Educação permanente interdisciplinar no serviço de emergência em saúde mental

Entre as especialidades psiquiátricas, destacam-se as emergências que envolvem risco de vida ou risco social grave (comportamento violento, surto psicótico, risco de homicídio, tentativa de suicídio, etc.), que necessitam interferências imediatas. As urgências são situações que envolvem riscos menores, mas que necessitam intervenções em curto prazo (comportamento bizarro, agitação aguda, ideação suicida, crises de pânico, outros). O presente estudo foi realizado com objetivo de identificar necessidades de melhorias no atendimento e formar o Programa de Educação Permanente visando à atuação interdisciplinar da equipe. Esta pesquisa foi realizada no Plantão de Emergência em Saúde Mental (PESM) do Pronto Atendimento Cruzeiro do Sul (PACS), localizado no Centro de Saúde Vila dos Comerciários (CSVC), Rua Prof. Manoel Lobato, 151 – andar térreo. Para tanto, foi empregado Questionário e foram formuladas perguntas abertas como instrumentos de levantamento de dados e formação de Grupo Avaliativo, com a participação dos profissionais da equipe do PESM. Por meio da problematização das ações em saúde desenvolvidas no setor, procurou-se identificar as necessidades da especialidade em Saúde Mental e implementar atividades de Educação Permanente para o setor. Entre os resultados, destacam-se as seguintes necessidades: utilização da reunião sistemática do setor para planejamento de ações e proposições dos funcionários ao Núcleo de Educação Permanente; revisão periódica de rotinas; organizar espaços de atividade de Educação Permanente à carga horária do profissional, agregando competência e desse modo, participando do desenvolvimento de políticas públicas capazes de enfrentar as dificuldades da atualidade, a exemplo da epidemia de dependência química na população refletindo nos elevados índices de violência registrados. / Among the psychiatric specialties, there are emergencies that involve life-threatening or serious social risk (violent behavior, psychotic outbreak, homicide risk, suicide attempt, etc.) that require immediate interference. Urgencies are situations that involve minor risks but require short-term interventions (bizarre behavior, acute agitation, suicidal ideation, panic attacks, others). The present study was carried out with the objective of identifying needs for improvement in healthcare and to form the Permanent Education Program aiming the interdisciplinary work of the team. This research was carried out at the Plantão de Emergência em Saúde Mental (PESM) of the Pronto Atendimento Cruzeiro do Sul (PACS), located at the Centro de Saúde Vila dos Comerciários (CSVC), Street Prof. Manoel Lobato, 151 - ground floor. Among the results, the following needs stand out: use of the systematic meeting of the sector for planning actions and propositions of the employees to the Nucleus of Permanent Education; periodic review of routines; to organize spaces of activity of Permanent Education to the workload of the professional, adding competence and in this way, participating in the development of public policies able to face the difficulties of the present, as the epidemic of chemical dependence in the population reflecting in the high rates of violence registered.

The research aims to identify the ways that the multidisciplinary team act and know the stories of these professionals about the situation experienced in the transition between the instituting and instituted on home care modalities. PID has as a prior focus the elderly person in stable medical conditions, not to replace the hospital care, but to offer a therapeutic support turned to the exercise of their autonomy and coexistence with the situation of diseases. The home in their internal coexistence rules preserves own customs. As the hospital care migrates to the home care, it happens in the confrontation and rationality negotiation and becomes something new, that is going to be directed by an instituting dimension. In the view of New History, that suggests an interdisciplinary approach and interprets the problems on its time and from the technique of thematic oral history, it can be seen that working in interdisciplinary team is able to incorporate new values in the way of healthcare assistance, it longs for maintaining the maximum functional capacity of patients, it presents results as the prevention of diseases, costs reduction in connection with the Hospital Service, empowers and expands the possibilities for the patient recovery by aligning with the daily life and the opportunity of the patient being assisted by a multiprofessional team, interacting on the concrete reality. Therefore, PID is in line with the contemporary demands and as an instrument to be considered in the review of a wider concept of the health-disease process / O presente estudo trata de uma an?lise sobre as possibilidades e desafios da migra??o do cuidado em sa?de do hospital para o domic?lio a partir da abordagem sobre o Programa de Interna??o Domiciliar-PID em Natal/RN. Busca identificar as formas de atua??o da equipe multiprofissional e conhecer as hist?rias dos profissionais da equipe sobre a trajet?ria vivenciada na transi??o entre o institu?do e o instituinte nas formas de cuidado domiciliar. O PID tem como foco priorit?rio a pessoa idosa em condi??es cl?nicas est?veis, n?o para substituir a aten??o hospitalar, mas oferecer um suporte terap?utico voltado para o exerc?cio de sua autonomia e conviv?ncia com a situa??o da doen?a presente. The healthcare industry is not left out as its service delivery process involves numerous interfaces, information flows and patient hand-offs among professionals with different educational training, differing knowledge levels and possibly working from different locations as well. As healthcare delivery evolves to being more patient-centered, so does the team settings as well, becoming more collaborative. Such changes also translate into a need for support systems to evolve to be able to provide support for the extent of collaboration that would be needed. A framework is needed to guide in the development of such systems. However, due to the varying needs of patients, team types and make-up would generally differ, so we explored the different types of team settings studying what they entail based on their various degrees of collaboration. We therefore present in this thesis a model of team based concepts, an ontology formalizing the model, team based scenarios designed using the ontology and then application of the scenarios to test the ability of BPMN (Business Process Modeling Notation) to model healthcare teams.
