Foram estudados 83 pacientes com esofagite erosiva graus I e II, pela classificação de Savary-Miller modificada, divididos em 3 grupos. Um sem Helicobacter pylori, dois outros com Helicobacter pylori, com e sem o gene cagA. Avaliou-se a participação da bactéria e de seu gene cagA, associados à estudo histopatológico de antro e corpo e à gastrinemia basal, na cicatrização da mucosa do esôfago após tratamento com pantoprazol 40 mg ao dia por 6 semanas. Verificou-se que a presença do Helicobacter pylori, independentemente da presença do gene cagA, facilita a cicatrização esofágica. Indivíduos com gastrinemias maiores também tendem a cicatrizar melhor. Não houve relação do resultado do estudo histopatológico com a resposta terapêutica / Eighty three patients with grade I-II of the modified Savary-Miller classification have been studied. Por padronização aceita internacionalmente, posiciona-se o sensor distal de pH-metria esofágica a 5 cm acima da borda superior do esfíncter inferior do esôfago, identificado por manometria esofágica. Porém, vários autores sugerem técnicas alternativas de posicionamento que prescindem da manometria; dentre essas, destaca-se a da viragem do pH. Esse tema é controverso: alguns autores consideram a técnica da viragem confiável, outros afirmam que não é. Considerando-se a polêmica existente e o pequeno número de trabalhos prospectivos com amostra e metodologia adequadas, idealizamos o presente estudo, que tem por objetivos avaliar a adequação do posicionamento do sensor distal de pH-metria baseado na técnica da viragem, analisando-se a presença, o grau e o tipo de erro de posicionamento que tal técnica proporciona e a influência da posição adotada pelo paciente durante a manobra. Foram estudados, de modo prospectivo, 1031 pacientes encaminhados para realização de pH-metria esofágica. Durante entrevista clínica, foram registrados os dados demográficos e as queixas clínicas apresentadas. Todos pacientes foram submetidos à manometria esofágica para localização do esfíncter inferior do esôfago e à técnica da viragem do pH, que consiste na introdução do sensor de pH-metria na câmara gástrica e na tração gradual do mesmo até que o pH vire para níveis superiores a 4. A identificação do ponto de viragem foi realizada de dois modos distintos, caracterizando dois grupos de estudo: com o paciente sentado (Grupo I - 450 pacientes) e em decúbito dorsal horizontal (Grupo II - 581 pacientes). Após a identificação do ponto de viragem, o sensor distal de pH-metria era posicionado na posição padronizada, baseada na localização manométrica do esfíncter. Registrava-se onde seria posicionado o sensor de pH se fosse adotada a técnica da viragem. Para avaliação da adequação do posicionamento, considerou-se que o erro é representado pela diferença (em centímetros) entre a localização padronizada e a localização que seria adotada caso fosse empregada a técnica da viragem. Considerou-se que o erro seria grosseiro se fosse maior que 2 cm. Analisou-se também o tipo de erro mais freqüente: se acima ou abaixo da posição padronizada. Observou-se que se fosse adotada a técnica da viragem, haveria erro no posicionamento do sensor em 945 pacientes (91,6%). Quanto ao grau de erro, haveria erro grosseiro em 597 (63,2%) casos. Em relação ao tipo de erro, o sensor seria posicionado abaixo do local padronizado em 857 (90,7%) pacientes. Em relação à interferência da posição adotada pelo paciente durante a técnica da viragem, observou-se que não houve diferença significante entre os grupos de estudo, em nenhum dos parâmetros analisados. Conclui-se que: 1. o posicionamento do sensor distal de pH-metria pela técnica da viragem não é confiável; 2. a técnica da viragem proporciona margem de erro expressiva; 3. o tipo de erro mais comum que tal modalidade técnica proporciona é o posicionamento mais distal do sensor, que pode superestimar a ocorrência de refluxo; 4. não há influência da posição adotada pelo paciente durante a realização da técnica da viragem do pH, na eficiência do método / By internationally accepted standardization, the esophageal pH-meter distal sensor is positioned 5 cm above the superior border of the esophagus lower sphincter, identified by esophageal manometry. However, several authors suggest alternative positioning techniques that leave out the manometry; among such techniques, the pH step-up is the one to be pointed out. This subject is controversial; some publications state that the step-up technique is not reliable while some others consider it reliable. Considering the existent controversy and the small number of prospective works with suitable sample and methodology, we have idealized the present study, that aims the evaluation of the suitability of the pH-meter distal sensor positioning based on the step-up technique, by analyzing the presence, the degree and the type of the error of positioning that such technique provides and the influence of the position adopted by the patient during the procedure. 1031 patients conducted to the esophageal pH-meter procedure were studied in a prospective way. During the clinical interview, the demographic data and the presented clinical complaints were registered. All the patients were submitted to both esophageal manometry in order to localize the esophagus lower sphincter and the pH step-up technique, that consists of the introduction of the pH-meter sensor in the gastric chamber and in the sensors gradual traction until the pH steps up to levels over 4. The step-up point was identified by two distinct ways, characterizing two study groups: with the setting patient (Group I - 450 patients) and in supine position (Group I I- 581 patients). After the step-up point identification, the pH-meter distal sensor was placed in the standard position, based on the sphincter manometric placement. It was registered where the pH sensor would be positioned if the step-up technique were adopted. To evaluate the positioning suitability, the error was considered to be represented by the difference (in centimeters) between the suitable placement (manometry) and the one that would be adopted in case the step-up technique were adopted. The positioning error was considered rough if it were larger than 2 cm. The most frequent type of error was also analyzed: if above or below the standard position. It was observed that if the step-up technique were adopted, there would be error in the sensor positioning in 945 patients (91.6%). In terms of error degree, there would be a rough error in 597 (63.2%) cases. Concerning the type of error, the sensor would be positioned below the standard place in 857 (90.7%) patients. As to the interference of the position adopted by the patient during the step-up technique, it was observed that there was no significant difference among the groups of study, in any of the analyzed parameters. It was concluded that: 1. the positioning of the pH-meter distal sensor by the step-up technique is not reliable; 2. the step-up technique provides expressive error margin; 3. the most common type of error that such technique mode provides is the placement of the sensor below the standard positioning, which may overestimate the reflux occurrence; 4. there is no influence in the position adopted by the patient during the pH step-up technique procedure, in terms of method efficiency

INTRODUÇÃO: O cigarro é citado como um possível fator externo que influencia sobre a fisiopatologia e a evolução da doença do refluxo gastroesofágico (DRGE). O cigarro contém uma quantidade significativa de óxido nítrico (NO). Os objetivos deste trabalho foram avaliar, em pacientes fumantes com DRGE erosiva, o papel do NO, pela análise de seus precursores: nitratos salivar (SNO3) e gástrico (GNO3) e nitrito salivar (SNO2), nos resultados de cicatrização após tratamento clínico, bem como comparar esta cicatrização com não-fumantes. MATERIAIS E MÉTODOS: 31 pacientes (grupo GF) adultos fumantes, com sintomas típicos de DRGE e endoscopia digestiva alta (EDA) mostrando esofagite A ou B de Los Angeles e 10 adultos não-fumantes, com mesmas características de DRGE em sintomas e EDA (grupo GNF) realizaram manometria esofágica, pHmetria de 24 horas, dosagens de bicarbonato salivar, dosagens de SNO3, GNO3 e SNO2. Foram tratados com pantoprazol 40 mg/dia por oito semanas e repetiram a EDA. Comparou-se os grupos GF e GNF e também os grupos que cicatrizaram (grupo GC=18 pacientes) e não cicatrizaram (grupo GNC=23 pacientes). RESULTADOS E DISCUSSÃO: A cicatrização da esofagite erosiva ocorreu em dez pacientes (32,2%) em GF e oito pacientes (80%) em GNF (p<0,05). A manometria esofágica não mostrou diferenças estatísticas na avaliação do esfíncter inferior do esôfago (EIE) entre GF e GNF (p=0,517). A pHmetria de 24 horas mostrou maior intensidade de refluxo ácido em fumantes, com diferença estatisticamente significativa entre GF e GNF (p<0,05), com médias em GF: escore de DeMeester: 35,26, porcentagem de tempo com pH<4: 8,67 e tempo total com pH <4: 120,42 no GF, e médias no GNF: escore de DeMeester: 12,53, porcentagem de tempo com pH<4: 2,97 e tempo total com pH <4: 2,97. Bicarbonato salivar médio foi 4,54 uM/L em GF e 2,90 uM/L em GNF (p<0,05), portanto fumantes têm maior depuração esofágica. SNO2 média foi maior em GF (69,60 uM) que GNF (38,60 uM) (p<0,05), que pode estar relacionada à aquisição de NO diretamente da fumaça do cigarro e sua provável oxidação a SNO2 quando entra em contato com bicarbonato salivar (básico). SNO3 média foi 226,88 ?M em GF e 197,02 uM em GNF (p=0,304). GNO3 média foi 134,56 uM em GF e 125,63 uM em GNF (p=0,699). Comparando-se GC e GNC houve diferença estatística (p<0,05) somente em SNO3 (médias - GC: 172,36 uM e GNC: 256,57 uM). Este se mostrou relacionado à manutenção da erosão na mucosa esofágica, grupo (GNC) com 91,3% dos pacientes sendo fumantes. A manometria e a pHmetria tiverem, respectivamente, p=0,935 na avaliação do EIE e p=0,235 para escore de DeMeester, p=0,194 para porcentagem de tempo total com pH<4 e p=0,214 para tempo total com pH<4. The objectives of this work were, in smokers with erosive esophagitis, to evaluate the role of the nitric oxide (NO), carrying out an assessment of its precursors: salivary nitate (SNO3), gastric nitrate (GNO3) and salivary nitrite (SNO2), in the healing after clinical treatment, as well as to compare healing between smokers and non-smokers. MATERIALS AND METHODS: 31 adults smokers (group GS), who had typical GERD symptoms and an upper endoscopy which showed Los Angeles grade A or B esophagitis and ten non-smoker adults patients, with the same GERD symptoms and with similar esophagitis (group GNS), were submitted to esophageal manometry, 24-hour pHmetry, salivary bicarbonate count, and counts of SNO3, GNO3 and SNO2. Then they were treated with pantoprazole 40 mg/day for eight weeks and repeated upper endoscopy. We compared the groups GS and GNS, and the group where healing was observed (group GH=18 patients) with the group with no healing (GNH=23 patients). RESULTS/ DISCUSSION: Erosive esophagitis healing occured in ten patients (32.2%) of the GS and eight patients (80%) of the GNS (p <0.05). Esophageal manometry didn\'t show significantly statistical differences in the evaluation of the lower esophageal sphincter (LES), between GS and GNS (p=0.517). 24-hour pHmetry showed a more intense reflux in smokers, with significantly statistical differences between GS and GNS (p <0.05) in DeMeester score averages of: 35.26, percent of time with pH <4: 8.67 and total time with pH <4: 120.42 in GS, and DeMeester score averages of: 12.53, percent of time with pH <4: 2.97 and total time with pH <4: 2.97 in GNS. Mean salivary bicarbonate was 4.54 uM/L in GS and 2.90 uM/L in GNS (p <0.05), so smokers have more esophageal clearance. Mean SNO2 was higher (69.60 uM) in GS than in GNS (38.60 uM) (p <0.05), that can be related to the acquisition of NO directly from the cigarette smoke and its probable oxidation to SNO2 when it gets in touch with bicarbonate in the saliva (basic). Mean SNO3 was 226.88 uM in GS and 197.02 uM in GNS (p=0.304). Mean GNO3 was 134.56 uM in the GS and 125.63 uM in GNS (p=0.699). When comparing GH and GNH significantly statistical differences were found (p <0.05) only in SNO3 (mean - GH: 172.36 uM and GNH: 256.57 uM). Higher doses of SNO3 were related to the maintenance of the erosion in the esophageal mucuosae, group (GNH) with 91.3% of patients being smokers. Por padronização aceita internacionalmente, posiciona-se o sensor distal de pH-metria esofágica a 5 cm acima da borda superior do esfíncter inferior do esôfago, identificado por manometria esofágica. Porém, vários autores sugerem técnicas alternativas de posicionamento que prescindem da manometria; dentre essas, destaca-se a da viragem do pH. Esse tema é controverso: alguns autores consideram a técnica da viragem confiável, outros afirmam que não é. Considerando-se a polêmica existente e o pequeno número de trabalhos prospectivos com amostra e metodologia adequadas, idealizamos o presente estudo, que tem por objetivos avaliar a adequação do posicionamento do sensor distal de pH-metria baseado na técnica da viragem, analisando-se a presença, o grau e o tipo de erro de posicionamento que tal técnica proporciona e a influência da posição adotada pelo paciente durante a manobra. Foram estudados, de modo prospectivo, 1031 pacientes encaminhados para realização de pH-metria esofágica. Durante entrevista clínica, foram registrados os dados demográficos e as queixas clínicas apresentadas. Todos pacientes foram submetidos à manometria esofágica para localização do esfíncter inferior do esôfago e à técnica da viragem do pH, que consiste na introdução do sensor de pH-metria na câmara gástrica e na tração gradual do mesmo até que o pH vire para níveis superiores a 4. A identificação do ponto de viragem foi realizada de dois modos distintos, caracterizando dois grupos de estudo: com o paciente sentado (Grupo I - 450 pacientes) e em decúbito dorsal horizontal (Grupo II - 581 pacientes). Após a identificação do ponto de viragem, o sensor distal de pH-metria era posicionado na posição padronizada, baseada na localização manométrica do esfíncter. Registrava-se onde seria posicionado o sensor de pH se fosse adotada a técnica da viragem. Para avaliação da adequação do posicionamento, considerou-se que o erro é representado pela diferença (em centímetros) entre a localização padronizada e a localização que seria adotada caso fosse empregada a técnica da viragem. Considerou-se que o erro seria grosseiro se fosse maior que 2 cm. Analisou-se também o tipo de erro mais freqüente: se acima ou abaixo da posição padronizada. Observou-se que se fosse adotada a técnica da viragem, haveria erro no posicionamento do sensor em 945 pacientes (91,6%). Quanto ao grau de erro, haveria erro grosseiro em 597 (63,2%) casos. Em relação ao tipo de erro, o sensor seria posicionado abaixo do local padronizado em 857 (90,7%) pacientes. Em relação à interferência da posição adotada pelo paciente durante a técnica da viragem, observou-se que não houve diferença significante entre os grupos de estudo, em nenhum dos parâmetros analisados. Conclui-se que: 1. o posicionamento do sensor distal de pH-metria pela técnica da viragem não é confiável; 2. a técnica da viragem proporciona margem de erro expressiva; 3. o tipo de erro mais comum que tal modalidade técnica proporciona é o posicionamento mais distal do sensor, que pode superestimar a ocorrência de refluxo; 4. não há influência da posição adotada pelo paciente durante a realização da técnica da viragem do pH, na eficiência do método / By internationally accepted standardization, the esophageal pH-meter distal sensor is positioned 5 cm above the superior border of the esophagus lower sphincter, identified by esophageal manometry. However, several authors suggest alternative positioning techniques that leave out the manometry; among such techniques, the pH step-up is the one to be pointed out. This subject is controversial; some publications state that the step-up technique is not reliable while some others consider it reliable. Considering the existent controversy and the small number of prospective works with suitable sample and methodology, we have idealized the present study, that aims the evaluation of the suitability of the pH-meter distal sensor positioning based on the step-up technique, by analyzing the presence, the degree and the type of the error of positioning that such technique provides and the influence of the position adopted by the patient during the procedure. 1031 patients conducted to the esophageal pH-meter procedure were studied in a prospective way. During the clinical interview, the demographic data and the presented clinical complaints were registered. All the patients were submitted to both esophageal manometry in order to localize the esophagus lower sphincter and the pH step-up technique, that consists of the introduction of the pH-meter sensor in the gastric chamber and in the sensors gradual traction until the pH steps up to levels over 4. The step-up point was identified by two distinct ways, characterizing two study groups: with the setting patient (Group I - 450 patients) and in supine position (Group I I- 581 patients). After the step-up point identification, the pH-meter distal sensor was placed in the standard position, based on the sphincter manometric placement. It was registered where the pH sensor would be positioned if the step-up technique were adopted. To evaluate the positioning suitability, the error was considered to be represented by the difference (in centimeters) between the suitable placement (manometry) and the one that would be adopted in case the step-up technique were adopted. The positioning error was considered rough if it were larger than 2 cm. The most frequent type of error was also analyzed: if above or below the standard position. It was observed that if the step-up technique were adopted, there would be error in the sensor positioning in 945 patients (91.6%). In terms of error degree, there would be a rough error in 597 (63.2%) cases. Concerning the type of error, the sensor would be positioned below the standard place in 857 (90.7%) patients. As to the interference of the position adopted by the patient during the step-up technique, it was observed that there was no significant difference among the groups of study, in any of the analyzed parameters. It was concluded that: 1. the positioning of the pH-meter distal sensor by the step-up technique is not reliable; 2. the step-up technique provides expressive error margin; 3. the most common type of error that such technique mode provides is the placement of the sensor below the standard positioning, which may overestimate the reflux occurrence; 4. there is no influence in the position adopted by the patient during the pH step-up technique procedure, in terms of method efficiency

Avaliação da aplicação clínica da coagulação com plasma de argônio na ablação do esôfago de Barrett / A clinical evaluation of argon plasma coagulation in Barrett´s esophagus mucosal ablation therapy

O objetivo deste trabalho foi avaliar a aplicação clínica e a efetividade da coagulação com plasma de argônio(CPA) usada para realizar a ablação do esôfago de Barrett. A presença desta moléstia é considerada uma condição pré maligna com potencial para o desenvolvimento do adenocarcinoma de esôfago. Um estudo clínico prospectivo foi realizado com um grupo de 30 pacientes portadores de esôfago de Barrett, diagnosticado por meio de endoscopia digestiva alta e histologia. 25 pacientes eram do sexo masculino com idade variando entre 12 e 72 anos (média = 47,1) e 5 pacientes eram do sexo feminino cuja idade variou entre 45 e 60( média=49.9). Os pacientes eram submetidos à cirurgia antirefluxo e depois encaminhados para o tratamento com a CPA. O tratamento era realizado sob sedação em regime ambulatorial. Os pacientes eram submetidos a sessões com intervalos de 30 dias até obter o desaparecimento completo da lesão à endoscopia. Após três meses eram realizadas biópsias para o controle de cura. O tamanho médio da área aplicada mostrou correlação com a sintomatologia e o aparecimento de complicações.Uma área maior que quatro cm correlacionou-se positivamente com estas variáveis. O número médio de sessões foi de 1 a 6 (média=2,1). O tempo médio de seguimento variou entre 29 dias a 4 anos com média de 1 ano e 4 meses. Em dois casos houve presença de epitélio de Barrett sob o neo revestimento que foi tratado novamente com bom resultado. As complicações ocorreram em 2 casos (6,6%) de estenose de esôfago e um caso(3,3%) de pneumo mediastino todas tratadas com bons resultados. O índice de sucesso com ablação total foi de 93,4%. Não houve mortalidade neste estudo. / The aim of this study is to determine the effectiveness of the Argon Plasma Coagulation (APC) in ablation therapy of specialized columnar epithelium in Barrett´s esophagus. The presence of Barrett´s epithelium is considered a premalignant condition with potential development of adenocarcinoma. The incidence of esophageal adenocarcinoma has been rising for the past 3 decades A prospective study performed with a group of 30 patients(25 men/5 women) with 47 years mean age (range 12 to72) for the men and 49,9 years mean age ( range 45 to 60) for the women presented with Barrett´s esophagus demonstrated by endoscopy and histology.They were referred to us after antireflux surgery and were assimptomatic at the beginning of the study. Application of APC was carried out under sedation (midazolan and meperidine) with a gas flow of 2,5 l/min & 70 w. The Barrett\'s epithelium was coagulated from the most proximal to the gastrointestinal junction to the most distal limit. The maximum size treated each time was 4 cm long. Special care was taken on coagulation of the visible small islands of remaining intestinal metaplasia tissue. The treatment performed in large areas, bigger than 4 cm led to the increasing rate of complications and transient symptoms. The patients returned monthly to new session until complete ablation of the Barrett´s esophagus was showed by endoscopy. Then, 3 months later after that, a extensive random biopsies were taken to histology search for Barrett\'s. The mean number of APC sessions was 2,1 (range 1 to 6). In two cases, endoscopy showed absence of intestinal metaplasia but histology shows small islands of intestinal metaplasia under the neosquamous epithelium. Two(6,6%) esophageal stenosis and one (3,3%) pneumomediastin case have been occurred. The follow up ranged to 27days to 4 years (mean one year and four months). Reepitelialization with squamous epithelium indistinguishable of the esophageal mucosa was demonstrated by endoscopy in 93,4%. There has been no relapse or evidence of the development of dysplasia. No deaths or major complication occurred in this study.
