Constructing reform in the Ethiopian healthcare system : unintended consequences for hospitals and patients

In the last decade, the reach of New Public Management (NPM) has stretched well beyond its Western origins as modernising African governments and their global health partners have sought to import new approaches. Public health systems in Africa are entirely different to those of the West however, and this sort of application introduces a number of contextually-specific questions that are not considered by the majority of the NPM literature. The few studies that do investigate NPM in Africa are evaluative in content, seeking to understand whether reforms work and to identify barriers to success. Invariably, whether they find in favour of public management reform or not, the same issues are highlighted: lack of capacity, weak institutions, and improper implementation. This thesis will build a theory of NPM reform that is particular to the African context. I develop this theoretical extension through an intensive ethnographic case study of one of the most important on-going public health reform efforts in Africa—the transformation of the entire Ethiopian hospital system to an NPM-style administrative regime. I develop a constructionist theory of African NPM through thick description of the hospitals under reform. I detail the various ways in which the reforms are indigenised as they meet up with local understandings, institutions, and market contexts, and the inevitable unintended consequences as managers seek to ‘make do’ in environments radically different to those of NPM’s origins. I then conduct a detailed consumer analysis to describe the strategies employed by patients seeking care. Patients from different class positions use very different strategies to get health services and I demonstrate that the reform has very different consequences for Ethiopians across classes. Many patients are not recognisable as the ‘customers’ described in the reform documents, and so the hospitals do not organise their reform efforts to serve them.

The master's thesis is focused on analysing a marketing strategy of a specific wellness facility. The analysis itself and recommendations based on it are both aims of the thesis. The theoretical part consists of three main segments - health and wellness tourism, marketing and the location description. The practical part is introduced by characterizing the facility and its business environment, by using PEST analysis, among others. Marketing mix is then used for the description of the product. Above gained findings are summarized in SWOT analysis. The innovation of the product which took place during writing the thesis is mentioned in one subchapter as well. In the conclusion recommendation for the upcoming period and overall summary are presented. Comparative and analytic methods are used in the master's thesis.

O objetivo desta pesquisa foi identificar o perfil gestor dos dentistas do município de São José dos Campos, e elencar as principais carências e necessidades de capacitação destes profissionais para a melhor organização e administração de suas clínicas. Os métodos e recursos utilizados para o desenvolvimento deste trabalho foram as pesquisas bibliográficas e a avaliação quantitativa por meio de questionário elaborado com perguntas estruturadas para respostas de múltipla escolha, formatado em três seções: I - Perfil do Entrevistado, II - Perfil Empreendedor, III - Rotina Operacional, para mapear e diagnosticar as práticas adotadas na gestão clínica. Os recursos utilizados para a submissão do questionário aos dentistas foram: Criação de QR Code para captura por meio de tecnologia móvel, criação de URL para acesso via internet e material impresso para resposta presencial. O estudo avaliou 219 dentistas e o indicador de estabilidade da pesquisa é de 95%. Os resultados confirmaram estatisticamente que os dentistas durante a formação, recebem pouca ou nenhuma informação de como realizar a gestão adequada de um consultório odontológico. The objective of this research was to identify the management profile of dentists in São José dos Campos city and to highlight the main needs and qualification needs of these professionals for the best organization and administration of their clinics. The methods and resources used for the development of this work were the bibliographical researches and the quantitative evaluation through a questionnaire elaborated with structured questions for multiple choice answers, compounded in three sections: I - Interviewee Profile, II - Entrepreneur Profile, III - Routine Operational, to map and diagnose the practices adopted in clinical management. The resources used to submit the questionnaire to the dentists were: Creation of QR Code for capture through mobile technology, creation of URL for access by internet and printed material for personaly response. Submitted diploma thesis deals with the topic of Marketing in Occupational Therapy. The work is divided into theoretical and practical part. The theoretical part is focused on two main areas - Marketing and Occupational Therapy. The first part is focused on the familiarization with this field. The second part is devoted to marketing, where its general level to specific characteristic rising from the health care are described. The practical part offers the introduction of Children's Rehabilitation Centre Lentilka, where the questionnaire research based on the charting of satisfaction and evaluation of the occupational therapy service by parents of the occupational therapy clients took place. There was also conducted the quantitative study in the form of questionnaire research among doctors of specific specialism working in the region of Pardubice. This questionnaire research among doctors was concentrated on the charting of the foreknowledge in the occupational therapy field and its possibilities. Getting Them In: An Exploratory Mixed-Methods Study with Implications towards Marketing Marriage and Family Therapy

The purpose of this sequential exploratory mixed methods study was to develop and test a theory of planned behavior questionnaire that includes both direct and belief-based measures for seeking professional help from a MFT for participants and their spouses during times of relationship distress. To complete this goal, three focus groups (N = 24) were conducted to elicit the salient behavioral, normative, and control beliefs associated with seeking professional help from a MFT for participants and their spouses during times of relationship distress. The data was member checked and then analyzed using thematic analysis. Next, two quantitative measures were constructed, one using the salient beliefs elicited during the focus groups and the second using general questions that assessed participants' overall attitude, perceived norm, and perceived behavioral control. Both measures were placed online pilot tested (N = 102) using Qualtrics panels. The results suggest that the measures were accurate predictors of behavioral intention. The main stage then used both measures to assess the predictive ability of the elicited beliefs. The results also suggest that the beliefs accurately predicted participants' behavioral intentions for seeking professional help from a MFT for them and their spouses during times of relationship distress. The results also indicate that an intervention could be used to encourage troubled individuals, couples, and families to seek professional help from a MFT when experiencing relational issues. / Ph.
