BACKGROUND:There is a need for more Comparative Effectiveness Research (CER) on Chinese medicine (CM) to inform clinical and policy decision-making. This document aims to provide consensus advice for the design of CER trials on CM for researchers. It broadly aims to ensure more adequate design and optimal use of resources in generating evidence for CM to inform stakeholder decision-making.METHODS:The Effectiveness Guidance Document (EGD) development was based on multiple consensus procedures (survey, written Delphi rounds, interactive consensus workshop, international expert review). To balance aspects of internal and external validity, multiple stakeholders, including patients, clinicians, researchers and payers were involved in creating this document.RESULTS:Recommendations were developed for "using available data" and "future clinical studies". The recommendations for future trials focus on randomized trials and cover the following areas: designing CER studies, treatments, expertise and setting, outcomes, study design and statistical analyses, economic evaluation, and publication.CONCLUSION:The present EGD provides the first systematic methodological guidance for future CER trials on CM and can be applied to single or multi-component treatments. While CONSORT statements provide guidelines for reporting studies, EGDs provide recommendations for the design of future studies and can contribute to a more strategic use of limited research resources, as well as greater consistency in trial design.

Improving the impact of Australian aid: the role of AusAID's Office of Development Effectiveness

This research is in response to the current debate on aid in Australia. The debate focuses on the volume of money allocated to aid rather than the impact. While Australian aid is still far from the UN commitment of 0.7 per cent of gross national income, this focus has kept public debate superficial and has deflected attention away from the more important discussion: is aid achieving outcomes and impacting positively in areas identified by developing countries as essential for their sustainable development. / The release of the first Annual Review of Development Effectiveness provided the impetus to investigate whether the newly formed Office of Development Effectiveness (ODE) will introduce changes that will improve Australia’s approach to aid. Framed within national interest, development and aid literature, this research analyses what limitations, if any, there are to reform of aid policies and practices in Australia. / The thesis concludes that the potential for the ODE to significantly improve the effectiveness of Australia aid is limited. It is one of many voices – including the powerful national interest agenda furthered by foreign policymakers – shaping Australian aid policy and practice. However, the furthering of Australian national interest – narrowly defined as security and economic considerations – through the aid program is at the expense of poverty alleviation objectives. This negatively affects how the development ‘problem’ is framed and thus the focus of aid policy. Furthermore, efforts to prioritise national interest considerations undermine the adoption of ‘good’ practice essential for sustainable development. / This is a political reality that is unlikely to change. Thus, the role of the ODE is to provide recommendations within this restricted framework. However, it is only through scrutiny, discussion and debate that the discrepancy between ‘good development’ in theory and in practice can be narrowed. This should also be the role of the ODE.

Nowadays, corporations have to consistently advance the value of human assets in their own organizations for maintaining their competitiveness. To achieve this goal, the Human Resource (HR) Dept. plays a decisive role in the aspects of selection, training, employment, and retaining of human resource in the organization. How to improve the efficiency of the HR Dept. and enhance its status in the organizations has become the top agenda to enterprises. The development of Information Technology (IT) transforms the role of the HR Dept. in the organization. It enables HR to be a real strategic partner of corporations through the process of Organization Development. A Human Resource Information System (HRIS) based on the network technology will change the working environment of HR and further lead the transformation of the functions and roles of the HR Dept. Through conducting interviews with 9 companies, this research tries to answer the question, that is, what is the impact to the effectiveness of Human Resource Management when a HRIS is leaded in. It used literature research to analyze the characteristics of Human Resource Management (HRM) and HRIS and sorted out the possible impacts. It also used case study to sharpen the insight by sampling 9 companies which had used HRIS, including foreign and local ones. By doing the work, it drew a conclusion of the impact brought by HRIS and provided a direction of HRIS vendors. The study found that the factors of successfully leading HRIS into a company include the integration with original data, combination with the existing Enterprise Resource Planning System (ERPS), the compatibility of the existing HRIS, HR people¡¦s inability of maintaining HRIS, the suitability of HRIS for company¡¦s specific needs, and the support from high-level management. Leading in a HRIS could benefit the company from decreasing the inconvenience of human operations and increasing the quality of HR¡¦s services. The impacts on the effectiveness of HRM can be seen as the indexes which have positive reaction to HRIS: 1. Reward Management: HRIS provides fast calculation of payment and less human operations. Its tracing function in Payment Module and integration with ERPS help data exchange between HRIS and ERPS. 2. Training Development: HRIS keeps thorough training records of employees. It also can display the average training hours per person, training cost, training results, and etc. 3. Appraisal Management: The whole appraisal process can be done through HRIS, even crossing areas. 4. HR Strategy and Planning: Since HRIS is able to provide more precise data analysis, the quality of decision making of high-level management is enhanced with a great progress. 5. Team Management: Through humanity design of HRIS, more participation from employees is involved. Management spreading in different places still can use HRIS as a platform to exchange HR related information.

A methodology for the evaluation of training effectiveness during early phase defense acquisition

Today's economic environment requires for a greater emphasis to be placed on the development of cost-effective solutions to meet military capability based requirements. The Joint Capabilities Integration and Development System (JCIDS) process is designed to identify materiel and non-materiel solutions to fill defense department capability requirements and gaps. Non-materiel solutions include: Doctrine, Organization, Training, Materiel, Leadership and Education, Personnel, Facilities, and Policy (DOTMLPF-P) changes. JCIDS specifies that all non-materiel solutions be analyzed and recommendations be made accordingly following a capability-based assessment (CBA). Guidance for performing CBA analysis provides minimal information on how to predict training effectiveness and as a result training investments are not properly assessed and considered as a viable alternative. Investigations into the ability to predict versus evaluate training performance and to quantify uncertainty in training system design are two identified gaps in the capability of existing training evaluation methods. To address these issues, a Methodology to Predict and Evaluate the Effectiveness of Training (MPEET) has been developed. To address the gap in predictive capability MPEET uses primary elements of learning theory and instructional design to predict the cost-effectiveness of a training program, and recommends training alternatives based on decision-maker preferences for each of the cost and effectiveness criteria. The use of educational and instructional theory involves developing and ensuring human performance requirements will be met after training. Utility theory is used to derive an overall criterion consisting of both cost and effectiveness attributes. MPEET uses this criterion as a key variable in determining how to properly allocate resources to gain maximum training effectiveness. To address the gap in quantifying uncertainty in training performance, probability theory is used within a modeling and simulation environment to create and evaluate previously deterministic variables. Effectiveness and cost variables are assigned probability distributions that reflect the applicable range of uncertainty. MPEET is a systems engineering based decision-making tool. It enhances the instructional design process, which is rooted in the fields of education and psychology, by adding an objective verification step to determine how well instructional strategies are used in the design of a training program to meet the required learning objectives. A C-130J pilot case study is used to demonstrate the application of MPEET and to show the plausibility of the approach. For the case study, metrics are derived to quantify the requirement for knowledge, skills, and attitudes in the C-130J pilot training system design. Instructional strategies were defined specifically for the C-130J training program. Feasible training alternatives were generated and evaluated for cost and effectiveness. Using information collected from decision-maker preferences for cost and effectiveness variables, a new training program is created and comparisons are made to the original. The case study allows tradeoffs to be performed quantitatively between the variable importance weightings and mean value of the probabilistic variables. Overall, it is demonstrated that MPEET provides the capability to assess the cost and effectiveness of training system design and is an enabler to the inclusion of training as an independent non-materiel alternative solution during the CBA process. Although capability gaps in the defense acquisition process motivated the development of MPEET its applicability extends to any training program following the instructional design process where the assumed constraints are not prohibitive.

Adventure based experiential learning programmes (AEPs) are certainly nothing new (Louw et al., 2007). Its inception can be traced back as far as Plato after which it was revised as a result of the contributions of Kurt Hahn, the German educationalist and organisations such as Outward Bound (Hattie et al., 1997). The positive contribution to personal- and life effectiveness as a result of participation in AEPs and which adventure enthusiasts have laid claim to, has been in existence since the start of Outward Bound in 1942 (Neill, 2000:3). Witt and Crompton (1996) further emphasised value of AEPs by stating that they are internationally recognised as a scientific method that assist in developing the life effectiveness of adolescents and adults. One of the most critical challenges for adventure based experiential learning organisations in the 21st century is the development of applicable AEPs for less privileged groups, according to Neill and Flory (2000a). High school pupils in this day and age pose much bigger challenges to educational institutions. They are more diverse in terms of age, race, socio-economic status, culture, sex, stability, attitudes and values (Upcraft, 1993). To adhere to the above mentioned challenges Kimball and Bacon (1993) recommend AEPs that take place in nature as this serves as a therapy for the gaps in the current educational- and community systems. The purpose of this study was to obtain scientific information regarding the contribution of an adventure based experiential learning program (AEP1) to the improvement of the life- and personal effectiveness of black high school pupils, specifically in light of the current transformation- and empowerment processes. Black high school students were specifically used in this study because no results regarding their participation in AEPs have been found in South Africa. The results of this study will deliver important scientific information regarding this topic, especially in light of the current focus on transformation and empowerment. The test subject consisted of an experimental- (20 boys and 20 girls) and a control group (20 boys and 20 girls) aged between twelve and seventeen years with an average age of 14.5. The experimental group participated in a five day AEP in the Vredefort Dome in the North West province. The study is conducted in the form of a quantitative pre-test post-test design (Thomas & Nelson, 1996:321-322). The questionnaires were administered before, directly after and six months after the initial evaluation to determine the immediate and long term effect thereof. To measure life effectiveness, use was made of the Life Effectiveness Questionnaire (LEQ-H) (Neill et al., 1997:6) with a Cronbach Alpha-value of between 0.78 and 0.93 to measure the development of life effectiveness of the group. The instrument focuses on psychological and behavioural aspects that are key components of life effectiveness. The LEQ-H consists of 24 questions and assesses the following dimensions: achievement motivation, active initiative, emotional control, intellectual flexibility, self confidence, social competence, task leadership and time management. The results yielded an overall intra-group effect size of d= 0.03 in the short term and a negative effect size of d=-0.03 in the long term. The inter-group results, however, yielded an overall small effect size of d=0.35 in the short term and a medium effect size of d=0.49 in the long term. The overall results obtained indicate that an AEP had a significant intergroup short- and long term effect on the life effectiveness of black high school pupils but it did not have a significant intra-group effect. The hypothesis regarding the positive, immediate and long term effect of the AEP on the life effectiveness of black high school pupils was thus rejected. Albeit that the life effectiveness of black high school pupils did not necessarily improve as a result of the AEP, it is important to note that the socio-economic environment as well as the family structures of the participants seems to have had an impact on the outcome of the study. Furthermore, the fact that the questionnaires were not in their home language, could also have impacted on the results. To measure personal effectiveness, use was made of the Review of Personal effectiveness and Locus of Control (ROPELOC) with a Cronbach Alpha-value of between 0.79 and 0.93 (Richards et al., 2002:1-4). The ROPELOC consists of 45 questions and is made up of six major components with sub-constructs namely personal abilities and beliefs (self-confidence, self-efficacy, stress management, open thinking), social abilities (social effectiveness, cooperative teamwork, leadership ability), organisational skills (time management, quality seeking, coping with change) active. The difference can be determined with regard to the main components as well as the underlying subcomponents. The results yielded an overall small intra-group effect size of d=0.28 in the short term and a small effect size of d=0.42 in the long term. The inter-group results yielded an overall medium effect size of d=0.54 in the short term and a large effect size of d=0.87 in the long term. The overall results obtained indicate that an AEP had a significant short- and long term effect on the personal effectiveness of black high school pupils. Measuring uncertainty in economic evaluations : a case study in liver transplantation

It is important to account for all sources of uncertainty when evaluating the clinical or cost-effectiveness of health care technologies. Therefore, this thesis takes as its basis a cost-effectiveness study in liver transplantation and identifies two previously unexplored issues that can arise in clinical and cost-effectiveness studies. A literature review of studies evaluating the effectiveness, costs or cost-effectiveness of solid organ transplantation confirmed that these issues were important and relevant to other transplantation studies. The first issue concerns the selection of an appropriate method for estimating mean study costs in the presence of incomplete (censored) data. Twelve techniques were identified and their accuracy was compared across artificially created mechanisms and levels of censoring. Lin's method with known cost histories and short interval lengths is recommended for accurately estimating mean costs and their uncertainty. It is assumed that these findings are generalisable to any solid organ transplant study where censoring is an issue. The second issue explored in this thesis relates to methods for measuring uncertainty around survival, HRQL and cost estimates derived from prognostic models in the absence of observed data. Probabilistic sensitivity analysis is recommended for measuring prognostic model parameter uncertainty and estimating individual patient outcomes and their uncertainties, as it is able to incorporate the additional uncertainty from using prognostic models to estimate control group outcomes. This thesis shows the quantitative importance of these issues and the methodological guidance offered should enable decision makers to have more confidence in clinical and cost-effectiveness estimates. Providing decision makers with a fuller estimate of the uncertainty around clinical and cost effectiveness estimates will aid them in decisions about the necessity of conducting further research in to the clinical or cost-effectiveness of health care technologies.

The application of the excellence model to enhance military health service delivery and performance excellence.

This article examines the appropriateness of applying the South African Excellence Model for Public Service Performance Excellence in developing a strategy for the South African Military Health Service (SAMHS) to enhance Military Health Service delivery and performance excellence. The need for a strategy to improve Military Health Service delivery and performance excellence was theoretically, empirically and practically informed by different strategic imperatives. The production of the White paper on the Transformation of the Public Service by the South African Department of Public Service and Administration highlighted one of the main reasons for undertaking a study of this nature. The White Paper provided a framework to enable public services to develop performance excellence strategies. The results achieved by the application of the South African Excellence ModeI for Public Service Performance Excellence Self-assessment Questionnaire provide potential benefits for the SAMHS to enhance Military Health Service delivery and performance excellence. The Excellence Model identifies SAMHS strengths and areas for improvement based upon well-established internationally accepted theoretical frameworks and recognised criteria for performance excellence. From this study it is inferred that determining Military Health Service performance excellence continues to be a challenge. It became evident that applying the South African Excellence Model for Public Service Performance Excellence as an integrated self-assessment framework within the SAMHS as a public service health care organisation could be the starting point for a regular strategic planning process within the organisation and could ensure continuous improvement in the performance excellence levels of Military Health Service organisations. / Prof. J Uys
