Implementace Open Source BI ve zdravotnictví / Implementation of Open Source BI in Healthcare

This diploma thesis deals with implementation of open source Business Intelligence solution Pentaho Community Edition 5.0 (Pentaho CE) in particular healthcare institution and it tries to prove its usability in full operation. In theoretic part there are defined concept and principles of Business Intelligence and specifics BI solution implementation in healthcare. In practical part there are detailed analysis of Pentaho CE and demonstration of usage in healthcare presented by implementation into specific healthcare institution. The main benefit of this thesis is description of implementation Pentaho CE using author's practical experience in healthcare area and advanced knowledge of Pentaho CE. The main goal, the verification of usability in production, is fulfilled in both theoretical and practical parts of this diploma thesis. There are also calculated costs and approximate suggested schedule of main activities. Functionality of Pentaho CE is attested in detailed analysis which is possible to use as guide on its own describing general architecture of Business Intelligence solution Pentaho CE.

It consists of theoretical background, analysis of the current state of the network and certain areas of the hospital. The practical part is devoted to the risk analysis and the design of concrete measures that result in an increase in the information security level. This part also includes an economic evaluation of the design implementation.

Veiksnių, lemiančių šeimos gydytojo pasirinkimą, įvertinimas / Assesment of factors determining the choice of family doctor

Darbo tikslas – įvertinti šeimos gydytojo pasirinkimą ir keitimą lemiančius veiksnius priklausomai nuo įstaigos tipo. Uždaviniai :1. Nustatyti ir įvertinti PSP paslaugų teikėjo pasirinkimą lemiančius veiksnius priklausomai nuo įstaigos tipo. 2. Įvertinti ir palyginti šeimos gydytojo keitimą įtakojančius motyvus priklausomai nuo įstaigos tipo. 3. Įvertinti ir palyginti pacientų elgseną ir pasitenkinimą paslaugomis priklausomai nuo įstaigos tipo. Tyrimo metodika. Tyrimas atliktas 2013m. sausio- kovo mėn. Naudota anoniminė anketinė viešosiose: Prienų rajono PSPC ir Birštono PSPC bei privačiose PSP įstaigose besilankančių pacientų apklausa( n═247). Atsakas dažnumas 81,5 proc. Tyrimo rezultatų statistinė analizė atlikta naudojant SPSS 17.0 ( Statistical Package of Social Scienles ) for Windows statistinių duomenų paketą. Ryšiai tarp požymių vertinti pagal Chi kvadrato ( x2 ) kriterijų. Rodiklių skirtumas laikytas statistiškai reikšmingu, kai p< 0,05. Rezultatai. Pacientų tyrimo rezultatų analizė atskleidė, jog (48 proc.) privačių PSPC ir (46 proc.) viešųjų PSP įstaigų pacientų yra patenkinti šeimos gydytojo teikiamomis paslaugomis. Tyrimo metu nustatyta, kad statistiškai šeimos gydytojo paslaugų vertinimas neturi sąsajų su išsilavinimu, socialiniu statusu ir mokėjimu už paslaugas. Privačiose gydymo įstaigose (40 proc.) respondentų nepasirenka šeimos gydytojo dėl nepatenkinamų paslaugų kokybės, o (75 proc.) viešųjų PSP centrų respondentų - dėl netinkamo šeimos gydytojo... [toliau žr. visą tekstą] / The aim of the study is to assess factors that determine the choice and change of family doctor depending on the type of institution. Objectives: 1. To identify and assess factors that determine the choice of a primary healthcare service provider depending on the type of institution. 2. To assess and compare motives that influence the change of a family doctor depending on the type of institution. 3. To assess and compare patientes behaviour and satisfaction depending on the type of institution. Methods of the research. The research was conducted in between January and March, 2013. An anonymous survey was performed on patients who visit public institutions of Prienai district primary health care and Birštonas primary health care and also private primary health care institutions (n=247). The response in public primary health care institutions was 81.5%. The statistical analysis of the research results was performed using SPSS 17.0 (Statistical Package for Social Sciences). The relationships between the features were assessed using x2 criterion. Results. The analysis of patient research results showed that 48% of patients of private primary healthcare and 46% of public healthcare institutions are satisfied with the services provided by family doctors. There is an increased need for complete medical device traceability in the healthcare industry. The two main reasons are the healthcare industry's global supply chain and decentralised adverse events reporting, where different laws apply for each country and where each country has their own database for incidents without international governance. The idea of improving traceability procedures in the surgical department at Karolinska University Hospital was formed in the light of a near miss event where guidelines regarding incident management of a Single-Use Medical Device (SUMD) were not followed properly. Hence, this thesis project will investigate the issue of finding an effective way to trace SUMDs at Karolinska University Hospital, in order to improve the incident management process and suggest improvements of patient safety at other Swedish hospitals as well. The collection of data consisted of different data sources; observations at the research site and interviews with relevant participants. By employing multiple sources to this study, a more holistic approach could be achieved. In addition to observing the current situation of device registration, it was of importance to ask individuals with competence and different perspectives on the issue of traceability of SUMDs. To answer the research questions, the acquired data was categorized into the different identified cornerstones of traceability of SUMDs. These were registration process, perioperative supply chain and incidents management. Each section was divided into an investigation of the current process, issues and suggested improvements, in order to clearly answer to the research questions. Furthermore, these acquired answers and insights, from observations and interviews, were translated and summarized to form a basis for the results. Based on the data acquisition and compilation from the different perspectives, key findings and themes are presented in the results. The thesis proposal include a visual representation that show the physical flow of a SUMD from the point of being delivered to the hospital by the distributor, through different entities where registration occur, until it is either discarded or saved for incidents reporting. In order to avoid many of the current issues and to realize the acquired suggestions from this thesis, interoperability between the systems within the healthcare organization as well as between the different entities throughout the entire supply chain is an essential part of the solution, which should be further studied.
