Diagnóstico de alergia por componentes em pacientes adultos com dermatite atópica / Component resolved diagnosis in adult patients with atopic dermatitisBoufleur, Karine di Latella 22 May 2018 (has links)
Dermatite atópica (DA) é uma dermatose inflamatória, de caráter crônico e recidivante, com alta prevalência mundial, caracterizada por eczema localizado ou generalizado, xerose cutânea e prurido intenso, mais comum em crianças, porém podendo acometer adultos, muitas vezes prejudicando de forma considerável a qualidade de vida dos pacientes e seus familiares. O diagnóstico de alergia inclui testes in vivo (testes cutâneos de hipersensibilidade imediata ou prick test) e testes in vitro com mensuração de anticorpos IgE no sangue. Está disponível método para detecção de alergia baseado no princípio de diagnóstico por componentes, o ImmunoCAP Immunosorbent Allergen CHIP (ImmunoCAP-ISAC) que marca a transição para o diagnóstico molecular de alergia. No presente estudo, tivemos por objetivos determinar o perfil de resposta IgE a alérgenos purificados, naturais ou recombinantes, em pacientes adultos com DA e comparar o valor do diagnóstico de alergia por componentes com métodos diagnósticos existentes. Foram selecionados 39 pacientes adultos com DA dentre aqueles Atendidos nos Ambulatórios de Alergia e Dermatologia do HCFMRP-USP. A idade dos pacientes variou de 14-66 anos (média ± DP 34,3 ± 2.1 anos), 64% mulheres. O tempo médio de doença foi de 16 anos. SCORAD médio foi de 36,6 (2-90). Média geométrica (MG) de IgE total foi 1,963 kU/L (24-63,000 kU/L). Alérgenos de ácaros foram dominantes, com sensibilização a Der p1 e a Der f1, Der p2 e Der f2 em 82% e 85% dos pacientes, com MG 27,6; 50; 39,2; 45,4 ISU-E respectivamente, seguidos de gato (38%), cachorro (36%) e pólen de gramíneas (36%). IgE para alérgenos de baratas, fungos, pólen de árvores, látex e veneno de insetos foi encontrada em menos de 20% dos pacientes em baixos níveis. Sensibilização para castanhas (33%) e camarão (31%) foram os alérgenos mais prevalentes entre os alimentos, enquanto a reatividade IgE para leite e ovo esteve presente em 10% ou menos dos pacientes, com baixos níveis de anticorpos IgE na maioria dos casos. Apenas 6 pacientes (15,4%) apresentaram IgE para o panalérgeno tropomiosina, e 3/39 (7,7%) foram negativos para todos os 112 componentes de alérgenos testados no ImmunoCAP-ISAC, com níveis de IgE total de 24, 38,7 e 156 kU/L. Concluímos que o perfil de sensibilização IgE entre os pacientes adultos com dermatite atópica difere daquele entre os pacientes com alergiarespiratória, apresentando menos sensibilização para baratas e para o panalérgeno tropomiosina, e difere ainda do perfil presente em crianças com DA, com predomínio de sensibilização IgE a castanhas e camarão, e baixa taxa de sensibilização a alérgenos de leite de vaca e ovo. / Atopic dermatitis (AD) is an inflammatory, chronic, relapsing dermatosis with a high global prevalence characterized by localized or generalized eczema, cutaneous xerosis and intense pruritus, more common in children, but it can affect adults, often causing considerable damage to the quality of life of patients and their families. The diagnosis of allergy includes in vivo tests (skin tests of immediate hypersensitivity or prick test) and in vitro tests with measurement of IgE antibodies in the blood. An allergy detection method based on the principle of component diagnosis is available, ImmunoCAP Immunosorbent Allergen CHIP (ImmunoCAP-ISAC), and marks the transition to the molecular diagnosis of allergy. In the present study, we aimed to determine the IgE response profile to purified, natural or recombinant allergens in adult patients with AD and to compare the value of component allergy diagnosis with existing diagnostic methods. Thirty-nine adult patients with AD were selected from among those attending the Outpatient Clinic of Allergy and Dermatology at HCFMRP-USP. The age of the patients ranged from 14-66 years (mean ± SD 34,3 ± 2.1 years), 64% women. The mean duration of illness was 16 years. SCORAD mean was 36,6 (2- 90). Geometric mean (GM) of total IgE was 1,963 kU / L (24-63,000 kU / L). Mite allergens were dominant, with sensitization to Der p1 and Der f1, Der p2 and Der f2 in 82% and 85% of the patients, with GM 27,6; 50; 39.2; 45.4 ISU-E respectively, followed by cat (38%), dog (36%) and grass pollen (36%). IgE for cockroach, fungus, tree pollen, latex and insect venom allergens was found in less than 20% of patients at low levels. Sensitization to nuts (33%) and shrimp (31%) were the most prevalent allergens among foods, while IgE reactivity to milk and egg was present in 10% or less of the patients, with low levels of IgE antibodies in most cases. Only 6 patients (15.4%) presented IgE for the pan-allergen tropomyosin, and 3/39 (7.7%) were negative for all 112 allergen components tested on ImmunoCAP-ISAC, with total IgE levels of 24; 38,7 and 156 kU / L. We concluded that the IgE sensitization profile among adult patients with atopic dermatitis differs from that among patients with respiratory allergy, presenting less sensitization to cockroaches and to the pan-allergen tropomyosin, andalso differs from the profile present in children with AD, with a predominance of IgE sensitization to nuts and shrimp, and low sensitization rate to cow\'s milk and egg allergens.

A survey of the literature has suggested that most of IT system failure in information system development is due to problems of identifying and meeting users' requirements. Conventional systems that support the waterfall approach try to focus on defining information processing requirements rather than looking at IT from a wider perspective. This approach complicates the relationship between the client who 'owns' the problem and the developer who seeks to solve it. Therefore it is common for systems to be created which do not satisfy the needs of their human operators even though they are technically sound. The main aim of the research is to develop a new methodology that can contribute to the effective determination of user requirement. The new methodology has been constructed from unifying ORDIT (Organisational Requirement Definition for Information Technology) and ISAC (Information System Work and Analysis of Change) methodologies. Therefore it can solve a certain set of problems, some which are solved by ORDIT, some which are also solved by ISAC and some which neither of the two methodologies can solve. The activity model used in ISAC is insufficient for solving the organisational issues, therefore it is replaced with the responsibility model which is taken from ORDIT. The responsibility model is used in order to give a clearer understanding of the organisation's structure, aim, objectives and policies. The tables and tools, which are used in the change analysis stage ofISAC, will be used in the new methodology for the purpose of identifying the business problem, user objectives and change needs. These tools and models are used in order to elicit requirements for different problem owner in different levels of the organisation. The new methodology has been applied to a real case study in order to demonstrate and evaluate its performance and usefulness. This case study showed the new methodology to be useful and effective.

Comprehensive Ionization Model Development for the FEBIAD Ion Source and Its Application for TRIUMF’s Radioactive Ion Beam ProgramMaldonado Millan, Fernando Alejandro 28 July 2022 (has links)
Radioactive isotopes enable advanced medical treatments and the study of nuclear
structure, nuclear astrophysics, and fundamental symmetries. TRIUMF, Canada’s
Particle Accelerator Centre, generates radioactive isotope beams using the Isotope
Separation On-Line method. At TRIUMF, the Forced Electron Beam Induced Arc
Discharge (FEBIAD) ion source is used to ionize specific isotopes but often presents
limited performance and lower efficiencies compared to other facilities. To investigate
the source limitation, elucidate the ionization mechanism, and propose improved and
highly efficient sources for upcoming facilities, a combined numerical and experimental
campaign has been undertaken. The developed numerical ionization model is able to
describe the source as an electron impact ion source that is governed by strong space
charge effects. The spatially distributed ionization rate is higher at specific locations
of the anode volume, and this has been confirmed experimentally. The validated
numerical model has been further used to propose simulation-based optimizations.
With the overall optimization, the ionization efficiency for the TRIUMF FEBIAD is
expected to increase ten-fold. / Graduate

The thesis describes the process of transformation of the behavior of processor described in C language into VHDL language. Individual steps of automatized transformation are compared to manual design of processor. The thesis highlights advantages of the internal representation of program in the form of graph. Optimizations based on various factors are introduced in this thesis. One of them are algebraic modifications of expressions. The time of computation or space requirements of the circuit can be lowered by proper aplication of properties of math operators - associativity, comutativity and distributivity. Special attention is payed to optimizations, that make use of parallelism of operations for the process of planning. Algorithms of time-constrained scheduling and resource-constrained scheduling are discussed. The end of this thesis is devoted to resource allocation.

Para atingir o objetivo proposto foi aplicada uma metodologia, adaptada do método Smartline proposto para a costa Australiana, denominada ISAC – Índice de Sensibilidade Ambiental Costeira, que se apresenta neste estudo dividida da seguinte forma: (a) análise das variáveis regionais de vulnerabilidade costeira da região centro-norte Catarinense; (b) avaliação de critérios como geologia, geomorfologia, clima, dinâmica marinha, topografia e socioeconomia da área de estudo; (c) classificação hierárquica frente a fatores de resistência e/ou função ecológica e posterior ponderação de acordo com a sensibilidade de cada critério a eventos extremos e a elevação do nível médio dos oceanos; (d) elaboração de mapas temáticos contendo informações da sensibilidade ambiental presente nas praias do litoral centro e norte de Santa Catarina para cenários de ondulação provenientes de Leste e Sudeste. O ISAC demonstrou que 39% da zona costeira, apresenta sensibilidade Alta para o cenário de ondulação proveniente de Leste, seguido pelos valores de 18% que apresenta sensibilidade Baixa, 17% com sensibilidade Muito Alta e 16% com sensibilidade Muito Baixa. Para o cenário de ondas provenientes de Sudeste, 41% da área de estudo apresenta Alta sensibilidade em seus segmentos costeiros, com valores menores na sequência, onde 20%, 15% e 13% da costa demonstraram sensibilidade Baixa, Muito Baixa e Muito Alta respectivamente. Com relação ao risco socioambiental costeiro, no cenário com ondulações de Leste, 51% da linha de costa apresenta risco Médio, 30% risco Alto e 18% risco Baixo e referente ao cenário de Sudeste 53%, 27% e 20% do segmento costeiro apresentam risco Médio, Alto e Baixo respectivamente. The ISAC and the Social Environmental Risk analysis were effective for a regional assessment of the central northern coast of Santa Catarina. However, it is noted that to perform a detailed local analysis is necessary to obtain a greater volume of information about the criteria or environmental parameters through field campaigns.

Entretanto, salienta-se que para a realização de uma análise local, de maior detalhe, se faz necessário a obtenção de um volume maior de informações, a cerca dos critérios ou parâmetros ambientais, através de campanhas de campo. / The aim of this study is to contribute to the understanding of environmental sensitivity and Social Environmental risk for the central-north coastal sector of the Santa Catarina State (from Biguaçu to Itapoá). To achieve the proposed objective the ISAC - Coastal Environmental Sensitivity Index methodology was applied, adapted from the Smartline method proposed for the Australian coast. In this study the ISAC methodology is divided as follows: (a) regional variables analysis of coastal vulnerability for the central-north region of Santa Catarina; (b) assessment of the criteria such as geology, geomorphology, climate, marine dynamic, topography and social-economy of the study area; (c) hierarchical classification against resistance factors and/or ecological function and subsequent pondering according to the sensitivity of each criterion to extreme events and mean sea level rise; (d) development of thematic maps containing the environmental sensitivity information of the study area for two different wave scenarios, waves from the East and Southeast directions. The ISAC showed that 39% of the coastal area has High sensitivity for the East waves, followed by the values of 18% which has Low sensitivity, 17% with Very High and 16% with Very Low sensitivity. For the Southeast waves scenario, 41% of the study area has High sensitivity in the coastal sections, with lower values in the sequence, where 20%, 15% and 13% of the coast showed the sensitivity as Low, Very Low and Very High respectively. Regarding the social environmental coastal risk, for the East wave scenario, 51% of the coastline presents Medium risk, 30% High risk and 18% Low risk. For the Southeast waves scenario, 53%, 27% and 20% of the coastal segment has Medium, High and Low risk respectively. The ISAC and the Social Environmental Risk analysis were effective for a regional assessment of the central northern coast of Santa Catarina. However, it is noted that to perform a detailed local analysis is necessary to obtain a greater volume of information about the criteria or environmental parameters through field campaigns.

This work deals with designing a library of processor models used in embedded systems. Processor architectures are described using the ISAC language. The ISAC language is one of several outcomes of the Lissom project that is taking place at the Faculty of Information Technology, BUT, Brno. The beginning of this work is aimed to provide the introduction to processor architectures used in today's embedded systems. Remaining sections are devoted to presentations of exemplary processor architectures and the description of their implementation. This work is finalized by concluding the gathered experience with emphasis on the suitability of the ISAC language for architecture description and the efficiency of its simulation.

The main goal of the diploma thesis is to propose new innovative solutions leading to the improvement of the company's information system as well as the implementation of new solutions and innovations, in order to maximize the turn-over and market value of the company. The theoretical part is focused on the definition and characteristics of the development of the solved issue, including units dealing with information systems, analysis of information systems and database systems themselves. The subject of my own work is the characteristic of Zawmark Plus, the analy-sis of the internal and external environment of the company, financial analysis, analysis of the company's information system and its components, evaluation of analyzes and concepts of the proposals themselves for the improvement of the information system. Suggestions for IS improvement based on analyzes can be used and implemented in other companies with a similar business model. In its own part, the possible complications associated with the implementa-tion of new proposals are summarized. The conclusion itself contains a summary of the whole scope of the issue and information on the benefits resulting from the implementation of new refinements.
