Impacto da cirurgia bari?trica (bypass y-de-roux) nos componentes da s?ndrome metab?lica e no uso de medicamentos associados

Bariatric surgery for bypass technique, however, has shown promising and effective results in both, reducing weight and associated comorbidities of obese patients II and III. Objective: to evaluate the impact of gastric bypass in MetS components and in the use of associated drugs.Method : historical cohort, follow-up for six months, analyzing a sample composed by 273 obese people [236 (86.4%) female and 37 (13.6%) males] grade II and grade III, aged ? 20 years, with or without MetS who underwent bariatric surgery by Roux-en-Y bypass technique between January 2007 and March 2014. All patients were followed for a period of six months after surgery and evaluated at 60, 120 and 180 days after surgery. The socio-demographic, anthropometric, biochemical, clinical and medication use data were collected from medical records of patients.Results : the results show significant statistical differences in post-operative period, either 60, 120 or 180 days, concerning the following variables: weight (p <0.001), BMI (p <0.001), CA (p <0.001), SAP (p <0.001), DBP (p <0.001), glucose (P = 0.005 post 60 and p <0.001 post 120 and 180 days), triglycerides (p <0.001). In relation to HDL levels did not change was observed (p> 0.05). Concerning the use of drugs, were also observed significant statistical reductions in the use of antihypertensive drugs (p <0.001), hypoglycaemics (p <0.001) and lipid-lowering (p <0.001).Conclusions : that patients who underwent Roux-en-Y gastric bypass had a weight loss and an improvement in MetS components (except HDL) and in the use of drugs associated with obesity and MetS. / Introdu??o : embora a preval?ncia da obesidade e da s?ndrome metab?lica (SM) esteja aumentando mundialmente, terapias de modifica??o comportamental (dieta e pr?tica de exerc?cio f?sico regular) e a farmacoterapia apresentam pouco sucesso e baixa ades?o. A cirurgia bari?trica pela t?cnica de bypass (Y-de-Roux) tem mostrado resultados promissores e eficazes tanto na redu??o do peso quanto nas comorbidades associadas de pacientes obesos II e III. Objetivo: avaliar o impacto do bypass g?strico (Y-de-Roux) nos componentes da SM e no uso de medicamentos associados.M?todos : coorte hist?rica, follow-up de seis meses. A amostra foi composta por 273 indiv?duos obesos [236 (86,4%) do sexo feminino e 37 (13,6%) do sexo masculino] grau II e grau III, com idade ? 20 anos, portadores ou n?o de SM e que realizaram cirurgia bari?trica pela t?cnica de bypass (Y- de- Roux) entre janeiro de 2007 e mar?o de 2014. Todos os pacientes foram acompanhados por um per?odo de seis meses ap?s o procedimento cir?rgico, com avalia??o em 60, 120 e 180 dias ap?s a cirurgia. Os dados s?cio-demogr?ficos, antropom?tricos, bioqu?micos, cl?nicos e de uso de medicamentos foram coletados dos prontu?rios m?dicos dos pacientes.Resultados : foram verificadas diferen?as estatisticamente significativas no p?s-cir?rgico, ap?s 60, 120 e 180 dias, no peso (p<0,001), no IMC (p<0,001), na CA (p<0,001), na PAS (p<0,001), na PAD (p<0,001), na glicemia (p=0,005 no p?s 60 e p<0,001 no p?s 120 e 180 dias) e nos triglicer?deos (p< 0,001). Em rela??o aos n?veis de HDL, n?o se observou altera??o (p>0,05). Quanto ao uso de medicamentos, tamb?m foram observadas redu??es estatisticamente significativas no uso de anti-hipertensivos (p<0,001), hipoglicemiantes (p<0,001) e hipolipemiantes (p<0,001).Conclus?o : pacientes submetidos ao bypass g?strico (Y-de-Roux) apresentaram perda de peso e melhora significativas nos componentes da SM (exceto no HDL) e no uso de medicamentos associados ? obesidade e ? SM.
