Um material selador ideal deve possuir boas propriedades físicas, químicas e biológicas, sendo, o Mineral Trióxido Agregado (MTA) um dos materiais indicados. Assim, o objetivo desse estudo foi avaliar a resposta biológica de três materiais seladores: MTA-Angelus branco® (MTA), Endo-C.P.M.-Sealer® (CPM) e cimento de óxido de zinco e eugenol (OZE). Para isso, foram utilizados os primeiros molares superiores de 60 ratos; o primeiro molar superior do lado esquerdo hígido foi utilizado como controle e o do lado direito foi realizada a perfuração no assoalho da câmara pulpar com uma fresa nº ¼. Os materiais foram manipulados e inseridos na perfuração e a cavidade de acesso foi selada com ionômero de vidro. Decorrido os períodos de 07, 15, 30 e 60 dias, os animais foram sacrificados e os fragmentos das maxilas removidos e fixados em formaldeído. Após a descalcificação em EDTA, os fragmentos de maxila foram desidratados, diafanizados e incluídos em parafina. Os cortes foram corados com hematoxilina & eosina (H&E), tricrômico de Masson ou submetidos ao método do TRAP (“Tartrate Resistant Acid Phosphatase”) para realização das análises morfológica e morfométrica. A análise morfométrica foi realizada sob três parâmetros: espessura do ligamento periodontal (LP), número de células inflamatórias/mm2 (CI) e número de osteoclastos TRAP-positivos. Os resultados morfométricos revelaram aumento significante (p_0,05) no espaço do LP,em todos os períodos, em relação ao controle. Entretanto, aos 60 dias, este espaço foi menor em todos os grupos experimentais; o OZE foi o que provocou maior espessamento do LP, não havendo diferenças significantes... / The therapy success in the treatment of root perforations depends on the material used to fill them. The best sealing material should present good physical, chemical and biological properties. More recently, the mineral trioxide aggregate (MTA) has been indicated for sealing of root perforations. Thus, the aim of the present study was to evaluate the biological response of three sealing materials: White MTA-Angelus (MTA), Endo- C.P.M.-Sealer (CPM) and zinc-oxide and eugenol cement (OZE). Sixty adult rats weighing 250g were divided into three groups: MTA, CPM and OZE. Furcal perforations were performed in maxillary first molars with a round bur no. ¼; the contra-lateral maxillary first molars – without perforations - were used as control. The perforations were carried out and the materials were placed into these defects; access cavities were sealed with glass ionomer. After 07, 15, 30 and 60 days, the animals were killed, and fragments of maxilla containing first molars were removed and immediately immersed in 4% formaldehyde. After decalcification in EDTA, the specimens were processed and embedded in paraffin. Sections were stained with hematoxylin-eosin, Masson’s trichrome and submitted to TRAP (Tartrate Resistant Acid Phosphatase) reaction. The morphometric analysis, in the furcation region of the periodontal ligament, was performed considering three parameters: thickness of periodontal ligament (PL), number of inflammatory cells (IC) and number of TRAP-positive osteoclasts in the alveolar bone surface. The experimental groups showed, in all periods, significant increase (p_0.05) in the PL thickness in comparison to control group; this thickness was more accentuated in the OZE group. The periodontal space in the CPM group was less thick in all experimental periods in comparison to MTA and OZE groups, except in the period of 60 days; in this period, statistical differences in the periodontal space were not detected between CPM and MTA groups.

Le système de soutien ligamentaire du col de l’utérus et de l’apex vaginal / Supporting ligaments of the uterine cervix and vaginal apex

Le col de l'utérus et l'apex vaginal sont soutenus par le ligament utérosacral (LUS) et cardinal (LC). Le LUS s'étend de la région autour des vertèbres sacrées S2-S4 vers la face dorsale du col de l'utérus et/ou du tiers supérieur de la paroi vaginale postérieure. Il contient plus de fibres nerveuses que le LC. Ce dernier est défini comme le fourreau peri-vasculaire s'insérant au niveau proximal autour de l'origine de l'artère iliaque interne, et au niveau du col et/ou le vagin en distal. L'étude de la géométrie spatiale des ligaments chez la femme vivante à l'IRM et après reconstruction 3D a permis d'identifier le LC sous la forme d'un maillage fibreux centré autour de l'axe des vaisseaux iliaques internes, partant dans une direction crânio­caudale vers l'utérus, le col, la vessie et le tiers supérieur du vagin. Les fibres du LC et du LUS convergent vers le col de l'utérus et le tiers supérieur du vagin. La reconstruction 3D a confirmé que le LC fonctionne dans une direction crânio­caudale et le LUS dans une direction dorso-ventrale. L'angle moyen entre le LC et l'axe corporel est de 18°, alors que l'angle moyen entre le LUS et l'axe corporel est de 92°. Le LC est plus long et plus courbé que le LUS. La tension théorique calculée au niveau du LC est de 52% plus importante que celle au niveau du LUS. En cas de prolapsus, il existe une élongation ligamentaire significativement plus importante lors de la manœuvre de Valsalva Des distorsions géométriques majeures sont constatées après cure de prolapsus par promontofixation, et sont plus marquées pour le LC / The uterine cervix and vaginal apex are supported by the uterosacral (USL) and cardinal (CL) ligaments. The USL extends from the presacral fascia between sacral vertebrae S2-S4 towards the dorsal margin of the uterine cervix and the superior third of the posterior vaginal wall. It contains more nervous fibers than the CL. The latter is defined as the perivascular sheath around the internai iliac artery, inserting distally into the cervix and the vagina The geometrical study of the ligaments in living women using MRI and 3D reconstruction shows the CL as a web-like structure around the internai iliac vessels, and going in a cranial to caudal direction towards the cervix and upper third vagina where fibers ofboth the CL and USL mingle together. The USL operates in a dorsal to ventral direction. The mean angles between the body axis and the CL and USL are respectively at 18° and 92°. The CL is longer and more curved than the USL. Theoretical calculated tension on the CL is 52% larger than on the USL. In case of pelvic organ prolapse, significant ligament elongation is observed during Valsalva maneuver. Major geometrical distortions exist after prolapse surgery by sacrocolpopexy, specially for the CL

Occlusal Displacement of Teeth Due to Flexure of the Mandible

Indiana University-Purdue University Indianapolis (IUPUI) / It has been proposed that jaw deformation during function may be a contributor to supraosseous tooth eruption. This could be through a transient pressure gradient in the flexed bone and/or tension in the PDL fibers produced by socket deformation. Thus, the purpose of this study was to determine if forces applied to a fresh frozen canine mandible can lead to occlusal displacement of a tooth. Seven hemisected canine mandibles (14 specimens) were used to test this hypothesis. A force cycle (10 N to 100 N to 300 N) was applied by a Bionix testing machine (MTS TM Systems Corp., Minneapolis, Minn.) to the mandible in three point bending. Digital radiographs were taken at each change in force level using computer digitized radiography (CDR, TM Schick Technologies, Inc., Long Island City, N.Y.). By measuring the relative movement of metal markers (1 mm steel balls) placed into the mandible and the crown of the 2nd premolar, the amount of tooth eruption was calculated. With cyanoacrylate cement (Archer® Instant Bonding Adhesive, Tandy Corp, Ft. Worth, Texas) injected into the PDL space of the same tooth, the test was repeated one week later. These "ankylosed" teeth served as controls. The eruption distances were compared between the experimental and the ankylosed teeth by means of repeated measures analysis of variance. The only significant occlusal displacement was noted as the force was initially raised to 100 N (21.7 ± 40.6 μm). Due to the possible breakdown of the cyanoacrylate cement, the ankylosed teeth also showed evidence of eruption with the continued application of force. Although significant eruption was noted, the displacements were very small. PDL degradation, viscoelastic behavior, measurement of small displacements, limited resolution radiography, and two dimensional analysis are contributing factors to the uncertainty of the results. Further investigation is required to determine if jaw deformation during function is a viable mechanism leading to the occlusal displacement of teeth.

Ruby Joint Stabilization System as a Suitable Method of Extracapsular Repair

Objective: To characterize the effect of the Ruby Joint Stabilization System (Ruby) on the motion of the cranial cruciate ligament (CrCL) deficient stifle. To compare the motion with the Ruby to that of the CrCL-intact and CrCL-deficient stifle. Study Design: Each canine pelvic limb was mounted in a loading jig under 30% body weight. Motion data was collected using an electromagnetic tracking system at stifle angles of 125o, 135o and 145o with the CrCL-intact, CrCL-deficient and the Ruby applied. Results: Total translation of the CrCL-deficient stifle following the Ruby was reduced, but remained greater than the CrCL-intact stifle at angles of 125o, 135o and 145o. Internal rotation of the Ruby groups was greater than the CrCL-intact group at 145o, but not 125o and 135o. Varus motion of the Ruby group was decreased compared to the CrCL-deficient group, but increased compared to the CrCL-intact group at angles of 125o, 135o and 145o. Conclusion: Total translation and internal rotation of the CrCL-deficient stifle following the Ruby differed from that of the CrCL-intact stifle. However, the Ruby reduced total translation and internal rotation of the tibia relative to the femur in the CrCL-deficient stifle to levels that may yield clinically acceptable results. / Master of Science / Cranial cruciate ligament disease is a common pathology of the canine stifle. Loss of this ligament results in instability of the stifle that results in pain and osteoarthritis, and can lead to damage of other intra-articular structures like the menisci. An abundant number of surgical procedures are described, with the goal of surgery being the restoration of normal stifle stability and function. A common surgical procedure for treatment is the lateral suture technique, which is an extracapsular method of stabilization. This procedure faces many complications; however, it remains a popular choice of stabilization due to its lower cost and less invasive nature. The Ruby Joint Stabilization procedure is a method of extracapsular repair that aims to restore stifle stability and circumvent several complications that plague the lateral suture. This cadaveric study sought to investigate how stifle motion of the normal canine stifle compared to that of the cranial cruciate ligament deficient stifle with the Ruby Joint Stabilization System applied. The results of this investigation demonstrated that the Ruby Joint Stabilization System adequately restored stifle motion to a level that could yield clinically acceptable results, as was demonstrated in a previously published clinical investigation of this technique.
