Le coût des biomédicaments impacte lourdement le fardeau financier déjà important imposé par la polyarthrite rhumatoïde (PR) à la société. Le but de ce travail était de rechercher les motivations de la prescription des médicaments biosimilaires en rhumatologie, tout particulièrement pour prendre en charge les patients atteints de PR. Les travaux effectués ont permis d’estimer les économies annuelles réalisables en France avec le médicament biosimilaire de l’infliximab à 13,6 millions d’EUR, pour la seule prise en charge de la PR. Par ailleurs, une enquête menée auprès des rhumatologues et pharmaciens a permis d’identifier les principaux enjeux liés à l’utilisation de ces médicaments. Enfin, le suivi de leur utilisation en région Grand Est entre 2015 et 2016 rend compte de leur utilisation encore modeste. La mise sur le marché régulière de médicaments biosimilaires, combinée à l’expérience croissante des acteurs et aux dispositifs d’incitation mis en place, devraient permettre de faire progresser la part d’utilisation de ces médicaments dans les prochaines années, et de s’assurer qu’ils tiennent bien leurs engagements sur le plan des économies réalisées. / Biological drugs are associated with high procurement costs and heavily impact the financial burden imposed by rheumatoid arthritis on society. The purpose of this work was to investigate the benefits of using biosimilar drugs in rheumatoid arthritis patients. This work showed the introduction of biosimilar infliximab could lead to substantial annual cost savings of up to €13.6 million nationally, to treat rheumatoid arthritis patients only. Moreover, a biosimilar survey involving rheumatologists and pharmacists allowed us to identify the main issues at stake. Finally, the monitoring of their use in Grand Est region between 2015 and 2016 account for their relatively low utilization rate. The regular launch of new biosimilar medicines, together with growing experience of healthcare players and incentive policies, should allow to increase biosimilar uptake in the future, and also to make sure they meet well their commitments in terms of savings generated.

Výkaz zisku a ztráty v sektoru pojišťoven v ČR / Profit and loss in the insurance sector in the Czech RepublicMácová, Petra January 2013 (has links)
The graduation thesis aims to specify and define, with respect to the Czech legislation, the profit and loss statement of the insurance companies and those items of revenues and expenses that are specific for the insurance companies. The thesis deals with the principal characteristics of the revenues and the expenses of the insurance companies as well as with the matter whether insurance companies can in any means influence their management result. It also focuses on the field of the insurance security -- what is the insurance security in the Czech Republic in relation to the European Union, and on the quantity limits of the profit and loss statement, in the Czech insurance sector. It also assesses the efficiency of the local insurance companies in relation to this, what is the share of the earned premium when taking in account the insurance claims, the operating costs and the management result. The graduation thesis came to the conclusion that an insurance company can quite easily influence its accounting management result and therefore it is necessary for the company to respect the relevant accounting measures. Furthermore, the Czech Republic in comparison to the other EU countries indicates relatively low level of insurance security. With respect to the efficiency it is possible to conclude (in relation to the year 2012 and the local insurance companies) that the earned premium comprises of more that 60% of the claimed damages, 30% of the operating costs and 10% of the earned premium represents the net profit.

Analyses of experiences of vicarious traumatisation in short-term insurance claims workersLudick, Marne 05 September 2013 (has links)
Thesis (Ph.D.(Psychology))--University of the Witwatersrand, Faculty of Humanities, 2013. / The research entailed a comprehensive study of vicarious trauma in short-term insurance claims
workers, compared to trauma counsellors and a control group of holiday booking consultants. A
well-known, comprehensive model of compassion fatigue/secondary traumatic stress, developed for
therapists formed the basis of the study. The research attempted to determine whether this model
can be applied more widely to include administrative populations exposed to traumatised clients on
a regular basis. To this end, the model was deconstructed into its eleven constituent parts and each
element was investigated in addition to other variables of interest to the study. This was done to
determine the importance and applicability of each model element and other selected variables to
the administrative context.
A mixed methods approach was utilised, which combined quantitative and qualitative data. The
results yielded by the study were collectively utilised to construct an etic and an emic voice from
the research. At the same time, effects from vicarious trauma were considered from an overarching
bio-psychosocial stance, systematically gauging effects on various levels of functioning. Scores
from quantitative measures on secondary traumatic stress, negative cognitive schemas, empathy,
social support and compassion satisfaction were statistically analysed, which revealed significant
differences between the worker groups. Widely accepted relationships between the study variables
were tested and found to hold true within and across groups. Regression analysis determined the
roles of empathy, social support and compassion satisfaction in vicarious trauma, as measured by
secondary traumatic stress and negative cognitive schemas. In addition, constructivist selfdevelopment
theory was employed to interpret the negative cognitive effects from vicarious
traumatisation.
Qualitative data were utilised to further elucidate the role and nature of vicarious trauma in each of
the worker groups. The themes of exposure to client suffering, detachment, level of empathic
engagement, personal trauma history and difficult life demands were unearthed from the qualitative
data, which illuminated the importance and role of each of these elements to claims workers. Other
areas of interest, being utilisation of sick-leave as a means to cope, work-related illness, attitudes
towards professional counselling, feelings evoked by traumatised clients, and the language utilised
by workers in response to client traumata were investigated. Further effects on participants as well

as effects that reach beyond the person were identified and examined. Effects on the social and
work contexts were also elucidated.
Finally, interesting themes that emerged spontaneously from the data were considered. The
consideration of the various model elements and other areas of interest systematically revealed that
administrative workers dealing with traumatised clients are also affected by the process of vicarious
trauma. Furthermore, the model was found to be largely suitable to the context of claims workers.
However, the model was expanded to augment its usability within the more general administrative
domain. Finally, the overarching aim was to enrich, contextualise and elaborate on the experiences
of claims workers within their unique work context, to facilitate insight and a deeper understanding
of vicarious trauma in more administrative populations that have largely been overlooked in
research.

Climate change has become one of the most debated environmental risks. The world is faced with the threat of weather variability. There has been an increase in the frequency and severity of extreme weather events. There is rising concern that weather losses might affect the sustainability of insurance businesses. The primary obective of the study was to ascertain the significance of changes in temperature, precipitation and wind speeds in explaining changes in weather-related claims.Furthermore the reseach had three key secondary objectives, firstly to find if changes in annual average temperature levels lead to changes in weather-related claims. Secondly to determine if average annual wind speeds lead to changes in weather-related loss .Thirdly to establish if the average changes in annual rainfall or precipitation levels lead to changes in weather-related claims. Quantitatively this study explored the relationship between climate change and weather losses in South Africa. Temperature, rainfall, and wind speed were the main weather variables analysed. Lack of properly recorded insured weather losses was the major challenge. Nonetheless, total economic weather losses were used as a proxy for insured weather losses.The analysis employed regression, cointegration and vector error correction models. Study findings showed that climate change is influencing weather losses. The existence of correlation and causality between weather variables and losses was also affirmed. Thus the insurance industry should comprehensively incorporate climate change into its business strategy to minimise exposure. / Business Management / M. Com. (Business Management)
