Como objetivos, pretendeu-se verificar a existência de padrão evolutivo nas válvulas cardíacas destes animais. No total foram utilizados 92 animais, onde foram tomadas medidas morfométricas e contagem das válvulas do coração. O número de fileiras de válvulas variou de 2 a 6, com variação interespecífica. Em sua maioria as primeiras válvulas do cone arterioso são maiores e apresentam a extremidade anterior tricúspide. A análise estatística indicou diferenças morfométricas no coração entre as ordens, assim como diferença significativa no número de válvulas entre as espécies, porém algumas medidas não são significativas e portando não adequadas para uso com fins taxonômicos. Para a ordem Myliobatiforme, o número de válvulas condiz com a posição evolutiva, onde os animais apresentaram a maior quantidade de válvulas. / The present study presents a description of the anatomy of the heart in 9 species of rays belonging to 3 orders, as follows: Rhinobatiformes (Pseudobatos percellens, Pseudobatos horkelii and Zapteryx brevirostris), Rajiformes (Rioraja agassizi, Atlantoraja cyclophora and Gurgesiella dorsalifera) and Myliobatiformes (Dasyatis hypostigma, Rhinoptera bonasus and Mobula thurstoni), all occurring on the continental shelf and slope of Southeastern and Southern Brazil. As objectives, it was intended to verify the existence of an evolutionary pattern in the cardiac valves of these animals. In total, 92 animals were used, where morphometric measurements and heart valve counts were taken. The number of valve ranks ranged from 2 to 6, with interspecific variation. In most cases the first valves of the arterial cone are larger and present a tricuspid anterior extremity. Statistical analysis indicated morphometric differences in the heart between the orders, as well as a significant difference in the number of valves between the species, however some measures are not significant and carrying not suitable for use with taxonomic purposes. For the Myliobatiform order, the number of valves corresponds to the evolutionary position, where the animals presented the largest number of valves.

Platelet function and activation in mixed ancestry subjects with hyperglycemia, from the Western Cape, South Africa

Thesis (MTech (Biomedical Technology))--Cape Peninsula University of Technology, 2016. / Type 2 diabetes mellitus (T2DM) is a metabolic disorder which is characterised by insulin resistance, defective insulin secretion or both. The chronic state of hyperglycemia in diabetes is associated with microvascular complications and macrovascular complications which account for an estimated 80% of deaths as a result of cardiovascular complications. Type 2 diabetes mellitus is an inflammatory disease and pro- inflammatory stimuli in the form of activated endothelial cells, render the vascular endothelial surface attractive for both platelets and leukocytes. Activated platelets bind to the exposed extracellular matrix (ECM) and also to each other in both physiological haemostasis and pathological thrombosis. They can also adhere to leukocytes. Platelet leukocyte interactions are divided into 3 stages; these include the initiation of the interaction, stabilization of the aggregates and amplification of leukocyte activation. This study attempts to contribute to the current knowledge of T2DM by investigating the percentage of monocytes and neutrophils forming aggregates with platelets in pre-diabetes and diabetes and comparing this to non- diabetes individuals as well as the up regulation of pro-thrombotic and activation antigens on the surface of monocytes and neutrophils (Tissue Factor and CD69). Levels of platelet activation and function will be determined by both plate monocyte aggregates (PMA) and platelet neutrophil aggregates (PNA). A total of 124 individuals were recruited from Bellville South, Cape Town, South Africa. This comprised of diabetes (DM) (n=15), pre-diabetes (pre-T2DM) (n=25) and controls (n=84). All individuals were screened for diabetes using the oral glucose tolerance test (OGTT). Platelet leukocyte measurements were performed using the Navios 8-colour flow cytometer. The median percentage of circulating platelets bound to monocytes (%PMAs) was significantly increased in the T2DM 49.04[36.78-62] and the Pre-T2DM 48.96[36.72-61.2],groups, compared to the control group 7.2[5.4-9], p<0.0001. The median %PNAs, which show interactions between neutrophils and platelets, were significantly increased in the T2DM group 13.56[10.17-16.95] compared to the control group 6.01[4.51-7.51] p<0.0001. Platelet monocyte aggregates (PMAs) were higher in both the pre-T2DM and T2DM groups when compared to the control group indicating increased interactions between platelets and monocytes. In addition to forming aggregates with leukocytes, the platelets were able to initiate activation and phenotypic change to the leukocytes by increasing the expression of CD69 and TF (CD142). This finding provides further evidence that there is a link between the inflammatory process and the prothrombotic activity evident in diabetes and pre-diabetes individuals. Lactato sanguíneo em cães com insuficiência cardíaca congestiva decorrente de degeneração valvular mitral /

Orientador: Marlos Gonçalves Sousa / Banca: Aparecido Antonio Camacho / Banca: Patrícia Mendes Pereira / Resumo: Este trabalho teve como objetivo caracterizar os valores sanguíneos de lactato em cães com degeneração valvular mitral em fases distintas da doença e correlacioná-los com as variáveis eletrocardiográficas, radiográficas, ecodopplercardiográficas e de pressão arterial, averiguando o seu valor como indicador diagnóstico da hipoperfusão tecidual em cães com insuficiência cardíaca. Foram avaliados 44 cães, distribuídos em grupo controle, composto por cães hígidos e os grupos Ia, Ib, II e IIIa, compostos por cães com insuficiência cardíaca congestiva classe Ia, Ib, II e IIIa respectivamente. A análise das concentrações séricas de lactato identificou valores estatisticamente superiores nos cães da classe II comparativamente aos animais controle, o mesmo ocorrendo com os animais da classe IIIa comparativamente aos animais controle, classe Ia e classe Ib. Contudo, os cães da classe II apresentaram resultados semelhantes aos animais incluídos na classe Ia e Ib e os cães da classe IIIa não apresentaram quaisquer diferenças significativas com os animais classe II. Houve correlação positiva do lactato sanguíneo e diâmetro interno do ventrículo esquerdo, fração de encurtamento e relação átrio esquerdo/aorta. Da mesma forma, identificou-se correlação positiva entre o lactato sanguíneo e frequência cardíaca, VHS e amplitude da onda P. Foi observada correlação negativa do lactato sanguíneo com a pressão arterial sistólica. Na análise de sensibilidade e especificidade o lactato jugular apresentou a maior área sob a curva ROC, sendo identificada a concentração de lactato sanguíneo de 3,25 mmol/L como a de maior sensibilidade e especificidade para diferenciar cães saudáveis de cães com ICC. Os resultados mostram que o lactato sanguíneo aumenta à medida que progride o quadro de insuficiência cardíaca congestiva / Abstract: This study was aimed at characterizing the concentration of serum lactate in dogs with varying stages mitral valve degeneration, searching for a correlation between such parameter and the electrocardiographic, radiographic, echocardiographic and blood pressure data, as to investigate its value as a diagnostic indicator of tissue hypoperfusion in dogs with heart failure. The analyses of the serum concentration of lactate disclosed results estatistically higher in dogs belonging to CHF class II as compared to control animals, as well as between class IIIa dogs as compared to controls, class Ia, and class Ib dogs. However, the results of class II dogs were similar to those of classes Ia and Ib animals. Also, no difference was documented between the results of classes II and IIIa dogs. A positive correlation was identified between serum lactate concentration and the left ventricular internal diameter, fractional shortening, and left atrium-to-aorta ratio. A positive correlation was also observed between serum lactate and mean heart rate, VHS, and P wave amplitude. A negative correlation was documented between serum lactate and systolic blood pressure. A larger area under the ROC curve was disclosed for the jugular lactate, which showed the concentration of 3,25 mmol/L to provide the highest sensibility and specificity in differentiating healthy dogs from those presenting CHF. A necessidade do entendimento do processo de remodelação cardíaca tem acarretado em aumento do número de pesquisas envolvendo terapias biológicas. Neste contexto, peixes teleósteos emergem como opção de modelo experimental, pois possuem homologia morfofuncional e imunológica com mamíferos. Tilápias-do-Nilo (Oreochromis niloticus) já vêm sendo empregadas no estudo de diversas doenças que afetam seres humanos. Com a presente pesquisa, objetivou-se desenvolver técnica de acesso videocirúrgico e induzir lesão cardíaca mecânica, em tilápias-do-Nilo; avaliar a eficácia da metadona no controle da dor associada à lesão; e caracterizar os mecanismos de remodelação do miocárdio ventricular, empregando-se avaliações por eletrocardiograma e por microscopias. As avaliações foram conduzidas antes, 1 h depois, e transcorridos 7, 14, 30, 60 e 90 dias da lesão cardíaca. O acesso à cavidade celomática foi realizado utilizando-se um punch de biopsia de 3 mm, permitindo a introdução de endoscópio para posterior amputação do ápice do coração. O procedimento foi executado rapidamente e com baixo índice de mortalidade transoperatória (5,60%) e pós-operatória (3,38%). Aproximadamente 6% de tecido cardíaco, apresentando epicárdio e miocárdio, foi removido com a pinça de biopsia, sendo que a videocirurgia permitiu boa visibilização interna, padronização do tamanho da lesão e alta precisão. The need for understanding cardiac repair mechanism has increased the number of researches biologic therapy field. In this context, teleost fish show as a experimental model, as they have morpho-functional and immunologic homology whit mammals. Nile tilapia (Oreochromis niloticus) has been used to study several diseases that affect humans. The present study aimed to develop a video surgery access and mechanically induce cardiac injury in Nile tilapia; to evaluate the analgesic activity of methadone in effectively control the pain associated with the injury; and characterize myocardial repair mechanisms by microscopy and electrocardiogram analysis. Data were recorded before cardiac injury and then 1 hour, 7, 14, 30, 60 and 90 days after cardiac injury. The access to coelomatic cavity was made with a 3-mm biopsy punch, allowing the passage of an endoscope for amputation of the cardiac apex. The procedure was quickly performed with intraoperative (5.6%) and post-operatory (3.38%) low mortality rates. Approximately 6% cardiac tissue, including epicardium and myocardium were removed with the biopsy forceps, once video surgery allowed good visualization, standard lesion areas and great precision. AvaliaÃÃo do autocuidado de pacientes apÃs transplante cardÃaco acompanhado na consulta de enfermagem.

nÃo hà / O transplante cardÃaco à uma modalidade terapÃutica de alta complexidade e exige do enfermeiro uma assistÃncia especÃfica, com qualidade, tendo como foco o autocuidado, uma vez que possibilita o envolvimento do paciente de maneira participativa no tratamento. Teve-se como objetivo geral avaliar o autocuidado de pacientes que realizaram transplante cardÃaco, baseado no Modelo do Autocuidado de Orem. E como especÃficos: identificar os fatores condicionantes que interferem na prÃtica do autocuidado de paciente transplantado cardÃaco apÃs a alta hospitalar; verificar os dÃficits de autocuidado de pacientes adultos que realizaram transplante cardÃaco; e correlacionar os fatores condicionantes com o Perfil de Engajamento do Autocuidado. Trata-se de um estudo descritivo-analÃtico, com delineamento transversal e natureza quantitativa, desenvolvido na Unidade de Transplante e InsuficiÃncia CardÃaca de um hospital pÃblico terciÃrio da cidade de Fortaleza-CearÃ-Brasil. A amostra foi composta por 63 pacientes transplantados cardÃacos, que atenderam aos critÃrios de inclusÃo. A coleta de dados foi realizada mediante uma entrevista individualizada, no perÃodo de outubro a dezembro de 2011. O projeto foi aprovado pelo Comità de Ãtica e Pesquisa, sob protocolo do CEP/HM: 109/11. Como resultados dos fatores condicionantes, obteve-se: sexo masculino (88,9%); a idade variou de 23 a 72 anos, predominando de 40 a 59 anos (68,3%); cor da pele nÃo-branca (74,6%), catÃlico (81,0%); casado (77,8%); procedentes do interior do estado (49,2%); nÃvel de escolaridade-ensino fundamental (71,4%); aposentado ou nÃo trabalha (82,5%); renda de atà um salÃrio mÃnimo (47,6%); miocardiopatia chagÃsica (28,6%); tempo pÃs-transplante entre um e trÃs anos (39,7%). Em relaÃÃo ao autocuidado do requisito universal, tÃm-se como dÃficit os seguintes dados: oxigenaÃÃo/respiraÃÃo (26,9%); higiene pessoal (31,7%); higiene do ambiente domiciliar (47,6%); ingestÃo de lÃquidos (39,6%); ingestÃo de alimentos (68,8%); eliminaÃÃes (20,6%); prÃtica de exercÃcio fÃsico (87,3%); sono e repouso (79,3%); interaÃÃo social (90,4%); comportamento emocional (58,7%); prevenÃÃo de doenÃa/promoÃÃo da saÃde (77,7%); tabagismo (1,58%); etilismo (3,17%); prÃtica sexual (61,9%); prevenÃÃo do cÃncer (60,3%). Quanto ao autocuidado do requisito desenvolvimental, apresentou os seguintes dÃficits: participaÃÃo das atividades educativas (39,6%); adaptaÃÃo Ãs mudanÃas apÃs transplante cardÃaco (34,9%). E ao requisito desvio de saÃde, tÃm-se os seguintes dÃficits: comparecimento Ãs consultas da equipe de saÃde (41,2%); imunizaÃÃo bÃsica (100%); uso de mÃscara descartÃvel (46%); contato com pessoas e animais domÃsticos (38%); e conhecimento (20,6%). O Perfil de Engajamento do Autocuidado variou de 88 a 113, constatando-se que a maioria dos pacientes apresentou algum dÃficit de autocuidado, pois 57,1% estavam na classe que âfrequentemente realizava autocuidadoâ. No entanto, embora nenhum paciente realizasse 100% das prÃticas de autocuidado recomendadas para o transplantado cardÃaco, 42,9% sempre realizavam o autocuidado. Conclui-se que os pacientes transplantados cardÃacos apresentam dÃficit de autocuidado para manutenÃÃo e promoÃÃo da saÃde. Portanto, à necessÃrio que os profissionais da equipe de transplante cardÃaco estejam atentos para os fatores condicionantes do autocuidado dos pacientes transplantados cardÃacos, visando estabelecer estratÃgias para reduÃÃo do dÃficit de autocuidado.

M.Cur. (Intensive General Nursing) / The incidence of coronary heart disease is still rising due to man's stressful way of life. Angioplasty is a relatively new procedure used to dilate coronary arteries which have been narrowed by sclerosis. This is seen as the 'instant solution' for a select group of patients with coronary artery sclerosis. It fits in well in the fast paced society. Angioplasty is painful and causes anxiety as the patient is awake during the whole procedure. The goal of the study is to determine the patient's experience during angioplasty and to set guidelines for nurses for the preparation of other patients for angioplasty. The Phenomenological method of interviewing was used for data-collection. Interviews were conducted with 9 participants who had had their first experience of angioplasty. Data was subjected to Phenomenological analysis and verified with suitable literature. The conclusions that were reached were used to set guidelines and make suggestions for the counselling of patients for angioplasty.

Improving Care for Patients Hospitalized with Heart Failure

Background: Heart failure is a clinical syndrome occurring from the heart’s inability to effectively fill and or pump blood, it is the most common reason for admission in elderly patients. Guideline directed medical therapy refers to implementation of all class I agents to reduce patient morbidity and mortality, unless there is an appropriate contraindication. Appropriate beta blocker (BB), angiotensin converting enzyme inhibitor (ACEI) or angiotensin receptor blocker (ARB), and aldosterone antagonist (AA) are recommended to be prescribed together prior to discharge for a hospital admission for decompensated heart failure with reduced ejection fraction (HFrEF). Get With The Guidelines – Heart Failure (GWTG- HF) is an online quality improvement project that assists hospitals in providing guideline directed care. Objective: The purpose of this study was to determine if implementation of the GWTG-HF program, increases provider adherence to guideline directed medical therapy (GDMT) for patients admitted with a primary diagnosis of decompensated HFrEF at Banner University Medical Center Tucson (BUMCT). Design: This is a quality improvement project with a pre and post test descriptive design. Setting: BUMCT from 10/04/17 – 11/08/17 Participants: Fifty-five patients discharged with the primary diagnosis of decompensated HFrEF Measurements: Baseline guideline adherence for a 30-day period was compared to guideline adherence after the initiation of the GWTG-HF program. Results: The 24 patients pre intervention were compared to 31 patients post intervention. The following results were found when comparing pre and post adherence rates: BB adherence 92% versus 100%, ACEI/ARB adherence 100% versus 94%, AA adherence 67% versus 84%, and guideline directed medical therapy 58% versus 81%. There were no statistically significant differences for the pre and post adherence rates. Conclusion: Although, there were no statistically significant differences found to support that implementation of the GWTG-HF program, increases providers adherence to GDMT for patients admitted with a primary diagnosis of decompensated HFrEF, the trends were clear. In three out of four class I agents, there was an increase in appropriate provider prescribing per the guidelines.

How chromatin-remodeling complexes modulate gene networks to control organ-specific properties is not well understood. For example, Baf60c (Smarcd3) encodes a cardiac-enriched subunit of the SWI/SNF-like BAF chromatin complex, but its role in heart development is not fully understood. We found that constitutive loss of Baf60c leads to embryonic cardiac hypoplasia and pronounced cardiac dysfunction. Conditional deletion of Baf60c in cardiomyocytes resulted in postnatal dilated cardiomyopathy with impaired contractile function. Baf60c regulates a gene expression program that includes genes encoding contractile proteins, modulators of sarcomere function, and cardiac metabolic genes. Many of the genes deregulated in Baf60c null embryos are targets of the MEF2/SRF co-factor Myocardin (MYOCD). In a yeast two-hybrid screen, we identified MYOCD as a BAF60c interacting factor; we showed that BAF60c and MYOCD directly and functionally interact. We conclude that Baf60c is essential for coordinating a program of gene expression that regulates the fundamental functional properties of cardiomyocytes.
