Evaluation of a drug-drug interaction: fax alert intervention program

BACKGROUND:Clinicians often encounter information about drug-drug interactions (DDIs) during clinical practice. This information is found within product information (hardcopy and electronic) and various electronic systems. Prescribers may receive medication-related communications in practice that are distributed by facsimile (fax), mail, or telephone from pharmacies and pharmacy benefit managers (PBMs). The purpose of this study was to determine if near-real time fax alerts for potential drug-drug interactions (PDDIs) would influence prescribing.METHODS:A prospective study, in cooperation with a pharmacy benefit manager (PBM), was conducted targeting 18 clinically important PDDIs. Fax alerts included an individualized letter to the prescriber with a list of the interacting drugs, PDDI evidence summaries with citations, and recommended clinical management strategies. Among the 18 PDDIs, 13 PDDIs could be assessed for prescription therapy changes using pharmacy claims data. A prospective cohort design was used to evaluate changes in prescription dispensing 90-days following a PDDI fax alert.RESULTS:A total of 8,075 fax alerts were sent to prescribers and there were 4,712 alerts for the 13 PDDIs that could be assessed for change using pharmacy claims data. There were 2,019 patients (interventions) for which fax alerts were sent to their prescribers who were matched with a control group consisting of patients with the same PDDIs but for whom no fax alert was sent. Overall, this study found 154 (7.6%) of patients in the fax alert group compared to 132 (6.5%) in the control group had changes in therapy (p=0.177).CONCLUSIONS:This fax alert intervention program observed no statistically significant differences in prescribing with a fax alert compared to the control group. If PBMs chose to send individualized, evidence-based information to clinicians regarding drug-drug interactions, this study suggests it may not be an effective intervention to mitigate harm.

Οι τρόποι κωδικοποίησης κειμένου για Fax καθορίζονται από το πρωτόκολλο «Facsimile Coding Schemes and Coding Control Functions for Group 4 Facsimile Apparatus» ITU-T Recommendation T.6 και το στάνταρντ JBIG2. Στη διπλωματική αναλύονται όλες οι λεπτομέρειες του στάνταρντ JBIG2, πολλές από τις οποίες δεν είναι σαφώς καθορισμένες στο στάνταρντ. Περιλαμβάνονται οι μέθοδοι για κωδικοποίηση κειμένου και bi-level (binary) εικόνων. Αναφέρονται οι σχετικές μεθοδολογίες, όπως η μέθοδος αναγνώρισης προτύπων και αντικατάστασης (pattern-matching and substitution, PM&S). / The word coding methods for fax applications are defined with the use of «Facsimile Coding Schemes and Coding Control Functions for Group 4 Facsimile Apparatus» ITU-T Recommendation T.6 protocol and JBIG2 standard. All details concerning the JBIG2 standard are thoroughly analyzed throughout this thesis, many of which are not clearly defined in the standard itself. Methods for word and bi-level images coding are included. We also focus on techniques like the pattern-matching and substitution, PM&S method.

The CitySurfer is equipped with a high-resolution display along with buttons and a joystick that allows the user to view and navigate through various types of information received over DARC. </p><p>Sectra Wireless Technologies AB has, among other services, also developed a paging system that enables personal message transmission over DARC. The background of this thesis is a wish to be able to send fax documents using the paging system and to be able to view received fax documents in the CitySurfer. </p><p>The presented solution is a central PC-based fax server. The fax server is responsible for receiving standard fax transmissions and converting the fax documents before redirecting them to the right receiver in the DARC network. The topics discussed in this thesis are fax document routing, fax document conversion and fax server system design.</p>

Mit HylaFAX+ wurde eine gute Grundlage für eine neue Faxlösung an der Technischen Universität Chemnitz gefunden. Durch die vielen Schnittstellen, die HylaFAX+ bietet, konnte es um fehlende Funktionen erweitert werden, sodass es den gestellten Anforderungen entspricht. Auf Basis dieser Arbeit, kann unter Berücksichtigung der Richtlinien des URZ, eine neue Faxlösung implementiert werden, welche den XMediusFAX-Server ablöst. Das FoIP-Gateway nimmt E-Mails entgegen und verarbeitet diese inklusive Anhang, dieser kann eine PDF, ein Word Dokument oder ein Bilddokument in den Formaten JPEG oder PNG sein. Bevor das Fax abgesendet wird, setzt ein Skript die richtige Faxnummer des Absenders ein und prüft ob der Absender die nötigen Rechte hat um ein Fax intern/national/international zu versenden. Die eigentliche Übertragung des Faxes erfolgt über das T.38 Protokoll, sollte der Empfänger dieses nicht beherrschen, wechselt die Faxlösung automatisch auf die Pass-Through Methode. Ist das Fax erfolgreich übertragen, erhält der Sender eine Sendebestätigung per E-Mail oder Ausdruck. Wird eine Faxnachricht empfangen, wird noch vor während der Rufannahme geprüft ob der Empfänger innerhalb des Universitätsnetzes existiert. Falls es die Empfängernummer nicht gibt, wird der Anruf augenblicklich beendet. Ist die Faxnummer einem Nutzer zugeordnet, ermittelt ein Pythonskript die E-Mailadresse des Empfängers. Dieser erhält die Faxnachricht anschließend als PDF im Anhang einer E-Mail. Sollte im LDAP ein Drucker hinterlegt sein bekommt der Nutzer einen Ausdruck auf dem eingestellten Drucker.
