Qualitat visual dels pacients amb implantació de lent fàquica tipus ICL per la correcció de la miopia

INTRODUCCIÓ: La correcció de la miopia moderada i alta mitjançant la implantació de lent fàquica tipus ICL ha demostrat la seva seguretat, eficàcia i predictibilidad. La qualitat visual d'aquests pacients als que se'ls ha implantat una lent fàquica no ha estat, però, objecte d'estudis publicats en la literatura científica.OBJECTIUS: Demostrar que la qualitat visual dels pacients als que se'ls implanta una ICL millora, amb milloria de la sensibilitat al contrast, disminució en la puntuació del qüestionari subjectiu de qualitat visual i milloria en les aberracions d'alt ordre. També demostrar que la qualitat visual depèn de la edat dels pacients, diàmetre pupil·lar, miopia prequirúrgica, efectivitat quirúrgica i distància entre la lent i cristal·lí (vault).MATERIAL I MÈTODES: Prèviament a la cirurgia se ha realitzat un anàlisi de las aberracions oculars d'alt ordre mitjançant un aberròmetre ZyWave (Bausch&Lomb), així com una mesura de la sensibilitat al contrast i el qüestionari de qualitat visual Refractive Status and Vision Profile. S'ha emprat la lent ICL model V4 en tots els casos. Postquirúrgicament s'han realitzat controls als 3 i 6 mesos, mesurant l'aberració del front d'ona, la sensibilitat al contrast i el qüestionari subjectiu.RESULTATS: S'han estudiat 78 ulls de 40 pacients, amb una edat mitjana de 35,03±8,21 anys (rang 18-50), con una miopia mitjana de -8,47 ± 1,93 diòptries. S'ha observat un augment significatiu de la sensibilitat al contrast en totes las freqüències excepte 18 cicles/grau. S'ha observat una disminució significativa en la puntuació del qüestionari als 3 mesos excepte en la subescala "glare" que ho és als 6 mesos. Les aberracions totals d'alt ordre disminueixen no significativament. El coma disminueix no significativament, però sí que ho és l'augment del trefoil. L'aberració esfèrica disminueix i el quadrafoil augmenta, ambdós amb significació estadística. De la mateixa manera, el pentafoil augmenta significativament, però les modificacions de coma secundari i trefoil secundari no són significatives.DISCUSSIÓ: Hi ha pocs estudis publicats que avaluïn la qualitat òptica de les lents fàquiques. Alhora, els pocs estudis existents presenten grans diferències en els mètodes exploració de les aberracions d'alt ordre (cicloplègia/no-cicloplègia, amb ulleres, lents de contacte...). D'altra banda, cal tenir en compte que no existeix una relació directa entre el valor total de les aberracions oculars (RMS) i la qualitat òptica, per possible compensació de certes aberracions entre elles. La qualitat òptica també es veu modificada per l'aberració cromàtica. La dependència de la qualitat òptica del diàmetre pupil·lar coincideix amb la majoria de publicacions, però no així la no dependència de l'edat . Una possible explicació és l'estret rang d'edat d'aquests pacients. CONCLUSIONS: La qualitat visual dels pacients millora, amb millor sensibilitat al contrast, millor puntuació en el qüestionari i disminució de l'aberració esfèrica (Z400). Hi ha hagut increment del trefoil, quadrafoil i pentafoil, i absència de variació de la resta d'aberracions d'alt ordre. La qualitat visual d'aquests pacients depèn del diàmetre pupil·lar, però no de l'edat, ametropia prequirúrgica, efectivitat de la tècnica ni del vault. La sensibilitat al contrast, el qüestionari RSVP i les aberracions d'alt ordre són un bon indicador de la qualitat visual i la satisfacció totals dels pacients. El RSVP preoperatori és un bon predictor de la qualitat visual després de la cirurgia. / INTRODUCTION: The correction of moderate to high myopia after the implantation of an ICL phakic intraocular lens (IOL) has shown its safety, efficacy and predictability. The quality of vision of these patients has not been extensively studied and published yet. PURPOSE: To prove that visual quality after ICL implantation improves, with better contrast sensitivity, lower score on the subjective questionnaire and improvement of the higher order aberrations. Also to prove that quality of vision depends on the age of patients, pupillary diameter, preoperative myopia, surgical effectivity and on the vault.MATERIAL AND METHODS: Before the surgery all the patients had a measurement of the higher order aberrations, with a ZyWave aberrometer (Bausch&Lomb), as a contrast sensitivity measurement and the Refractive Status and Vision Profile (RSVP) Questionnaire. A model V4 ICL has been used in all the cases. After the surgery, there has been a follow-up on the 3rd and 6th month, measuring the wavefront aberration, contrast sensitivity and RSVP Questionnaire. RESULTS: 78 eyes from 40 patients have been studied. The mean age is 35,03±8,21 years (18-50), with mean preoperative myopia of -8,47 ± 1,93 dioptres. There has been a statistically significant improvement of the contrast sensitivity in all frequencies, except 18 cycles/degree. Also there has been a significant decrease on the score of the RSVP Questionnaire on the 3rd month, except on the subscale "glare" which has decreased with statistical significance on the 6th month. Total higher order aberrations have decreased though without statistical signification. Third order coma has decreased without statistical significance, but the increase of the third order trefoil has been significant. Spherical aberration (Z400) has decreased and quadrafoil has increased, both significantly. Seemingly, pentafoil has increased with statistical significance, but variations on fifth order secondary coma and secondary trefoil have not been significant. DISCUSSION: There are very few studies assessing the optical quality of the phakic IOL. Also, there are huge differences between these few papers on the higher order aberration measurement methods (cycloplegic / non cycloplegic, with spectacles, with contact lenses.). On the other hand, we know that the correlation between the total amount of higher order aberration (RMS) and the optical quality is not totally direct, because there is some kind of compensation between some aberrations. Optical quality is also modified by chromatic aberration. There is a high coincidence with most of the publications about the correlation of the optical quality with the pupillary diameter, though not about the correlation the age. In our study, visual quality does not depend on the patient's age probably because of the narrow range of ages (18-50).CONCLUSIONS: Visual quality of the patients improves, with better contrast sensitivity, better RSVP Questionnaire score and spherical aberration decrease. There has been an increase on the trefoil, quadrafoil and pentafoil, with no variation of the rest of higher order aberrations. The quality of vision of these patients depends on the pupillary diameter, but not on age, preoperative ametropy, technical effectivity nor on the vault. Contrast sensitivity, RSVP Questionnaire and higher order aberrations are good signs of the visual quality and patient satisfaction. Preoperative RSVP Questionnaire is a good predictor of the postoperative visual quality.

From a Home in the Suburbs to a Retreat in the Wilderness: The Domestic Architecture of Frank T. Lent

The idea of home, the ownership of property, and the impact of the home on the moral character and identity of its inhabitants were important concepts in the late nineteenth century. These views were perpetuated by a wide range of supporters including writers, religious leaders, social reformers, politicians. Architects and developers became aware of these ideas and capitalized on the hopes and dreams of middle-class North America by designing, marketing and building the right kind of houses, but perhaps more importantly, by building them in the right location which most often meant the suburban areas around major cities. Architect and writer Franklin Townsend Lent (1855-1919) is but one of many architects practicing in North America who appreciated the contemporary consumers’ sensibilities, and their attachment to their homes. Not only did Lent understand the importance of the concept of home, he was able to develop a design vocabulary that drew on the contemporary fascination with the American colonial period. In addition, he appreciated the consumer’s desire for a beautiful home in the suburbs, and took advantage of this in his work in New Jersey notably on the development of a suburban neighbourhood called Roosevelt Manor. Lent contributed to the built environment by designing and building many suburban houses, island and seaside cottages, and other structures. He also wrote three books and several pamphlets that provide an understanding of his personal contribution to architecture in suburban American, and to the early development of resort architecture in the Thousand Island region of Upstate New York and Ontario, an area that has received very little scholarly attention. This paper will focus on the domestic architecture of Lent in an attempt to construct an understanding of this unique contribution in the context of late nineteenth century and early twentieth century society by examining his published architectural writings, his advertisements, and some representative houses that he designed and built in the United States and Canada. / Thesis (Master, Art History) -- Queen's University, 2012-10-12 14:16:06.91

Optical near-field characterization of Slow-Bloch Mode based photonic crystal devices / Nanopointes actives pour la nano-photonique

Les structures à cristaux photoniques bidimentionnels ont permis la fabrication d'une large variété de composants nanophotoniques. En particulier, dans les PC parfaits, la densité locale d'états optiques se trouve exacerbée au niveau des points de symétrie du diagramme de bande. Près de ces points, où la vitesse de groupe tend vers zéro, la faible courbure de bande courbure de bande donne lieu à des modes stationnaires, dits de lumière "lente" (ou modes de Bloch lents). Les propriétés de la lumière lente en font de bons candidats pour améliorer l'effet Purcell, pour produire des effets non-linéaires ou pour concevoir des lasers à faible seuil. Parmi ces modes, les modes de Bloch lent émettant dans la direction verticale, c'est à dire situé au niveau du point Γ de la zone de Brillouin, sont particulièrement intéressants pour intégrer des architectures 2D avec de l'optique en espace libre. En particulier, certains de ces modes ont permis la réalisation d'émission laser dans la 3e direction avec des propriétés de polarisation particulières. D'autres applications prometteuses concernent le désordre : en introduisant un désordre aléatoire mais contrôlé dans la structure photonique, il est possible d'induire une transition entre le mode de Bloch lent de la structure ordonnée vers un mode localisé par le désordre, de type localisation d'Anderson dans une structure faiblement désordonnée.Dans cette thèse, les modes de Bloch lents ont été étudiés et caractérisés en Microscopie optique en champ proche. Nous nous sommes concentrés sur les modes en Gamma de la structure graphite. Le SNOM a permis de visualiser la composante évanescente du mode avec une résolution spatiale inférieure à la limite de diffraction. Dans ce travail, nous avons montré que le champ lointain et de l'image en champ proche du mode à la surface du cristal photonique sont différentes et que seules les mesures en champ proche permettent de rendre compte du mode réel à l'intérieur de la membrane de cristal photonique, en accord avec prédiction théorique. L'importance du choix de la sonde (silice, pointe métallisée, Nano-antenne) pour l'étude des structures à cristaux photoniques a également été démontrée. Outre la mesure d'intensité du champ électromagnétique, la polarisation du champ électrique a été mesurée à l'échelle nanométrique pour la première fois par l'aide d'une antenne à ouverture papillon. Ces résultats permettent d'identifier sans équivoque des modes avec les simulations 3D-FDTD.Dans ce travail est également rapporté la première observation de la localisation de la lumière dans deux types de lasers aléatoires à base de cristaux photoniques bidimensionnels. Le caractère aléatoire est introduit soit en déplaçant les positions des motifs du cristal (trous d'air), soit en faisant varier de façon aléatoire le diamètre des trous. Pour la première fois nous avons observé directement par SNOM la localisation de la lumière dans le cristal désordonné. Cela nous a permis d'observer la transition de morphologie du mode de Bloch lent entre le cristal ordonné et le cristal désordonné. / 2D-Photonic crystal (PC) structures have enabled the fabrication of a wide variety of nanophotonic components. In perfect PCs, the exploitation of the enhanced local density of states at critical points of the band diagram has attracted considerable attention. Near these points, where the group velocity vanished, low curvature flat bands give rise to delocalized and stationary optical slow Bloch modes (or slow light modes). Properties of slow light make them good candidates to enhance Purcell or various non-linear effects or to design low-threshold lasers. Among these modes, slow Bloch modes (SBMs) emitting in the vertical direction, i.e. located at the Γ- point of the Brillouin zone are particularly interesting for integrating 2D PC architectures with free space optics. In particular, some SBMs proved to be suitable for achieving strong vertical emission with peculiar polarization properties. Other promising applications concern disorder: by introducing a controlled randomness into the PC structure, it is possible to induce a transition from slow Bloch mode (in ordered PC) to Anderson’s localization (in disordered PC) as a function of disorder degree. In this PhD dissertation, Slow Bloch modes have been studied and characterized by the means of Near-field Scanning Optical microscopy (NSOM). We particularly focused on Slow Bloch laser mode at Γ- point of a honeycomb 2DPC. This NSOM technique enables to visualize the evanescent component of the mode with a spatial resolution below the diffraction limit. In this work, we showed that the far-field and the near-field image of the mode at the 2D-PC surface are different and that near-field results yield a better insight in the real mode structure inside the PC slab in agreement with theoretical prediction. The importance of the probe selection (bare silica, metallized tip and bow-tie aperture nanoantenna) for studying III-V photonic crystal structures was also demonstrated. Besides intensity measurement of the electromagnetic field, the polarization of the electric field has been measured at the nanoscale for the first time by using a bow-tie nano-antenna probe. These results enable the unambiguous identification of the modes with the 3D-FDTD simulations.In this work is also reported the first observation of two-dimensional localization of light in two types of 2D random photonic crystal lasers, where Slow Bloch Mode (SBM) is scattered by artificial structural randomness in triangular PCs. The structural randomness is introduced whether by nanometer displacements in the positions of lattice elements (air holes), whether by variation of the hole diameters. The direct near-field imaging of the lasing mode by use of NSOM for the first time, allowed us to observe the transition of the extended planar SBM to be Anderson localized.
