Adverse Cardiac Events and the Impaired Relaxation Left Ventricular Filling Pattern

Increasing diastolic dysfunction (DD) grade is associated with increased heart failure (HF). Patients with preserved ejection fractions and grade 1 DD may have left atrial dilatation, e′ < 8 cm/sec, increased left ventricular (LV) mass, or variable E/e′ ratios. The aim of this study was to test the hypothesis that LV hypertrophy or E/e′ ratio > 8 may be associated with a greater incidence of HF. Methods Two hundred twelve patients with grade 1 DD and ejection fractions > 50% were retrospectively studied. Group 1 comprised 108 patients with E/A ratios < 0.8, without LV hypertrophy, e′ < 8 cm/sec, and E/e′ ratios < 8. Group 2 comprised 104 patients with LV hypertrophy or E/e′ ratios > 8. Patients with incident HF and valvular or coronary disease were excluded. Using two-dimensional Doppler echocardiography, LV and left atrial volumes and transmitral spectral and tissue Doppler were analyzed. Medical records were examined for laboratory data, HF admissions, and all-cause mortality from 2004 to 2012. Results Despite similar ejection fractions, patients in group 2 had greater LV and left atrial volumes, LV mass index values, and E/e′ ratios (P < .01 for all). HF incidence was greater in group 2 (30 vs 4, P < .001). Combined HF or all-cause mortality was greater in group 2 (46 vs 14, P < .001). Multivariate analysis revealed that HF was associated with E/e′ ratio (P < .0001), systolic blood pressure (P = .0123), and LV mass index (P = .042). Combined HF or all-cause mortality was associated with E/e′ ratio (P < .0001), LV mass index (P = .009), and lower calcium channel blocker use (P = .0011). Conclusions HF alone or HF and all-cause mortality were increased in patients with grade 1 DD in the presence of LV hypertrophy or elevated LV filling pressures.

Independent Influences of Excessive Body Weight and Elevated Blood Pressure From Childhood on Left Ventricular Geometric Remodeling in Adulthood

Background Obesity and hypertension are two risk factors of left ventricular hypertrophy (LVH) in adults. We aimed to examine the impacts of body weight and blood pressure (BP) from childhood on adult LV geometric remodeling. Methods The study cohort consisted of 1256 adults aged 27–42 years who had 2–10 measurements of body mass index (BMI) and BP from childhood in 1987 to adulthood in 2010. We calculated the cumulative and incremental values of BMI and BP from childhood to adulthood. In adulthood, four LV geometric patterns were defined based on the values of left ventricular mass index (g/m2.7) and relative wall thickness: normal geometry, concentric remodeling (CR), eccentric hypertrophy (EH) and concentric hypertrophy (CH). Results The prevalence of abnormal LV geometric patterns in adults was 26.4% for CR, 2.0% for EH and 2.5% for CH. For childhood values, systolic BP (Odds Ratio [OR] = 1.26, 95% confidence interval [CI] = 1.08–1.47) but not BMI (OR = 1.06, 95%CI = 0.93-1.18) was associated with adult CR, whereas BMI (OR = 3.53, 95%CI = 2.09–5.98) but not systolic BP (OR = 1.04, 95%CI = 0.65–1.66) was associated with adult EH. Both childhood BMI (OR = 2.69, 95%CI = 1.77–4.09) and systolic BP (OR = 1.64, 95%CI = 1.07–2.51) were independently associated with adult CH. For adulthood, cumulative and incremental values, BMI and systolic BP were independently associated with adult CR, EH and CH. Conclusion Excessive body weight and elevated BP from childhood have independent influences on the development of adult LV geometric remodeling.

Background/objectives There are few studies investigating the long-term association between childhood blood pressure (BP) and adult cardiovascular remodeling. We seek to examine the effect of elevated childhood BP on cardiovascular remodeling in early or middle adulthood.Methods We used the "Beijing BP Cohort Study", where 1259 subjects aged 6-18 years old were followed over 24 years from childhood (1987) to early or middle adulthood (2011). Anthropometric measures and BP were obtained at baseline and follow-up examinations. Carotid-femoral pulse wave velocity (cfPWV), carotid intima-media thickness (cIMT), and left ventricular mass index (LVMI) were measured to assess cardiovascular remodeling in early or middle adulthood. Multiple logistic regression models were used to assess the odds ratio (OR) and 95% confidence interval (CI) for cardiovascular remodeling.Results 82 out of 384 children with elevated BP (21.4%) had adult hypertension. Compared to those with normal BP, children with elevated BP were at 2.1 times (95% CI: 1.4-3.1) likely to develop hypertension in early or middle adulthood. Compared to those with normal BP, children with elevated BP were at higher OR of developing high cfPWV (OR = 1.8, 95% CI = 1.3-2.4), high cIMT (1.4, 1.0-1.9), or high LVMI (1.4, 1.0-1.9) in early or middle adulthood. The ORs for remodeling (for any measures) were 1.4 (0.9-2.0) in early adulthood for children age 6-11 years, and 1.6 (1.1-2.4) in middle adulthood for those aged 12-18 years.Conclusions Children with elevated BP from 6 years old have accelerated remodeling on both cardiac and arterial system in early or middle adulthood.

Predictors of left ventricular hypertrophy, diastolic dysfunction and atrial fibrillation:the roles of adiponectin, ambulatory blood pressure and dietary sodium intake

Abstract Left ventricular hypertrophy (LVH), a common complication of elevated blood pressure (BP), is a risk factor for cardiovascular (CV) morbidity and mortality. Adiponectin has been shown to have cardioprotective effects and is inversely associated with LVH. BP can be measured at a clinical visit, as a momentary value. Ambulatory blood pressure (APB) measurement (ABPM) is a method of repeated BP measurements through a defined period, targeted to evaluate the circadian BP profile. High BP and ABPM have been shown to be associated with LVH and left ventricular diastolic dysfunction (LVDD). A high sodium intake has been associated with elevated BP and adverse CV outcome. The aim of this study was to investigate the associations between adiponectin and left ventricular mass index (LVMI), a measure of LVH, ABPM and the development of LVDD during long-term follow-up, ABPM and the change in LVMI during long-term follow-up, and the role of dietary sodium intake in the incidence of AF. Adiponectin has been shown to have vasoprotective, anti-inflammatory and cardioprotective effects. Hypoadiponectinemia has been associated with hypertension, coronary artery disease (CAD) and LVH. In this study, adiponectin levels were inversely associated with LVMI, even after adjustment with conventional risk factors of LVH, in a fairly large sample of middle-aged subjects. Elevated BP and pulse pressure (PP) have been associated with echocardiographic measures of LVDD. In this study, the association between APBM and the development of LVDD during a 20-year follow-up was evaluated. Ambulatory PP (APP) was shown to independently associate with the development of LVDD, even after adjustment with conventional risk factors of LVDD. APBM has been associated with LVH in cross-sectional assessments and has also been shown to have predictive value in future LVMI or LVH. In a few studies the predictive value of APP in future LVMI was observed. In the present study, an increase in APP was shown to predict the change in LVMI during long-term follow-up. In this study, the association between dietary sodium intake and the incidence of AF was evaluated. A high sodium intake predicted the occurrence of AF, which is a novel finding. In conclusion, this study offers novel findings about predictive factors in the entity of cardiac remodelling. / Tiivistelmä Vasemman kammion hypertrofia on yleinen kohonneen verenpaineen seuraus ja sen on todettu olevan sydän- ja verisuonitapahtumien riskitekijä. Adiponektiinin on osoitettu suojaavan vasemman kammion hypertrofialta. Ambulatorinen verenpaineen mittaus on menetelmä, jossa verenpaine mitataan määritellyllä ajanjaksolla toistuvasti, mikä antaa kuvan verenpaineesta vuorokauden eri jaksoissa. Kohonneella ambulatorisella verenpaineella on osoitettu olevan yhteys vasemman kammion hypertrofiaan sekä vasemman kammion diastoliseen vajaatoimintaan. Runsas natriumin saanti on yhteydessä kohonneeseen verenpaineeseen sekä sydän- ja verisuonisairauksiin. Tämän tutkimuksen tarkoituksena on selvittää yhteyksiä adiponektiinin ja vasemman kammion massaindeksin välillä, ambulatorisen verenpaineen ja vasemman kammion diastolisen vajaatoiminnan kehittymisen välillä, ambulatorisen verenpaineen ja vasemman kammion massaindeksin muutoksen välillä sekä natriumin saannin ja eteisvärinän ilmaantuvuuden välillä. Adiponektiinilla on todettu olevan suotuisia vaikutuksia verisuonistoon, tulehdusreaktion hillintään sekä sydänlihakseen. Matalan adiponektiinitason on osoitettu olevan yhteydessä verenpainetautiin, sepelvaltimotautiin sekä vasemman kammion hypertrofiaan. Tässä tutkimuksessa adiponektiinihormonilla osoitettiin olevan käänteinen yhteys vasemman kammion massaindeksiin, vaikka perinteiset riskitekijät otettiin huomioon. Kohonneella verenpaineella sekä pulssipaineella on osoitettu olevan yhteys vasemman kammion diastoliseen vajaatoimintaan. Tässä tutkimuksessa arvioitiin ambulatorisen verenpaineen merkitystä vasemman kammion diastolisen vajaatoiminnan kehittymisessä. Ambulatorinen pulssipaine osoittautui riskitekijäksi, vaikka perinteiset riskitekijät otettiin huomioon. Ambulatorisella verenpaineella ja pulssipaineella on osoitettu olevan yhteys vasemman kammion hypertrofiaan poikkileikkaustutkimuksissa ja seurantatutkimuksissa. Tässä tutkimuksessa ambulatorisen pulssipaineen nousun havaittiin ennustavan vasemman kammion massaindeksin kasvua pitkäaikaisseurannassa. Tässä tutkimuksessa korkean natriumin saannin todettiin olevan yhteydessä lisääntyneeseen eteisvärinän ilmaantuvuuteen. Tätä yhteyttä ei ole aiemmin todettu muissa tutkimuksissa. Tässä tutkimuksessa löydettiin uusia riskitekijöitä sydämen patologisiin ilmentymiin liittyen.

Études de réseaux d’expression génique : utilité pour l’élucidation des déterminants génétiques des traits complexes

Les traits quantitatifs complexes sont des caractéristiques mesurables d’organismes vivants qui résultent de l’interaction entre plusieurs gènes et facteurs environnementaux. Les locus génétiques liés à un caractère complexe sont appelés «locus de traits quantitatifs » (QTL). Récemment, en considérant les niveaux d’expression tissulaire de milliers de gènes comme des traits quantitatifs, il est devenu possible de détecter des «QTLs d’expression» (eQTL). Alors que ces derniers ont été considérés comme des phénotypes intermédiaires permettant de mieux comprendre l’architecture biologique des traits complexes, la majorité des études visent encore à identifier une mutation causale dans un seul gène. Cette approche ne peut remporter du succès que dans les situations où le gène incriminé a un effet majeur sur le trait complexe, et ne permet donc pas d’élucider les situations où les traits complexes résultent d’interactions entre divers gènes. Cette thèse propose une approche plus globale pour : 1) tenir compte des multiples interactions possibles entre gènes pour la détection de eQTLs et 2) considérer comment des polymorphismes affectant l’expression de plusieurs gènes au sein de groupes de co-expression pourraient contribuer à des caractères quantitatifs complexes. Nos contributions sont les suivantes : Nous avons développé un outil informatique utilisant des méthodes d’analyse multivariées pour détecter des eQTLs et avons montré que cet outil augmente la sensibilité de détection d’une classe particulière de eQTLs. Sur la base d’analyses de données d’expression de gènes dans des tissus de souris recombinantes consanguines, nous avons montré que certains polymorphismes peuvent affecter l’expression de plusieurs gènes au sein de domaines géniques de co-expression. En combinant des études de détection de eQTLs avec des techniques d’analyse de réseaux de co-expression de gènes dans des souches de souris recombinantes consanguines, nous avons montré qu’un locus génétique pouvait être lié à la fois à l’expression de plusieurs gènes au niveau d’un domaine génique de co-expression et à un trait complexe particulier (c.-à-d. la masse du ventricule cardiaque gauche). Au total, nos études nous ont permis de détecter plusieurs mécanismes par lesquels des polymorphismes génétiques peuvent être liés à l’expression de plusieurs gènes, ces derniers pouvant eux-mêmes être liés à des traits quantitatifs complexes. / Complex quantitative traits are measurable characteristics of living organisms resulting from the interaction between multiple genes and environmental factors. This thesis proposes a more comprehensive approach to: 1) take into account the possible interactions between multiple genes for the detection of eQTLs and 2) consider how polymorphisms affecting the expression of several genes in a module of co-expression may contribute to quantitative complex traits. Our contributions are as follows: We have developed a tool using multivariate analysis techniques to detect eQTLs, and have shown that this tool increases the sensitivity of detection of a particular class of eQTLs. Based on the data analysis of gene expression in recombinant inbred strains mice tissues, we have shown that some polymorphisms may affect the expression of several genes in domain of co-expression. Combining eQTLs detection studies with network of co-expression genes analysis in recombinant inbred strains mice, we showed that a genetic locus could be linked to both the expression of multiple genes at a domain of gene co-expression and a specific complex trait (i.e. left ventricular mass). Our studies have detected several mechanisms by which genetic polymorphisms may be associated with the expression of several genes, and may themselves be linked to quantitative complex traits.
