared when there was no pretreatment of the enamel, when the enamel was etched by 50 per cent phosphoric acid, and when pins were used for retention. Retention was assessed on the basis of resistance of the restoration to displacement by a lingual force. No significant difference was observed in retention as related to cavity design in the control specimens. In all four cavity preparations, acid etching of the enamel and the use of two retentive pins increased the resistance of the restorations to displacement. (However; when only one "L" shaped retentive pin was employed in Conjunction with a flat incisal preparation the force required to accomplish displacement was no greater than for controls.) The acid etch technique when employed with a cavity preparation that extended 1.7 mm. or more onto the lingual surface of the enamel generally offered a higher resistance to lingual force than double pin retained restorations. There was no significant difference between the resistance offered by a circumferential preparation when the resin was retained by either acid etching or by two pins. In the acid etch technique the enamel surface area and its distribution are important factors in retention.

Pet ownership and its association with the oral health of older adults

OBJECTIVE: To explore the effect of pet ownership (PO) on the oral health of older adults. METHODS: NHANES data 2005-06 was analyzed (logistic/linear regression), restricted to aged 65 years and older. The outcomes of interest were the presence/absence of untreated dental caries, the presence/absence of dental restorations, and mean number of teeth. The primary independent variable was PO status within the past year. Potential confounders included demographic data (age, gender, race, marital status, poverty income ratio, education), smoking status, depression, physical activities, and mean sugar intake, taking into account NHANES complex sampling. RESULTS: Individuals aged 65-69 were more likely to own pets than those older than 70 years, with dogs being the most popular pet (62%) followed by cats (31%). After adjusting for potential confounders, there was no association between pet ownership in seniors and the outcomes of interest. Non-Hispanic blacks reported the highest tooth loss (-6.42 teeth) among the racial groups and were 40% less likely to have a dental restoration. In the unadjusted model Mexican Americans have 2.83 times the odds of untreated dental caries compared to Non-Hispanic whites, while current smokers had 7 fewer teeth than those who never smoked. CONCLUSIONS: Pet ownership was not associated with improved oral health for older adults. Despite the lack of oral health protection, pet ownership provides companionship, reduces loneliness, and increases socialization among older adults.
