Health maintenance organizations for British Columbia : are they feasible?

This policy/planning thesis takes the format of a hypothetical study done for the Senior Assistant Deputy Minister in the B.C. Ministry of Health. The Ministry had shown interest in restructuring as a means of setting some boundaries on an open-ended medical care system in order to reduce expenditures for health care services. Because the clients' specific interest was in the American health maintenance organization (HMO) model, the study was concerned with the feasibility of introducing this model into the health care structures of B.C. The problem situation of increasing expenditures for health care services in B.C. was described and relevant systems involved in the situation were identified as: the Ministry of Health, the medical profession and hospitals. The unique perspectives of each system were described so their implications for the feasibility of an HMO model could be assessed. The HMO model, in the American context, was analyzed according to its generic elements and variant characteristics. In addition, the policy process of developing and implementing the HMO strategy was described. The HMO was found to be a highly complex organization that integrates financial mechanisms and service delivery. Evidence reviewed about its performance indicated that HMOs are a less expensive means of providing care than fee for service practice, that hospitalization rates range from 20 to 40% lower and are the primary source of HMO cost saving and that enrollees probably receive comparable quality care. The publicly funded health insurance system presents a primary obstacle to adopting this model to a Canadian setting because of weakened financial incentives for competition. The principles upheld by the program also hamper enrolling a fixed population which is a basic HMO element. To implement an HMO model in B.C., considerable restructuring of financial systems would be necessary to redirect funds to an HMO so that it could be at financial risk for providing hospital and medical services to an enrolled population. In reviewing some policy options, it was apparent that an HMO model would be most easily adapted to B.C. within the context of publicly funded competition in medical care practice. However, there did not appear to be sufficient support from relevant constituencies for such a comprehensive approach. Protecting Usonia: A homeowner's and site manager's resource for understanding and addressing common preservation concerns in Frank Lloyd Wright's Usonian home

The primary goal of this thesis is to provide owners and stewards of Usonian homes with as many possible resources to make informed maintenance and conservation decisions, believing that in giving these resources to those most involved with the process these extant Usonian homes will be preserved to the highest degree of integrity. It was vital that most of the information come from homeowner’s themselves, as they have the most current, in-depth and practical knowledge about common preservation concerns in the Usonian style of home. As a part of this goal first hand knowledge from the Usonian homeowners were collected, analyzed, and synthesized to develop a usable resource based on their experiences and additional research and recommendation from professionals in the field. The secondary goal of this project is to contribute to the current database of the Frank Lloyd Wright Building Conservancy on Usonian homes. The results of the survey will be included in the database and will contribute to their mission. The Frank Lloyd Wright Building Conservancy works to provide in-depth resources for homeowner’s as well as serves as one of the premier advocates for the preservation of Wright’s extant buildings. As such, they offer extensive practical preservation resources for all Wright homeowners. Computerized Maintenance Management Systems: Misconceptions Between Management & Operations

abstract: Research findings have shown that many computerized maintenance management systems (CMMS) are largely underutilized, often leading to the loss of efficiencies in the organization’s maintenance program. A literature review is presented of the available research in CMMS and of operations and management roles in a maintenance program. In addition, research was conducted around CMMS users to identify if any misalignments exist between management and operations. The articles selected for review offer a variety of perspectives, considerations, instructions, and noted failures involved with implementation, day to day use and reporting expectations. Through conducting a survey of both management and operations this paper will show how management and operations conceptions of CMMS vary, even greatly in some areas. The objective of this research is to gain an in-depth perspective from CMMS in all roles and analyze where utilizations vary. On Optimal Maintenance Management for Wind Power Systems

Sound maintenance strategies and planning are of crucial importance for wind power systems, and especially for offshore locations. In the last decades, an increased awareness of the impact of human living on the environment has emerged in the world. The importance of developing renewable energy is today highly recognized and energy policies have been adopted towards this development. Wind energy has been the strongest growing renewable source of energy this last decade. Wind power is now developing offshore where sites are available and benefits from strong and steady wind. However, the initial investments are larger than onshore, and operation and maintenance costs may be substantially higher due to transportation costs for maintenance and accessibility constrained by the weather. Operational costs can be significantly reduced by optimizing decisions for maintenance strategies and maintenance planning. This is especially important for offshore wind power systems to reduce the high economic risks related to the uncertainties on the accessibility and reliability of wind turbines. This thesis proposes decision models for cost efficient maintenance planning and maintenance strategies for wind power systems. One model is proposed on the maintenance planning of service maintenance activities. Two models investigate the benefits of condition based maintenance strategies for the drive train and for the blades of wind turbines, respectively. Moreover, a model is proposed to optimize the inspection interval for the blade. Maintenance strategies for small components are also presented with simple models for component redundancy and age replacement. The models are tested in case studies and sensitivity analyses are performed for parameters of interests. The results show that maintenance costs can be significantly reduced through optimizing the maintenance strategies and the maintenance planning.

La siguiente propuesta de implementación está formada por cuatro capítulos, el primero describió el marco teórico necesario para desarrollar y sustentar la presente investigación. Se incluyeron temas como el proceso de generación eléctrica, estrategias de mantenimiento, así como la gestión de procesos aplicada al entorno del mantenimiento. En el segundo capítulo, se describió la situación de la empresa y su entorno en el mercado eléctrico nacional. Además, se describieron los objetivos estratégicos y los procesos de la compañía. También, se determinó la problemática actual del departamento de mantenimiento y se pudo determinar cuál es la causa raíz de este problema. En el tercer capítulo, se determinaron los objetivos y se propusieron las metas a alcanzar. Además, se seleccionó la mejor metodología para solucionar el problema para eliminar, controlar o mitigar las causas que lo originaron. Seguidamente, se comenzó a desarrollar de la metodología el RCM para los sistemas seleccionados. Con ello, se determinó los mejores planes y estrategias de mantenimiento y los recursos necesarios para poder eliminar y/o controlar adecuadamente fallas en los equipos más críticos garantizando la disponibilidad de la planta. En el Cuarto capítulo, se implementó un tablero de mando el cual controlará que los indicadores de mantenimiento se encuentren dentro de los límites establecidos. Esto permitirá medir y controlar los principales indicadores como la disponibilidad (D), tiempo medio entre fallas (TMEF) y tiempo medio para reparar (TMPR). Finalmente, se realizó el análisis económico del proyecto calculando el VAN, TIR para poder determinar su viabilidad. / The following implementation proposal is made up of four chapters, the first one describing the theoretical framework necessary to develop and sustain this research. Topics such as power generation process, maintenance strategies, as well as the management of processes applied to the maintenance environment were included. In the second chapter, the situation of the company and it’s environment in the national electricity market was described. In addition, the company's strategic objectives and processes were described. Also, the current problem of the maintenance department was settled, and the root cause of this problem could be determined. In the third chapter, the objectives were established and the goals to be achieved were proposed. In addition, the best methodology was selected to solve the problem to eliminate, control or mitigate the causes that originated it. Next, the RCM for the selected systems began to be developed from the methodology. With this, the best maintenance plans and strategies and the necessary resources were determined to be able to eliminate and / or adequately control failures in the most critical equipment guaranteeing the availability of the plant. In the Fourth chapter, a dashboard was implemented which will control that the maintenance indicators are within the established limits. This will allow measuring and controlling the main indicators such as availability (A), mean time between failures (MTBF) and average time to repair (MTTR).
