Radiographic Union Score for Tibia (RUST) scoring system in adult diaphyseal femoral fractures treated with intramedullary nailing: an assessment of interobserver and intraobserver reliability

Objectives The Radiographic Union Score for Tibia (RUST) scoring system has been validated in multiple studies assessing the healing of tibial fractures. Our objective was to assess the inter and intraobserver reliability for the RUST in diaphyseal femoral fractures treated with intramedullary (IM) nailing. Patients and Methods A total of 60 sets of anteroposterior (AP) and lateral radiographs of diaphyseal femoral fractures treated by reamed IM nailing were randomly selected from a prospectively collected database. The 60 sets of radiographs were then scored by three reviewers using the RUST system. Interobserver reliability was measured at initial scoring. The 60 sets of radiographs were scored again by the three reviewers to calculate the intraobserver reliability. Results The RUST scores ranged from 4 to 12 with a mean score of 11.3 ± 1.3. The interobserver intraclass correlation coefficient (ICC) was 0.87 (95% CI, 0.81-0.92) and the intraobserver ICC was 0.91 (95% CI, 0.88-0.94), which indicated excellent agreement. Conclusion This study demonstrated that the RUST system can be used reliably in the assessment of healing in diaphyseal femur fractures treated by reamed intramedullary nailing, with excellent interobserver and intraobserver reliability.

As fraturas do fêmur proximal são frequentes em idosos, consideradas uma devastadora consequência da osteoporose, acometem mais o sexo feminino, estão comumente associadas a traumas de pequena energia e, embora apresentem poucas complicações referentes a consolidação no caso das fraturas extracapsulares, estão relacionadas a déficits funcionais, a elevadas taxas de morbidade e mortalidade, além dos altos custos aos serviços de saúde. Representam importante problema de saúde pública mundial devido a sua alta incidência. Trata-se de um estudo retrospectivo com base em um levantamento de prontuários, objetivando caracterizar os pacientes com fraturas do fêmur proximal atendidos na Santa Casa de Misericórdia de Batatais SP. Foram analisados 258 prontuários, de 258 pacientes, correspondendo a 260 fraturas do fêmur proximal (duas fraturas bilaterais) de indivíduos atendidos na Santa Casa de Misericórdia de Batatais no período de janeiro de 2007 a dezembro de 2010 com idade média de 74,8 anos, com predomínio de fraturas na faixa etária entre 80 e 89 anos (35,8%), mulheres (64,6%) e que se declaravam brancos (73%). As fraturas trocantéricas foram as mais incidentes com 60% dos casos, o mecanismo de trauma mais comum foi a queda da própria altura com 91,1% e o lado mais acometido foi o direito com 51,9%. O tratamento cirúrgico foi aplicado a 95,7% das fraturas do fêmur proximal. Dos 258 pacientes incluídos no estudo, 50 foram a óbito, sendo frequente entre as mulheres (61,6%) e o período mais incidente foi com três meses após a fratura. / Proximal femur fractures (PFFs) are common among the elderly. Rendered as a devastating consequence of osteoporosis, women are the most affected. PFFs are commonly associated with low-energy traumas, and, although they have few complications related to consolidation in the case of extracapsular fractures, they are related to functional deficits, high morbidity and mortality rates, besides being costly to health services. Due to the high incidence, PFFs are considered a relevant global public health issue. This is a retrospective study based on a survey of patient records with the objective to characterize the patients with proximal femur fractures seen at the Santa Casa de Misericórdia de Batatais Hospital São Paulo. A total of 258 patient records were analyzed, corresponding to 260 proximal femur fractures (two bilateral fractures) of individuals seen at Santa Casa de Misericórdia de Batatais between January of 2007 and December 2010. Most subjects were women (64.6%) with a mean age of 74.8 years, most in the age group between 80 and 89 years (35.8%), and self-declared being white (73%). Displaced intracapsular neck of femur fractures: dislocation rate after total hip arthroplasty

Includes bibliographical references / Background: Dislocation is one of the most common orthopaedic complications after primary total hip arthroplasty (THA). The reported dislocation rate in elective THR is 5 - 8%. This number increases up to 22% for THA done for neck of femur fractures. Larger femoral head sizes increase the head-neck ratio and range of motion before impingement, therefore reducing the dislocation rate. Due to the reported increase in dislocation for trauma, some surgeons prefer to do a hemi-arthroplasty or open reduction and internal fixation (ORIF). Methods: A retrospective review of all THA done for neck of femur fractures during 2006 - 2012 was undertaken at a large referral hospital. Records were reviewed for patient related and surgical risk factors. We excluded all pathological fractures, extra-capsular fractures and failed ORIF. Results: A total of 96 cases were identified as suitable for analysis. Average age at surgery was 73.2 years (range 30 - 81). Delay to surgery was 5.3 days (range 1 - 63). Average follow up period was 18.3 months (range 3 months - 4.3years). Four patients (4.3%) had a confirmed dislocation. The four patients who had confirmed dislocation had the following characteristics, 28 mm femoral head size, age over 60 years, 2 posterior approaches and 3 females, although not statistically significant. Conclusion: The outcomes of THR in patients with neck of femur fractures can be favourable and provide good long-term prosthesis survival. We report on low dislocation rate post total hip replacement for intra-capsular neck of femur fractures.

Uso de DHS - dynamic hip screw - em fraturas instáveis do fêmur proximal : uma abordagem comparativa entre as deformações no fêmur normal e no fêmur com DHS

O estudo fez comparações entre a intensidade e a distribuição das deformações a que fica submetido o fêmur proximal intacto e o fêmur proximal fraturado e implantado com o sistema parafuso deslizante Dynamic Hip Screw – DHS, em fraturas transtrocantéricas do tipo 31 - A2 – 2. Para isso, foram produzidas deformações, mediante carregamentos cíclicos, em fadiga. As amostras constituíram-se de ossos de fêmur sintéticos, intactos, e de ossos de fêmur sintéticos onde foram feitos cortes em laboratório produzindo situações de fratura do tipo 31 –A2.2. Nas amostras fraturadas foram colocadas as placas DHS simulando situações reais de osteossíntese. Os conjuntos de ossos mais placas foram então submetidos a ensaios de fadiga. Os resultados demonstraram que as presenças da fratura e do implante DHS alteraram a amplitude das deformações em todas as regiões do fêmur. Houve absorção da carga pelo implante DHS em todas as regiões de medição. Na região do colo femoral, o DHS absorveu 98% da carga, na região lateral 97%, no centro medial transversal 48%, no centro medial longitudinal 92%, no centro medial a 45 graus 94%, no calcar, antes da linha da fratura, 80%, na região do calcar, depois da linha da fratura, 81%, na região do segundo parafuso cortical transversal 81%, na região do segundo parafuso cortical longitudinal 73% e na região do segundo parafuso cortical a 45 graus 92%. A região do centro do pino guia e a região do segundo parafuso cortical foram as regiões de maior instabilidade mecânica. Para as condições de fratura do tipo 31 – A2.2 o implante do tipo DHS não absorve totalmente as deformações geradas, apresentando mobilidade do foco da fratura.
