Environmental health work methods and procedures for the surveillance and control of avian influenza in the Eastern Cape Province, South Africa

Avian influenza is an infectious disease of birds caused by the Type A strain of the influenza virus. The disease, which was first identified in Italy more than 100 years ago, occurs worldwide (World Health Organization, 2006a). The current outbreak of the highly pathogenic avian influenza A (H5N1), which began in Southeast Asia in mid-2003, is the largest and most severe on record. Never before in the recorded history of this disease have so many countries been simultaneously affected. Since the last pandemic in 1968-1969, the risk of an influenza pandemic has not been considered greater than at the present time. The importance of intervention strategies had become increasingly evident throughout the world. The World Health Organization provides a generic outline for preparedness plans to assist countries in their preparations to respond to a possible avian influenza pandemic. These guidelines may be modified as the epidemiology of avian influenza evolves. The South African National Department of Health has developed national guidelines in the form of an avian influenza preparedness plan. These draft guidelines do not provide detailed Environmental Health work methods and - procedures for the effective surveillance and control of the disease. The general purpose of this study is to develop a standardised set of Environmental Health work methods and - procedures, which will contribute to the effective surveillance and control of avian influenza in the Eastern Cape province – South Africa. Within the context of the purpose of this study, a qualitative, explorative, descriptive, inductive and deductive research design will be used. The methods of data collection will be documentary research, telephonic as well as in-depth personal interviews. In this study, documentary research will be the primary method of data collection. With a qualitative approach, the researcher will be the human instrument for data analysis. The process of qualitative data analysis will be based on data reduction and interpretation; and will be conducted as an activity simultaneously with data collection, data interpretation and narrative reporting writing.

Environmental health work methods and procedures for the surveillance and control of cholera in the KwaZulu-Natal Province, South Africa

The aim of this study was to develop a standardised set of Environmental-Health work methods and procedures, with the purpose of contributing to the effective surveillance and control of Cholera in the KwaZulu-Natal province, South Africa. The researcher followed a qualitative research design, which was explorative, descriptive, inductive and deductive by nature. The methods of data collection were documentary research and focus-group interviews. A documentary research approach was employed as the primary method of data collection for the study. The researcher has used semi-structured questions to obtain relevant information from the participants in the focus groups. The purpose of using semi-structured questions in the focus groups was to draw on their knowledge and experience of communicable disease surveillance and control in relation to Cholera, as well as to ascertain their views on the role of the Environmental-Health Practitioners in the Communicable - Disease Outbreak- Response Teams at the three different spheres of government. The process of qualitative data analysis was followed. This was based on data reduction and interpretation; and it was conducted as an activity simultaneously with data collection, data interpretation (coding) and narrative writing. The information obtained from the analysed data assisted in the development of the environmentalhealth work methods and procedures for the surveillance and control of Cholera in the KwaZulu-Natal province, South Africa. In this study, firstly, the epidemiology of Cholera was described and analysed against the background of its manifestation in South Africa. Secondly, the national health care system, with specific reference to the place and role of environmental health practitioners at national, provincial and municipal spheres – in relation to Cholera surveillance and control – was analysed. Thirdly, the relevant research that has been done globally has been analysed against the background of the findings of the above, together with work methods and procedures to be used by environmental-health practitioners during the surveillance and control of Cholera in the KwaZulu-Natal Province, South Africa. These include, inter alia: Work methods and procedures for Cholera case investigation; Work methods and procedures for sanitary investigations; and Work methods and procedures for identifying the sources of contamination in environmental waters. Finally, the conclusion and limitations was presented, and appropriate recommendations were made. These include: he need to educate all role - players, on the recent developments in the identification of Vibrio cholerae from environmental surface waters; Communication systems should be developed that allow the Director: Environmental Health to communicate urgent environmental health information directly to the Minister of Health; he training of environmental-health practitioners on detailed work methods and procedures for the surveillance and control of Cholera; and the existing national Cholera control guidelines need to be updated to include relevant environmental health situations when emergencies arise.

Perception of air pollution and its impact on human health in the south urban basin : a community perspective

Recent and past events in the South Durban Basin (SDB) have highlighted the possible connection between perceived air pollution concerns and deteriorating health. The aim of this study is to explore how the SDB community perceives air pollution and if this can be related to some of the health problems that are experienced in these communities. The ultimate aim is to determine whether the respondents connect air pollution to specific illnesses and also how they perceive the actions used to combat air pollution and its negative health impacts. This information is gathered through a questionnaire conducted in various study areas both within and outside the SDB, then compared with demographic variables. This is done to determine if perception is related to direct industrial activity or if there are other factors influencing results. Variables such as race and level of education had little impact on the results. The results showed that areas within the SDB experience a high neighborhood satisfaction despite the current general belief of air pollution being connected to ill health. The control areas outside of the SDB support theories of gender and race and its relationship to air pollution, currently presented by researchers in the perception field. Strong associations’ do exist between general environmental satisfaction and gender. Perception of current legal enforcement is exerting a strong effect on air pollution perception formation. Vitally important is the connection of daily concrete experiences of air pollution with the lack of transparency and communication between industry and communities. It is resulting in the current perception of illness being connected to tangible air pollution. The aim is to further future studies on establishing links between health and air quality. Gaining insight from the study of public risk perceptions based on local knowledge and experience in particular places, will help shape the role of environmental policy and management response systems. / Geography / M.A. (Geography)

Fall out dust monitoring is a useful and cost effective method of providing trend analysis of dust deposition over a period of time. It also provides an indication of the main areas of dust generation and can be conducted for both health and nuisance purposes.The aim of the study was to develop a positioning guideline for fall out dust monitoring equipment in order to optimise the existing fall out dust programme.

An investigation of practices and effects of disposable infant diapers on the environment : a case study of Mashashane Village

The use of disposable infant diapers is an unmanageable challenge in many rural areas due to a lack of disposal equipment and proper disposal methods. Incorrect disposal of used disposable infant diapers causes immediate and long-term negative health, environmental, social and economic impacts on communities. Consequently, this study sought to establish the practices and effects of used diaper disposal in the community of Mashashane Village, Limpopo. Using snowball sampling, households with infants determined participants for this study. The data collection methods were observations and questionnaires. The results showed that most mothers used disposable infant diapers. All respondents indicated that they did not have proper places for disposal and as a result, they disposed of used disposable infant diapers at any convenient place such as near streams, in wetlands and open places. The study, therefore, concluded that disposal of used disposable infant diapers was not appropriately practised in Mashashane Village and that led to environmental, social and economic challenges that require immediate intervention by the municipality. The study recommended that the municipality should encourage proper disposal of used disposable infant diapers through provision of skip bins and waste collection trucks. / Risk and Vulnerability Unit University of Limpopo

Assessment of environmental exposure to air pollution within four neighbourhoods of the Western Cape, South Africa

Thesis (MTech (Environmental Health))--Cape Peninsula University of Technology, 2019. / Background: A recent review on the effects of ambient air pollution on human health in sub-Saharan Africa, specifically calls for an urgent need for more epidemiological studies in developing countries due to a lack of data in these countries. Air pollution information on exposure is important for understanding and addressing its public health impact in developing countries. In many African countries, the spatial distribution of air pollutants has not been quantified even though air pollution is a global public health risk. The main goal of the study was to quantify and compare the seasonal spatial variation of household air pollution in the 4 Western Cape neighbourhoods. Methods: Weekly indoor and outdoor measurements of Particulate Matter (PM2.5), Sulphur dioxide (SO2), Ozone (O3), Carbon monoxide (CO) and Nitrogen dioxide (NO2) were conducted at 127 households in four informal settlement areas (Khayelitsha, Marconi-Beam, Masiphumulele and Oudtshoorn) during one month each in summer and winter. PM2.5 measurements were conducted using Mesa Labs GK2.05 (KTL) cyclone with the GilAir Plus Air Sampling Pump, Gases were measured using Passam passive samplers. Statistical analyses were performed using Stata V12. Simple linear regression was used to evaluate the relationship between continuous exposure levels and the respective predictor variables. These include distance to major roads, bus routes, open grills and waste burning sites. Results: The highest average weekly outdoor PM2.5 and NO2 concentrations for summer were recorded in Milnerton (8.76 µg/m3 and 16.32 µg/m3 respectively). However, the highest average concentrations during winter for PM2.5 were recorded in Oudtshoorn (PM2.5: 16.07 µg/m3), whilst the highest NO2, was recorded in Khayelitsha (NO2: 35.69 µg/m3). SO2 levels were consistently low during both seasons. Noordhoek generally recorded the lowest average levels for all pollutants. Winter average weekly concentrations were generally higher than the levels recorded in summer for all pollutants. In a sub-sample of indoor and outdoor measurements, the results were comparable for PM2.5, NO2 and CO. However, the results of Ozone (O3) showed relatively higher (~10 times) outdoor compared to indoor levels. Linear regression modelling results revealed that significant predictors of elevated exposure to PM2.5 were proximity to construction activities and open grills. Analysis demonstrated a clear dose-response relationship with distance, with open grills within 1000m associated with a 0.33 µg/m3 increase in PM2.5 to 6.77 µg/m3 at a distance of 25 meters. Results from the linear regression modelling revealed that significant predictors of exposure to NO2 were proximity to rapid transport bus stops, bus routes, taxi routes and major routes. Distance to rapid transport bus stops demonstrated an increase in NO2 between 0.09 µg/m3 (at 1km) to 2.16 µg/m3 (at 50m) during summer. A similar pattern was observed for taxi routes and bus routes displaying an increase of 6.26 μg/m3and 6.82 μg/m3 respectively within the proximity of 1000 meters. / MAUERBERGER Foundation Scholarship

In recent years the number of people living in informal or 'squatter' settlements in South Africa has mushroomed and virtually every small town or city has one or more squatter settlements associated with it, often next door to the formal residential areas. Using field data collected from 1993 in two informal settlements in the Eastern Cape Province of South Africa this study examines, firstly, the ways in which men and women in these communities organise their lives in their households and in the wider society. Secondly, it assesses the physical environment of informal settlements where there is a lack of service infrastructure, especially water, sewerage facilities, refuse removal and roads. Also, it was assumed that the presence of large numbers of people in an informal settlement has a deleterious effect on natural resources like the soil, wood, vegetation and water and that this may have a significant contribution to environmental pollution and degradation. An environmental impact perspective of the management, treatment, and disposal of hazardous pharmaceutical compounds generated as medical waste at selected hospitals in Cape Town, South Africa

Thesis (MTech (Environmental Health))--Cape Peninsula University of Technology, 2011. / Pharmaceuticals have been formulated to influence physiological systems in humans, animals, and microbes but have never been considered as potential environmental pollutants by healthcare professionals. The human body is not a barrier to chemicals, but is permeable to it. Thus after performing their in-vivo functions, pharmaceutical compound introduced into the body, exit mainly via urine and faeces. Sewage therefore contains highly complex mixtures of chemicals in various degrees of biological potency. Sewage treatment works including those in South Africa, on the other hand, are known to be inefficient in removing drugs from sewage and consequently either the unmetabolised pharmaceutical compounds or their metabolites emerge in the environment as pollutants via several trajectories. In the environment, the excreted metabolites may even undergo regeneration to the original parent molecule under bacterial influence, resulting in "trans-vivo-pharmaceutical-pollution-cycles". Although all incinerators are known to generate toxins such dioxins and furans from the drugs they incinerate, all the medicines disposed by the hospitals under research, were incinerated, as the preferred option of disposal. The incineration process employed was found to be environmentally unsafe. Expired and unused medicines which the general public discard as municipal solid waste become landfilled. Because many landfill sites are not appropriately engineered, the unwanted drugs landfilled therein, leach into the surrounding ground water, which is the influent source of water treatment plants. Water treatment plants, including those in South Africa, are also inefficient in eliminating pharmaceutical compounds, releasing them in sub-therapeutic concentrations into potable tap water as pollutants, the full effects of which are yet to be determined.

The impact of environmental pollution on public health with specific reference to Sasolburg industrial area, South Africa

The Mining industry in South Africa is viewed as the backbone of the country’s economy, while benefiting from this engine of economic development, its impact on the environment and health has been of a major concern to different stakeholders. The majority of people acknowledge the role of mining in the economy and the country. Others put more emphasis on public health and the environment. The aim of the study was then to assess the impact of environmental pollution and public health on the Sasolburg community from a developmental perspective. The present study can be described as a quantitative descriptive survey that uncovered serious levels of pollution in Sasolburg that had dire health consequences for people involved. The findings of the study indicate that residents live with the constant smell of a variety of chemical pollutants released both by normal production and by periodic incidents. They experience chronic respiratory symptoms, burning eyes, hearing deficiency and skin irritations. The study revealed that environmental pollution consequences affecting residents are inversely related to distance from the mines. The results obtained in this study are evidence that environmental pollution in the Sasolburg area is a definite risk to the health of people living within the surrounding area. Although indicating the need to carry out a comprehensive study, the results call for immediate action to prevent continued public over-exposure to environmental pollution.
