A prospective cohort study was realized, to assess the main infections in patients with cerebrovascular accident (CVA), admitted to the Stroke unit of Hospital das Clínicas de Botucatu. It was demonstrated that the main infections refer to the respiratory and urinary tract 8.4% and 7.7% respectively. They are prevalent in the elderly people, with hypertension and ischemic stroke in use of invasive devices, which prevail peripheral venous access (95.1%) and nasoenteral tube (31.25%). The highest incidence of microorganisms consisted of gram-negative, among them: Escherichia Coli and multi-resistant microorganisms (MRSA). Non-alcoholic patients and non HIV carriers were protected from infection occurrence of the respiratory and urinary tract (p> 0.05). The non-use of the devices, like indwelling urinary catheter and central venous catheter, demonstrated protection to the occurrence of urinary tract infection (p> 0.05). In relation to the type of stroke, hemorrhagic stroke suggests predisposition to the occurrence of urinary tract infection (p> 0.05). Further studies may be developed under the perspective of the patient's prognosis with stroke; from the perspective of health professionals concerning health care-associated infections (HAI)) and patients with stroke; in addition to researches on community infections related to seasonality periods in the hospital environment.
