The trade-offs of using different physician attribution methods for audit and feedback interventions in general medicine inpatient care

BACKGROUND: Audit and feedback interventions have the potential to improve clinical care. Electronically captured administrative and clinical data routinely collected in Canadian hospitals may be used to provide feedback to physicians in general medicine in-patient care. The computation of appropriate quality indicator requires patient care to be attributed to individual physician(s). The appropriate attribution method in contexts where multiple physicians are involved in the care with varying degree of responsibilities that change over time is not straight forward. There has so far been little guidance in the literature of how to best accomplish this. The objective of this study is to identify trade-offs of different physician attribution methods by applying them to the same large clinical dataset. METHODS: A retrospective cohort study was conducted using the GEMINI dataset consisting of administrative and clinical data of hospitalized patients discharged from General Medicine service between April 1, 2010 and October 31, 2017 extracted from electronic systems at 7 hospitals in the Greater Toronto Area. A set of four quality indicators (length of stay, 30-day re-admission, in-patient mortality, use of advanced imaging) used in an audit and feedback intervention was calculated for each physician using 5 different physician attribution methods: STRICT (only patients with the same admitting, discharging, and most responsible physician with length of stay less than 14 days were included to capture those patients whose care was provided by only 1 physician), ADMIT (attribute care to admitting physician), DISCHARGE (attribute care to discharging physician), MRP (attribute care to most responsible physician), and ANY (attribute care to admitting, discharging, and most responsible physicians). The comprehensiveness and comparability of each attribution method were calculated. The actual differences of the indicator value and physician ranking for each indicator was compared between each pair of attribution methods. RESULTS: 222,490 hospitalization cared for by 203 physicians were included. STRICT attribution method was least comprehensive, capturing only 40% of patients cared for by a physician), while ADMIT, DISCHARGE, and MRP captured 70% of patients. All attribution methods produced patient populations for individual physicians that were comparable to those seen at each hospital. STRICT attribution method resulted in length of stay values 4.7 to 6.8 days shorter than other attribution methods and had poor rank correlation of physicians when compared to other attribution methods (spearman rank correlation 0.27 to 0.52). Absolute differences for the other 3 indicators were small between all attribution methods, and relative ranking of physicians were reasonably preserved (strong or very strong rank correlation). INTERPRETATION: Different attribution methods have different comprehensiveness, but all produced mostly comparable patient populations for physicians. Certain attribution method can affect apparent physician performance for some quality indicators but not others. The impact of physician attribution methods deserve consideration during the design of audit and feedback interventions. / Graduate

El presente proyecto profesional “SISTEMA DE INFORMACIÓN PARA LA GESTIÓN DE EVENTOS EN LA MISIÓN PERUANA CENTRAL SUR” tiene como objetivo el desarrollo de un sistema para la Misión Peruana Central Sur (MPCS) con la finalidad de planificar, ejecutar y controlar los resultados de la ejecución de eventos planificados en un periodo determinado. La necesidad de realizar este proyecto se origina debido a que en los últimos años se ha visto perjudicada la imagen de la Misión, la difusión de sus objetivos estratégicos, Todo esto debido a una mala estimación del presupuesto anual por parte de los departamentos que conforman la MPCS. El proyectó está conformado por 8 Capítulos. En el primer capítulo se desarrolla la fundamentación teórica , en el segundo capítulo se presenta la propuesta de solución , en el tercer capítulo se detalla el modelado del negocio identificando los actores, trabajadores y procesos involucrados, en el cuarto capítulo se detallan los requerimientos relacionados a la propuesta de solución, en el quinto capítulo se dará detalle de la arquitectura del software, en el sexto capítulo se detallan los patrones de diseño ,así como el modelo físico de datos y su diccionario. En el sétimo capítulo se dará detalle a la documentación correspondiente a la calidad y pruebas del software y finalmente en el último capítulo se detalla la gestión del proyecto donde se identifican a los interesados, y se realiza el detalle del cronograma de ejecución con las actividades que se llevaron a cabo para desarrollar el presente proyecto. / This professional project “INFORMATION SYSTEM FOR EVENT MANAGEMENT IN THE SOUTH CENTRAL PERUVIAN MISSION” aims to develop a system for the South Central Peruvian Mission (MPCS) with the purpose of planning, executing and controlling the results of the execution of planned events in a given period. Design and Deployment of a Location Aware Social Network Service

The emergence of Online Social Networking service which have introduced new paradigms of sharing have changed the way people interact with each other. They provide an easy and creative way to connect with people. As a result there are all kinds of people like working professionals, students, businessmen and many more who participate actively in such networks. Social Networking services are also turning out to be a medium for promoting small scale business, books, events etc. Apart from this there is substantial literature which suggests that social connections can be created, maintained and strengthened through computer mediated channels of communication with special importance being given to the role of internet in maintaining social relations. Thus the social networking services build digital connections among people who are socially connected. Though the users can leverage a host of applications/services like messaging, chatting, file sharing, discussions in groups, activities etc from these online social networking applications, there is no relevance given to the context/location of a user in the existing setup. With the technology moving from personal computers to hand held and intelligent devices with embedded technology and connectivity there is a rise in the development of pervasive computing applications which provide easy access to required information and applications through a class of ubiquitous equipment that operate when and where needed. This adds value to the existing infrastructure by providing services which are “anytime and anywhere”. Social networking applications hold a vast amount of personal data that can be used to improve the experience of users in the physical space. With the changing environment of users and also that of the people in their social network a system with a capability of examining the environment and providing highly personalized services to the user is highly desirable. Thus the work described in this document will project a social networking service that gives significance to the location of a user. In other words we will see how social networking services can migrate from networked personal computers to more ubiquitous hand held devices like PDA’s, Smart Phones which would involve the location information of the physical space in order to make social experiences more meaningful. The primary goals of this work are:- 1.To develop a social network service and deploy it to a select set of users and monitor the level of interaction. 2.Propose an architecture which could be used to implement location awareness in the social network service developed. 3.Develop a prototype and provide a “proof of concept” of the architecture and the system designed. 4.Survey privacy issues that are of great importance in such systems. This work will produce a framework and perception of designing a social networking service that is location aware. The resulting system will be deployed to a set of users’ in order to understand the usage pattern of such systems. This would give a good picture of how such systems would be perceived and compared to the existing online social network services that are insensitive to the change in location of its users.

This master thesis is conducted as a conclusion of Master of Science degree in Production Engineering and Management at KTH Royal Institute of Technology. The focus of this thesis work is to make the work more convenient and improve production efficiency by building a management information system (MIS) thus replacing manual and decentralized information management mode on the shop floor. With the use of the computers in the office and tablets beside the machine of each work center on the shop floor, personnel can have access to the necessary information easily and quickly.   This management information system is built according to the actual production situation and needs in Bosch Rexroth AB in Mellansel. First, research on the production process and the management mode in the factory is conducted. Then, the form and the architecture of the system are decided. After thorough understanding and comparison, web application form and currently widely used Browser/Server (B/S) architecture are adopted. Finally, during the detailed design and system implementation process, databases and functional modules are developed. Microsoft Access is chosen as the database implementation. Microsoft Visual Studio.NET combined with C# language and IIS (Internet Information Service) is chosen as the application development tool. When conducting the user trial test on the shop floor, functions of this application system well meet the requirements, and associates can easily and quickly access the information. This application system can speed up the information flow in production, realizing more scientific, standardized and efficient management, which ultimately promotes the production and management of the enterprise.   This management information system realizes the following functions in a digitized way: 1. system security management: user registration, log in and permission settings; 2. drawings viewing; 3. instructions viewing; 4. maintenance recording and checking; 5. deviation reporting and processing; 6. process confirmation recording and checking. / Denna masteruppsats är slutuppgiften för civilingenjörsexamen i produktionsteknik och management på Kungliga Tekniska högskolan. Vid införande och test i verkstaden visade sig systemets funktioner väl svar mot behoven och personalen kunde enkelt och snabbt komma åt informationen. Detta system kan öka informationsflödet produktionen och främja metodisk, standardiserad och effektiv förvaltning, vilket gynnar företagets produktion och styrning.
