A safety analysis of industrial accidents. Accident records of major coal producing countries are analysed to obtain fatal and non-fatal accident rates. Significant factors influencing these rates are identified with efficacy of preventive measures.

A comprehensive study of accident records which have occured in Coal Mining Industries of Europe and U. S. A are analysed. The intention of the research was to establish relationships between the various accidents and prevention methods adopted by each country are evaluated and to assess the impact of industrial legislation in these various countries on accident rate are examined. The study analyses in paricular the fatal accident rate, and major and minor rate. The Major health hazards associated with coal mining are described in detail and discusses together with the Measurement of safety performance and its application in the Safety field. The study also examines the role of human factors in accidents also includes a summaries of fatal and major injury rates for 46 countries. Wire Explosion via Electromagnetic Induction

This research is aimed at exploding a wire via electromagnetic induction, with a preference for obtaining restrike of the exploding wire in a ring shape or otherwise. Literature on both exploding wire and electromagnetic induction are introduced together. A mathematical framework to describe the wire explosion by induction is formulated from first principles using the idea of magnetic flux linkages. The environment in which the experiments took place is described, with reference to matters of laboratory safety and also measurement of transient electrical current and voltage in the wire explosion by induction. The results describe the approaches taken to explode a wire by induction to obtain a plasma conductor. Voltage and current data are displayed and described. Throughout this work, there are long-exposure digital photographic images of the experiments taking place. These contribute to determining the outcome of experiments, and support the conclusions. Wires were exploded by induction in an air-cored mutually coupled coils system, and restrike of those wires was achieved. Electrical characteristics of wire explosion by electromagnetic induction are displayed and discussed based on what is known about straight exploding wires. Future works involving creation of plasma rings, electromagnetic thrust and exploding wires in vacuum are discussed.

Environmental risk assessment for the electricity sector : a case of Swaziland electricity company

The provision of reliable, safe, affordable and accessible power supply is one of the most important cornerstones for economic development, particularly for third world countries. Notwithstanding all known benefits of power supply, there are environmental risks associated with electricity generation, transmission and usage that need to be identified, assessed and managed. These risks range from minor to major risks such as serious bodily harm and fatalities as well as environmental degradation. In order to minimise and control the operational risks, it is key for the sector to ensure that the risks are identified and assessed using appropriate tools and systems to ensure sustainability and safe work environments. This study explored, improved and designed a tool for environmental risk assessment within Swaziland Electricity Company, (SEC) based on clause 4.3.1 of ISO 14001 (2004), Environmental Management System and Occupational Health and Safety Assessment Series, OHSAS 18001 (2007), Occupational Health and Safety System. The previous risk assessment and evaluation tool used in the company ignored existing risk control practices employed and thus did not truly quantify identified risks. The methodology presented in this study focused on the identification of hazards/aspects and risks associated with processes in electricity generation, transmission and distribution. Risks were evaluated and all possible measures to control the identified risks were suggested. The top three significant hazards identified include; exposure to live wires, road hazards and the use of uncertified/ defective/wrong equipment and/or machinery whilst the significant impacts were: soil pollution, natural resource degradation/ depletion, loss of flora and fauna and social impacts. The distribution department was observed to have the most environmental risks when compared to the other departments. The developed tool and data could be used as baseline information by other sectors wishing to implement environment and safety systems. Propuesta para mejorar la aplicación del sistema de gestión de seguridad y salud en el trabajo para reducir la hipoacusia profesional en los trabajadores del área de compactación de una empresa distribuidora de gas natural en Lima Metropolitana

El desarrollo de la presente tesis busco comprobar los errores en la aplicación de la identificación de peligros, evaluación de riesgos y Controles (IPERC), el grado de compromiso del trabajador y la política del sistema de gestión de seguridad y salud en el trabajo, con el objetivo determinar una propuesta para mejorar la aplicación del Sistema de gestión de seguridad y salud en el trabajo para reducir la perdida de la capacidad auditiva denominada “hipoacusia profesional” en los trabajadores del área de compactación de una Empresa Distribuidora de Gas Natural en Lima Metropolitana. Durante el desarrollo se identificó en el área de compactación un porcentaje del 55% de trabajadores con hipoacusia profesional. Para identificar el peligro potencial de esta enfermedad ocupacional se realizó mediciones de la exposición al ruido en los trabajadores, determinando que la contaminación presentada durante sus actividades alcanzaron niveles de 98.90 dB superando los limites máximos permisibles de 85 dB para una jornada de 8 horas (Decreto supremo 357, 2012) convirtiéndose en el mayor riesgo del lugar de trabajo. La propuesta de mejora en el Sistema de gestión de seguridad y salud en el trabajo fue el de proponer una nueva matriz de identificación de peligros, evaluación de riesgos y controles (IPERC) estableciendo nuevos controles para reducir el riesgo por la exposición del ruido, para esto se calculo la protección auditiva adecuada para resguardar la salud del trabajador, proponiendo una doble protección auditiva; se buscó tener un personal capacitado para el puesto proponiendo un programa de certificación de trabajadores del área de compactación. Además de proponer un programa de incentivos para mejorar el compromiso del trabajador que cumpla con los estándares de seguridad y salud en el trabajo. The development of this thesis looking check for errors in the application of hazard identification, risk assessment and controls (IPERC), the degree of commitment and political worker management system safety and health at work, with the aim to determine a proposal for improving the implementation of the management system of health and safety at work to reduce the loss of hearing called "professional hearing loss" workers in the area of compaction of a Natural Gas Distribution Company in Delhi . During development it was identified in the compaction area percentage of 55% of hearing loss professional workers. To identify the potential of this occupational disease risk exposure measurements were performed on workers to noise, pollution determining that presented during their activities reached 98.90 dB levels exceeding the maximum permissible limits of 85 dB for an 8-hour day (decree Supreme 357, 2012) becoming the greatest risk in the workplace. The proposed improvement in the management system of health and safety at work was to propose a new array of hazard identification, risk assessment and controls (IPERC) establishing new controls to reduce the risk of noise exposure to this proper hearing protection was calculated to protect worker health, proposing a double hearing protection; He sought to have qualified for the position proposing a certification program area workers compaction staff. In addition to proposing an incentive program to improve employee engagement that meets the standards of safety and health at work.

Nowadays preservation of safety is one of the most important conditions for consequential air traffic development. That was the reason why the Safety Management System was formed by the ICAO. Thesis´ objective was especially to scrutinize all the aspects relating to implementation of this system. In the first place every component of the system was described, eventually its function. Next point was to draft an implementation plan for small aviation operators. Besides that ways and means of evaluating effectiveness of the system and expected benefits were described. The process of implementation was consulted with several aviation operators and organizations related to civil aviation whether from the Czech Republic or abroad. Their knowledge helped in many ways to make an issue of practical implementation of the Safety Management System clear. Finally it is true to say that the Safety Management System going to be an essential part of every aviation operator in few years.
