Postoperativt välmående efter överviktskirurgi

Introduktion: Depression, ångest, stress och ätstörningsproblematik har visat sig ha en hög prevalens hos personer som lider av obesitas. Syfte: Att undersöka om och i så fall hur ångest och depression mätt i HADS förändras i en grupp som genomgått överviktskirurgi. Av intresse var också att undersöka om viktnedgång samt om preoperativa symtom på stress, hetsätning och känslomässigt ätande predicerar utfallet av symtom på ångest och depression postoperativt. Metod: 30 patienter som genomgått överviktskirurgi på Akademiska sjukhuset i Uppsala under våren 2011 inkluderades i studien. Dessa patienter hade innan genomgången operation och vid en uppföljning sex månader postoperativt besvarat olika screeningformulär avseende psykiskt välmående, ätstörningar och stress. Resultat: Preoperativa symtom på stress, hetsätning och känslomässigt ätande predicerar inte utfallet av symtom på ångest och depression sex månader postoperativt. Viktnedgång predicerar inte utfallet av symtom på ångest postoperativt, däremot kan viktnedgång predicera utfallet av symtom på depression sex månader efter operation. Ingen signifikant skillnad fanns mellan pre- och postoperativa symtom på ångest eller depression. Slutsats: I en grupp patienter där symtom på ångest och depression inte varit högt förekommande preoperativt kan det vara svårt att upptäcka förändringar i hur dessa patienter mår. Fler studier behövs för att kunna ge ett utökat professionellt stöd till denna patientgrupp. / Background: People with obesity have a high prevalence of depression, anxiety, stress and binge eating/emotional eating behaviours. Aim: To analyze if and how anxiety and depression measured with the Hospital and Anxiety Depression Scale change in a group of patients who have gone through Gastric Bypass surgery. Of interest was also to study if weight loss and preoperative symptoms in stress, binge- eating and emotional eating behaviours could predict anxiety and depression postoperatively. Method: 30 patients whom all gone through Gastric Bypass surgery in Uppsala University Hospital were included in the study. All patients had answered screening questionnaires about physically wellbeing, eating disorders and stress before surgery and six months postoperatively. Results: Preoperative symptoms of stress, binge eating and emotional eating disorders do not predict anxiety and depression postoperatively. Weight loss does not predict symptoms of anxiety postoperatively, but could predict symptoms of depression six months after Gastric Bypass surgery. There was no statistically significant difference between pre- and postoperative symptoms of anxiety and depression. Conclusion: Changes in wellbeing following Gastric Bypass surgery are difficult to identify in a group of patients where the occurrence of symptoms of anxiety and depression is low preoperatively. Further research is needed to find and develop professional support to this group of patients.

Relationships among tonic and episodic aspects of motivation to eat, gut peptides, and weight before and after bariatric surgery.

yes / Background The interaction between motivation to eat, eating behaviour traits and gut peptides following gastric bypass (GBP) surgery are not fully understood. Setting Danderyd Hospital, Karolinska Institutet, Stockholm, Sweden. Method Appetite and hormone responses to a fixed liquid pre-load were assessed in 12 obese (BMI 45 ± 1.9 kg/m2) participants immediately before, 3 days, 2 months, and 1 year following gastric by-pass (GBP) surgery. Subjective appetite and plasma levels of ghrelin, leptin, insulin and GLP-1 were measured for a 3-hour postprandial period. Eating behaviour traits were also measured using the TFEQR18. Results There was a decrease in TFEQ Emotional Eating (EE) and Uncontrolled Eating (UE) from pre to 1-year post-surgery, but no significant change in Restraint. In addition, there was a reduction in subjective appetite ratings, and alterations in appetite peptides favouring an anorectic response. Pre-surgery EE was significantly related to fasting and AUC ghrelin; UE was associated with AUC desire to eat while there was a significant association between fasting desire to eat and ghrelin (fasting and AUC). 1 year post-surgery, UE was positively related to fasting insulin and Restraint was negatively associated with GLP-1. UE and subjective hunger were positively correlated, while the relationship between desire to eat and ghrelin remained. Conclusion The relationships amongst subjective appetite ratings, eating behaviour traits and appetite peptides in obese patients both before and at one-year post GBP surgery contribute to the reduction in a propensity to over-eat and weight loss.

The effects of obesity and surgically-induced weight loss on exercise ventilation: influence of central adiposity and serum leptin

Truncal adiposity impairs ventilation in obese adults by altering normal ventilatory mechanics. Leptin, an inflammatory adipocytokine, is elevated in obesity and has been shown to alter ventilatory responses to exercise. Leptin’s bioavailability appears to be regulated by its soluble receptor (LRe), which is reduced in obesity. Roux-en-Y gastric bypass surgery (RYGBS) is a weight loss intervention that reduces total fat mass and improves several obesity related co-morbidities including pulmonary dysfunction. The purpose of this study was to first evaluate the differences between ventilatory responses to carbon dioxide (VE/VCO2 slope) during progressive treadmill walking in morbidly obese and normal weight females. Second, we will analyze the relationships between the VE/VCO2 slope, truncal adiposity, serum leptin, and LRe. Lastly, we want to evaluate the changes in the ventilatory responses to exercise (VE/VCO2 slope), truncal adiposity, serum leptin, and LRe 3 months following Roux-en Y gastric bypass surgery. Thirteen obese (OB 37.7 ±11.4 years, 42.0 ± 4.8 kg/m2) and 12 normal weight females (NW 36.1 ±8.0 years, 22.8 ± 1.2 kg/m2) participated in this study. Blood samples for measure of fasting serum leptin and soluble leptin receptor were obtained prior to exercise. Cardiopulmonary variables were measured throughout exercise. Regional adiposity was determined through dual energy x-ray absorptiometry. Truncal adiposity was significantly greater in the obese group than the normal weight group. Serum leptin was greater in the obese group while LRe was lower than the normal weight group. The VE/VCO2 slopes were lower in obese group when compared to the normal weight group. There were no significant group differences in maximal ventilation, tidal volume or respiratory rate. Stepwise regression determined that truncal adiposity accounted for 31.5% of variance in VE/VCO2 slope (R= 0.561, R2 =0.315, p = 0.004). At 3 months post-surgery we observed significant reductions in the obese group in total percentages of fat, truncal adiposity, serum leptin. The soluble leptin receptor was not changed at any measured time point following RYGBS. There were no changes in 3 months post-surgery VE/VCO2 slopes in the obese group. Truncal adiposity, serum leptin and LRe were associated with reduced ventilatory responses to weight bearing exercise (VE/VCO2 slope) in obese females when compared to normal weight females. There were no differences between obese and normal weight females in maximal minute ventilation, tidal volume or respiratory rate. This result suggests that differences in VE/VCO2 slopes may not be entirely from maximal pulmonary capacity. Rather, the differences in VE/VCO2 slope may be attributed to truncal adiposity and its positive relationship with leptin. Elevated leptin in the obese group may indicate a state of central leptin resistance which has been shown to reduce the ventilatory responses to exercise. At 3 months post RYGBS significant reductions in total percent fat, serum leptin, truncal adiposity and BMI were observed. However, despite improvement in fat mass and serum leptin there were no changes in the VE/VCO2 slope and LRe at 3 months post RYGBS. Therefore, it is possible that the improvements in body composition and leptin following RYGBS were not sufficient to increase ventilation responses to weight bearing exercise in obese females.

Dagens samhälle kräver att varje enskild individ måste ta beslut över alla de val som vi ställs inför. När det gäller val som antingen gynnar eller missgynnar vår hälsa är möjligheterna idag oändliga. På grund av detta finns det många som ökar i vikt och därmed kan behöva hjälp med att göra en livsstilsförändring. Syftet med denna studie var att undersöka vilka faktorer som kan ha påverkat och motiverat i valet av att göra en gastric bypass operation, samt att urskilja vilka förändringar som följer efter den typen av livsstilsförändring. Semistrukturerade intervjuer genomfördes där fyra informanter deltog och studiens data analyserades med en kvalitativ innehållsanalys. Resultatet skrevs fram med tre huvudkategorier: Livet innan operationen, Faktorer som påverkat och motiverat till att välja gastric bypass samt Livet efter operationen. Detta visade att informanterna motiverades av deras oro inför den framtida hälsan samt deras förhoppningar om en förändrad framtid, det visade också att deras val påverkades av omgivningen. Sinnesstämningen och den sociala tillhörigheten hade förbättrats efter operationen och livskvaliteten hade ökat. / Today's society requires that each individual has to take decisions on all the choices that we face. When it comes to choices that either benefit or disadvantage our health the possibilities are endless. As a result of this there is a large part of the population that has increased in weight and might need help in making lifestyle changes. The purpose of this study was to investigate the factors that may have influenced and motivated the choice of going through a gastric bypass surgery, and to discern which changes following that kind of lifestyle change. Semi-structured interviews with the four participants were conducted, data were analyzed using qualitative content analysis. The result was partitioned into three main categories: Life before surgery, Factors that influenced and motivated to choose gastric bypass and Life after surgery. The findings displayed that the participants were motivated by their concern for the future health and their hopes for a changed future, the findings also showed that their choice was influenced by the surroundings. The surgery led to an improvement in mood and social life as well as an increase in quality of life for the participants.

Darbo tikslas Įvertinti antro tipo cukrinio diabeto (2 tipo CD) chirurginio gydymo klinikinį ir ekonominį efektyvumą. Uždaviniai : 1. Ištirti visus pacientus operuotus dėl nutukimo ir 2 tipo CD LSMUL KK Chirurgijos klinikoje 2005-2010 metais. 2. Palyginti šių pacientų klinikinę būseną ir gyvenimo kokybę prieš ir praėjus daugiau kaip 1 metams po operacijos. 3. Palyginti 2 tipo CD gydymo išlaidas operuojant ir neoperuojant bei įvertinti skrandžio apylankos operacijos ekonominį efektyvumą. Tyrimo metodika Tyrimui atlikti buvo išduotas Kauno regioninio bioetikos komiteto leidimas Nr. BE-2-59. Atlikta pacientų, kuriems dėl nutukimo ir 2 tipo CD 2005-2010m. Kauno Klinikose buvo atlikta skrandžio apylankos operacija, duomenų retrospektyvinė analizė. Praėjus daugiau kaip 1 metams po operacijos detaliai ištirta pacientų klinikinė būsena. Gyvenimo kokybės ir operacinio gydymo naudingumo įvertinimui taikyti SF-36 klausimynas ir SF-6D konversijos modelis. Ekonominio naudingumo analizės modelis naudotas apskaičiuojant ir lyginant operacinio ir konservatyvaus 2 tipo CD tiesiogines gydymo išlaidas. Duomenys apdoroti ir analizuoti naudojant statistinį duomenų analizės paketą SPSS 15. Patientens upplevelser efter en gastric byass operation : en kvalitativ intervjustudie

Syftet med studien var att beskriva patientens upplevelser efter en gastric bypass operation. En empirisk studie med kvalitativ ansats genomfördes, där intervjuer skedde med sex personer som genomfört en gastric bypass operation. Data analyserades genom kvalitativ innehållsanalys. Under intervjuerna framkom att informanterna upplevde att operationen inte hade någon negativ inverkan på deras vardagliga liv. De upplevde en ökad livskvalitet och förbättrad hälsa som lett till ett mer fysiskt aktivt liv. Viktnedgången tillsammans med omgivningens positiva reaktioner på den nya kroppen hade stärkt deras självförtroende. Den största förändringen informanterna upplevde i det vardagliga livet var deras förändrade matvanor. De beskrev att maten var i fokus och att de planerade sin vardag efter måltiderna. De följder som framkom upplevdes inte som något stort problem, utan viktminskningen och de positiva hälsoupplevelserna dominerade. Informanterna kände sig välinformerade och hade realistiska förväntningar på operationen, samt var förberedda på de konsekvenser som kunde uppstå. Studien visar att den specialiserade vården på överviktskliniker kan bidra till att underlätta tiden efter operationen för patienten. Det framkom att informanterna upplevde en kunskapsbrist om gastric bypass operationer bland sjukvårdspersonal, inom primärvård och slutenvård. För att patienten ska känna trygghet och förtroende är det därför viktigt att öka kunskapen om överviktsoperationer bland hälso- och sjukvårdspersonal.

On the Impact of Bariatric Surgery on Glucose Homeostasis

Obesity has grown to epidemic proportions, and in lack of efficient life-style and medical treatments, the bariatric surgeries are performed in rising numbers. The most common surgery is the Gastric Bypass (GBP) surgery, with the Biliopancreatic diversion with duodenal switch (DS) as an option for the most extreme cases with a BMI>50 kg/m2. In paper I 20 GBP-patients were examined during the first post-operative year regarding the natriuretic peptide, NT-ProBNP, which is secreted from the cardiac ventricles. Levels of NT-ProBNP quickly increased during the first post-surgery week, and later established itself on a higher level than pre-surgery. In paper II we report of 5 patient-cases after GBP-surgery with severe problems with postprandial hypoglycaemia that were successfully treated with GLP-1-analogs. The effect of treatment could be observed both symptomatically and in some cases using continuous glucose measuring systems (CGMS). In paper III three groups of subjects; 15 post-GBP patients, 15 post-DS, and 15 obese controls were examined for three days using CGMS during everyday life. The post-GBP group had high glucose variability as measured by MAGE and CONGA, whereas the post-DS group had low variability. Both post-operative groups exhibited significant time in hypoglycaemia, about 40 and 80 minutes per day <3.3mmol/l and 20 and 40 minutes < 2.8mmol/l, respectively, longer time for DS-group. Remarkably, only about 20% of these hypoglycaemic episodes were accompanied with symptoms. In Paper IV the hypoglycaemia counter regulatory system was investigated; 12 patients were examined before and after GBP-surgery with a stepped hypoglycaemic hyperinsulinemic clamp. The results show a downregulation of symptoms, counter regulatory hormones (glucagon, cortisol, epinephrine, norepinephrine, growth hormone), incretin hormones (GLP-1 and GIP), and sympathetic nervous response. In conclusion patients post bariatric surgery exhibit a downregulated counter regulatory response to hypoglycaemia, accompanied by frequent asymptomatic hypoglycaemic episodes in everyday life. Patients suffering from severe hypoglycaemic episodes can often be treated successfully with GLP-1-analogues.
