A mudança de decúbito é um dos cuidados prescritos pelo enfermeiro. Em Unidades de Terapia Intensiva Neurológica e Neurocirúrgica, o enfermeiro deve avaliar os riscos e benefícios para prescrever a mudança de decúbito para o paciente com injúria cerebral grave. O objetivo desse estudo foi construir um guia de boas práticas de mudança de decúbito para pacientes internados em Unidades de Terapia Intensiva acometidos por injúria cerebral grave. Trata-se de dois tipos de pesquisa. A primeira é uma Revisão Integrativa da literatura através da qual foi realizada uma busca ampla nas bases de dados com o objetivo de conhecer os resultados das produções científicas sobre mudança de decúbito no paciente internado em Unidade de Terapia Intensiva acometido por injúria cerebral grave no período de 2010 a 2015. Pesquisou-se nas bases de dados: Literatura Latino-Americana e do Caribe em Ciências da Saúde, Base de dados em Enfermagem, Literatura Internacional em Ciências da Saúde, Scientific Electronic Library Online e WEB OF SCIENCE. A segunda é uma pesquisa de abordagem qualitativa, do tipo exploratório e descritiva realizada através da técnica de grupo focal que possui como referencial teórico a teoria da complexidade. Foram realizados três encontros com oito enfermeiros de uma Unidade de Terapia Intensiva de um hospital público de referência em neurologia e neurocirurgia da grande Florianópolis com o objetivo de conhecer a vivência e experiência profissional dos enfermeiros sobre mudança de decúbito para o paciente que sofreu injúria cerebral grave. Os dados foram analisados de acordo com a técnica de análise focal estratégica. O estudo atendeu aos preceitos éticos da pesquisa com seres humanos de acordo com a Resolução 466/12, sendo aprovado pelo Comitê de à tica em Pesquisa da Universidade Federal de Santa Catarina sob Nº 1.163.099/2015 e da instituição onde foi realizada a pesquisa. Os resultados deste estudo estão apresentados na forma de dois manuscritos, o primeiro intitulado,  Mudança de decúbito no Paciente Acometido por Injúria Cerebral Grave: revisão integrativa da literatura é resultado da Revisão integrativa realizada. O segundo, intitulado,  Mudança de decúbito em pacientes com injúria cerebral grave na perspectiva de enfermeiros intensivistas é resultado do Grupo focal. A metodologia utilizada através da revisão integrativa e dos grupos focais possibilitou uma reflexão sobre a complexidade que envolve a mudança de decúbito para o paciente neurológico e/ou neurocirúrgico e, dessa forma, com base no estudo científico e na opinião e experiência dos enfermeiros, participantes do estudo, foi possível a construção do guia intitulado  Guia Para Mudança de Decúbito em Pacientes com Injúria Cerebral: Uma Construção Sob a Luz da Teoria da Complexidade . O guia de mudança de decúbito irá proporcionar melhora da assistência prestada assim como facilitará a coordenação desse cuidado prescrito pelo enfermeiro, contribuindo para a segurança, conforto e cuidado qualificado ao paciente com injúria cerebral grave.<br> / Abstract : The prescription of nursing care is the responsibility of the nurse and it s essential for the proper care of patients. The change of decubitus is one of the treatment prescribed by a nurse. In Neurological and Neurosurgical Intensive Therapy Units, the nurse must assess the risks and benefits to prescribe the decubitus change for patients with severe brain injury. The aim of this study was to construct a guide of good decubitus change practices to patients in Intensive Therapy Units affected by severe brain injury. Two types of research were conducted. The first is an integrative literature review through which a wide search in the databases was conducted in order to know the results of scientific production on decubitus change of patients hospitalized in the Intensive Therapy Unit affected due to severe brain injury in the period 2010 to 2015. The survey involved the following databases: Literatura Latino-Americana e do Caribe em Ciências da Saúde, Base de dados em Enfermagem, Literatura Internacional em Ciências da Saúde, Scientific Electronic Library Online e WEB OF SCIENCE. The second is a qualitative research of descriptive-exploratory type accomplished through the focus group technique which has the complexity theory as theoretical framework. It was conducted based on three meetings with eight nurses in an Intensive Therapy Unit of a public hospital reference in neurology and neurosurgery at the Greater Florianopolis Area in order to find out the nurses professional experiences about decubitus change for patients who suffered severe brain injury. The data were analyzed according to the strategic focal analysis technique. The study met the ethical precepts of research on human subjects according to the Resolution 466/12, being approved by the Ethics Committee of the Federal University of Santa Catarina under No. 1163099/2015 and by the Institution where the research took place. The results of this study are presented in the form of two manuscripts, the first, entitled "Decubitus Change in Patient Affected by Cerebral Injury: integrative literature review" is the result of the Integrative review. The second, entitled "Decubitus change in patients with severe brain injury from the perspective of intensivist nurses" is result of the focus group. The methodology used by the integrative review and the focus groups enabled a reflection on the complexity that involves the changing of decubitus of the neurological and/or neurosurgical patient and thus, based on scientific studies and the opinion and experiences of nurses that participated in the study, the construction of the guide entitled "Guide to decubitus Change in Patients with Brain Injury: A Construction Under the Light of the Complexity Theory" was possible. The decubitus change guide will provide improved care delivery as well as facilitate the coordination of this care prescribed by the nurse, contributing to the safety, comfort and qualified care to patients with severe brain injury.
