Occupational therapists as consultants in Florida schools : a survey

Little research has been done on how well prepared occupational therapists feel to perform the role of consultant. This study investigated the perceptions of occupational therapists acting as consultants in Florida schools, how much education/training they have received and how they perceive their consultation skills. Participants in the study were sent a questionnaire and measures of central tendency were calculated on each item. Crosstabulations using Fisher's exact test were completed to investigate any relationship between type of consultation model used and relationships with educators. A one way ANOVA was conducted to investigate any relationship between self-perceived consultant skills and age, amount of experience, and training. The results indicate that occupational therapists providing consultative services rate their abilities as more than adequate and believe formal training in consultation is only mildly important. No significant relationships were found between training and skills or model used and relationships with educators.

O atendimento ginecológico de qualidade na adolescência vem se tornando cada vez mais necessário na atualidade devido à ocorrência mais precoce da atividade sexual, com conseqüente aumento das gestações não planejadas, das doenças sexualmente transmissíveis e da probabilidade de câncer de colo uterino. Este estudo teve como objetivo avaliar a consulta ginecológica sob a ótica dos médicos e das adolescentes. Foi desenvolvido um estudo de corte transversal através de questionário estruturado aplicado a 191 ginecologistas filiados à Associação de Ginecologia e Obstetrícia do Estado do Rio de Janeiro e a 418 estudantes do ensino médio de escolas estadual, federal e privada da cidade do Rio de Janeiro. Os médicos responderam questões sobre dados pessoais, qualificação profissional, condutas na consulta ginecológica e sobre a necessidade de capacitação para o atendimento de adolescentes. Para as colegiais foram abordados aspectos sócio-demográficos, comportamento sexual e avaliação da consulta ginecológica. Para a análise dos dados utilizou-se o teste qui-quadrado e o t de student. Os resultados mostraram que não houve diferenças sgnificativas entre as escolares do ensino privado e da escola pública federal que, no entanto, apresentaram características distintas quando comparadas às estudantes da instituição estadual. Estas eram predominantemente da raça negra, com responsáveis de menor escolaridade e tinham piores condições de moradia. Apesar do maior número de parceiros, gestações e de abortamentos, além de histórico de violência sexual, foram à consulta ginecológica em idade mais tardia, devido à dificuldade de acesso a serviços de saúde sexual e reprodutiva. Os três grupos de estudantes manifestaram, em comum, o desejo de que o profissional investisse mais tempo, paciência e disponibilidade no atendimento ginecológico. Quanto aos profissionais, foi constatado que os mais jovens e as do sexo feminino apresentaram atitudes consideradas menos conservadoras na conduta médica. Os participantes informaram como principal obstáculo no atendimento desta faixa etária a maior duração da consulta e ressaltaram a importância de treinamento específico e da realização rotineira do exame colpocitológico. Concluiu-se que há necessidade de criação de estratégias que facilitem o acesso e a adesão deste grupo etário à rotina preventiva ginecológica e à capacitação profissional específica. Este trabalho oferece contribuições para o conhecimento da consulta ginecológica e identifica a necessidade de melhoria na qualidade da assistência prestada a esta faixa etária a fim de reduzir os agravos da atividade sexual precoce e desprotegida na adolescência.

SCREEN DOOR MEDICINE: THE INFORMAL MEDICAL CONSULTATION

This study explores the phenomenon of the informal medical consultation, a communication event in which an individual asks for medical information, advice, or care from an off-duty health professional with whom the individual has no formal patient-provider relationship. Using surveys and interviews, the study describes these consultations from the perspective of the health care professional and the informal patient. The study explores foundational theories that offer explanations for the phenomenon. The theories considered include social support, decision-making, social exchange, perceived partner responsiveness to needs, and uncertainty management. This study suggests health care providers perceive informal medical consultations to be more problematic than do the informal patients who consult them. The problematic nature of informal consultations increases as the type of request moves from purely informational to a request for treatment. Informal patients do not perceive this distinction. The informal patient’s motivation to pursue an informal consultation instead of a formal consult is affected by the relationship with, trust in, and access to the informal consultant. The willingness of the informal consultant to engage in an informal consultation is affected by the relationship with the informal patient, the type of request made, and perception of risk/benefit for both the provider and the patient. The study supports the idea that informal medical consultations are potentially problematic within the current medico-legal-ethical environment. Alternately, these consultations may be viewed as offering positive contributions to the health and well-being of informal patients. The study suggests translational research is needed to guide health professionals in considering requests for informal medical consultations.
