Avaliação do risco cardiovascular em indivíduos com periodontite

Tanto o mecanismo aterosclerótico envolvido na DCV como a P podem possuir como fator etiológico agentes infecciosos. Ainda não há um consenso sobre a associação entre a P e a DCV. Assim investigamos tal associação através de fatores de risco cardiovascular tradicionais (FRT) e não tradicionais (FRNT). Para tanto, indivíduos de ambos os sexos, não fumantes e aparentemente saudáveis constituíram um grupo com periodontite crônica generalizada (n=30), na faixa etária de 46 ± 6 anos de idade e o grupo sem P (n=30) com idade média de 43 ± 5 anos. A P foi bem caracterizada, pois se observou diferença significante entre os grupos para os parâmetros: profundidade de sondagem, nível de inserção clínica, sangramento à sondagem, índice de placa visível e índice de extensão e severidade, sendo obtido valores maiores para o grupo com P. Dentre os FRT, avaliamos as pressões arteriais, o índices de massa corpórea, triglicérides, colesterol total (CT) e lipoproteínas, os índices de risco coronariano: escore de risco de Framingham e as proporções de CT/HDL-colesterol e LDLcolesterol/ HDL-colesterol. Como FRNT analisamos o índice cintura/quadril, a circunferência abdominal, a proteína C reativa e a microalbuminúria. Investigamos também a função renal (FR) de ambos os grupos através dos compostos nitrogenados não protéicos séricos e urinários, da filtração glomerular renal e pela proporção albumina/creatinina. As análises estatísticas foram aplicadas em relação ao controle e ao intervalo de referência e revelaram um quadro... / Periodontitis (P) and cardiovascular disease (CVD) are chronic diseases prevalent in the adult population. Several infectious microorganisms are known to be etiological factors in both the atherosclerotic process involved in CVD and P. There is still a lack of consensus regarding whether P and CVD are associated. Hence, we investigated this association, by assessing both traditional (TRF) and non-traditional (NTRF) cardiovascular risk factors. The experimental group (n=30) were non-smoking adults of both sexes apparently in good general health, with generalized chronic periodontitis, aged 46 ± 6 years, the control group (n=30) being similar but without P, aged 43 ± 5 years. The P symptoms were welldefined, with significantly higher values of the following variables in the first group: probing depth (PD), clinical attachment level (CAL), bleeding on probing (BP), plaque assessment index (PAI) and extent and severity index (ESI). The TRF used were blood pressure, body mass index, serum triglycerides, total cholesterol (TC) and lipoproteins and the coronary risk indices: Framingham cardiovascular risk score and TC/HDL-C and LDL-C/HDL-C ratios. The NTRF analyzed were the waist/hip ratio and waist circumference, serum C-reactive protein and microalbuminuria. We also evaluated the kidney function (KF) in each group by analyzing the levels of non-protein nitrogen compounds in blood serum and urine, renal glomerular filtration rate and the albumin/creatinine ratio in the urine. Statistical tests were used to compare the experimental group with the control group and with the reference intervals, revealing diastolic and systolic prehypertension in the experimental group, both of which were correlated with P (p=0.005).
