The Mindful Path to Valued Living: Understanding the Associations Between Mindfulness and Valued Living

<p>When behavior is directed toward activities, people, and experiences that people find most important in their lives, they are engaged in valued living. Given that valued living is associated with well-being, quality of life, and happiness, understanding ways in which value-concordant behavior can be promoted, enhanced, and maintained is of utmost importance. Two studies sought to examine the associations between mindfulness--as a dispositional trait and as developed through training--and valued living. In Study 1 dispositional mindfulness was strongly related to three aspects of valued living: the general tendency for people to understand their values and act in value-consistent ways; directing behavior toward valued activities; and clarity of, action toward, and feelings of success and satisfaction with action toward specific important values. Study 2 compared valued living scores of people participating in a Mindfulness Based Stress Reduction (MBSR) program to a control group. Mindfulness training was related to increased general valuing processes and value-relevant behavior but was not related to increased clarity of, action, or success or satisfaction with action toward specific values. </p><p>The two studies also explored the mechanisms underlying the relationship between mindfulness and valued living. In Study 1, self-compassion, psychological flexibility, and self-clarity each partially mediated the relationship between dispositional mindfulness and valued living, but decentering did not. In Study 2, change in attention mediated the relationship between group (MBSR or control) and valued living, but acceptance, self-compassion, psychological flexibility, self-clarity, and decentering did not. Lastly, both studies found that valued living mediated the relationship between mindfulness and well-being. Taken together, these findings add to the growing body of literature demonstrating the benefits of mindfulness and highlight the important influence that clearly identifying and behaving in accordance with important values has on well-being.</p> / Dissertation

Mindfulness-Based Stress Reduction as an Intervention Among Family Caregivers of Persons with Neurocognitive Disorders

Providing care for a frail older adult who is suffering from dementia has been described as a stressful experience that may erode psychological well-being and physical health of caregivers. The present study investigated the effectiveness of Mindfulness-Based Stress Reduction (MBSR), among older family caregivers of persons with neurocognitive disorders, compared with an intervention based on progressive muscle relaxation (PMR). Participants were randomly assigned to either MBSR or PMR (n = 28). The MBSR group showed significantly greater reductions in self-reported levels of depression and isolation from pre- to post-intervention, and those changes remained significant at 8 weeks post-intervention. Both groups showed similar decreases in levels of perceived stress, cortisol awakening response, daily average cortisol, and in resting systolic blood pressure from pre- to post-intervention. Results suggest that MBSR and relaxation-based interventions may both be effective for caregivers, however, further research, employing waitlist control participants will be necessary for unambiguous interpretation of the present results.

Previous research has reported positive associations between anger, perceived stress and blood pressure. These associations have largely been based on cross-sectional data and a small number of longitudinal works. Using a prospective longitudinal cohort design, this study more directly tested the relationships between anger, perceived stress and blood pressure by using a psychological therapeutic intervention (mindfulness-based stress reduction) to manipulate anger and perceived stress. Anger and perceived stress were in turn evaluated for association with blood pressure. Despite improvements in psychosocial functioning and reductions in blood pressure, findings from this study failed to demonstrate an association between change in anger or perceived stress with change in daytime or 24-hour ambulatory blood pressure. A model where these variables interact indirectly via stress coping mechanisms or health behaviours may be the linking mechanism in this study. Results from this thesis have contributed evidence to a divided field dominated by cross-sectional research.

Pilot RCT of Mindfulness-Based Stress Reduction (MBSR) Versus Progressive Muscle Relaxation (PMR) to Reduce Symptoms of Distress Among Elderly Dementia Caregivers: Results at One Year Post-Intervention

Providing care for a frail older adult who is suffering from dementia has been described as a stressful experience that may erode psychological well-being and physical health of caregivers. The burden and stress is increased when the caregivers are themselves elderly. The present study investigated an 8-week stress-reduction program, Mindfulness-Based Stress Reduction (MBSR), and compared it to a similarly structured, alternative behavioral intervention, Progressive Muscle Relaxation (PMR), to determine if MBSR was as effective or more effective than PMR at reducing subjective burden, symptoms of depression, perceived loneliness or perceived stress among middle-aged and older family caregivers of persons with dementia and other neurocognitive disorders. Twenty-eight participants were randomly assigned to either MBSR or PMR. Self-report and biological measures were collected on five occasions: At the beginning and end of intervention training, and at 8 weeks, 6 months and 1 year following the end of intervention training. In addition to a packet of self-report questionnaires and home-collected salivary cortisol, a laboratory controlled emotional stress test was designed to elicit an emotionally stressful response relevant to caregivers’ experience of caregiving, and facilitate the measurement of stress-related changes in systolic blood pressure and cortisol reactivity. At 1 year post-intervention, the PMR group showed a significantly greater reduction in perceived stress and disruptive patient behaviors. A reduction in emotional reactivity to patient problem behaviors approached significance (p = .08) at 1 year post-intervention for the PMR group. The MBSR group showed significantly greater reductions in self-reported symptoms of depression and perceived isolation from pre- to post-intervention, and those changes remained significant at 8 weeks post-intervention. However, by 1 year post-intervention, interaction effects were non-significant as both groups showed similar decreases in symptoms of depression and perceived isolation. Both groups showed similar decreases in diurnal cortisol, cortisol awakening response, and daily average cortisol (but not laboratory cortisol) from pre- to post-intervention and further decreases at 8 weeks post-intervention, and showed similar reductions in magnitude of change by 1 year post-intervention. This pattern was similar for both groups with systolic blood pressure, showing decreases from pre- to post-intervention, additional decreases at 8 weeks post-intervention, and returning towards baseline by 1 year post-intervention. Both groups also reported similar increases in levels of dispositional mindfulness and self-compassion and similar improvement in overall sleep quality that was sustained at 1 year post-intervention. No changes were seen for perceived burden or loneliness. Significant correlations with amount of daily practice of the instructed stress-reduction approaches were observed for several of the dependent measures from pre- to post-intervention and 8 weeks post-intervention. From pre-intervention to 1 year post-intervention, an overall pattern emerged, where both groups showed similar improvements from pre- to post-intervention, and additional improvements at 8 weeks post-intervention, but displayed a curvilinear reduction in improvements—with some exceptions—and a return towards baseline at 6 months and 1 year post-intervention. In general, reductions in the magnitude of changes observed by 1 year post-intervention remained below baseline levels. Results suggest that both MBSR and relaxation-based interventions may be differentially effective in reducing psychological and physiological indices of chronic stress among older caregivers of relatives with neurocognitive disorders. However, further research, employing wait-list control participants, will be necessary for unambiguous interpretation of the present results.

Anxiety is a high-prevalence disorder, and it is often treated by medication which can be costly, has side effects, and is not available for everyone in need. Mindfulness-based interventions (MBIs) such as mindfulness-based stress reduction (MBSR) and mindfulness-based cognitive therapy (MBCT) are great alternatives to treat anxiety. Both MBIs are not only more accessible but have no side effects as well. To establish whether MBIs can become a main treatment for anxiety, the associated neural correlates and changes should be investigated further. The aim of this systematic review is to investigate whether neural correlates of MBIs are mainly caused by emotional or attentional neural mechanisms, or if both are involved. There were five studies selected and included according to a set of inclusion and exclusion criteria. The results suggest that MBIs are in fact effective in alleviating anxiety symptoms by enhancing both emotion and attention regulation. However, several of the included studies came with limitations such as having no significance in p-values, no control groups, and small samples. Thus, further research is needed to draw a conclusion on whether MBIs are the best alternative for the alleviation and treatment of anxiety.

Trauma-Informed Mindfulness-Based Stress Reduction to Increase Family Quality of Life for Mothers of Children with Autism: A Pilot Study

Mothers of children with autism have a higher rate of stress than mothers of neurotypical children and mothers of children with other disabilities. This impacts their family quality of life. This study aimed to show that by teaching mothers trauma-informed mindfulness-based stress reduction techniques they were able to switch their perspectives and feel that they had increased the relationships with their child with autism and increased the rating they give their family quality of life. The participants were recruited using word of mouth and through distribution of posters to autism communities following approval of the experiment through the institutional review board (IRB). The participants selected were mothers of children with autism. They interacted with the researcher through Zoom. They completed multiple measures to assess their levels of stress, well-being, mindfulness, and family quality of life. Measures included daily stress self-report, Freiburg Mindfulness Inventory-14 (FMI-14), Beach Center Family Quality of Life (FQOL), and a semi-structured interview. This was a multiple baseline study. Data analysis included visual analysis and changepoint analysis. Mothers of children with autism who utilized mindfulness, defusion, and trauma-informed problem-solving resulted in consistently decreasing levels of stress throughout the intervention. Family quality of life increased, especially in the areas of financial well-being and parenting. The mothers reported the most benefit in the practice of defusion and the least benefit from trauma-informed problem-solving. The research done in this experiment merits further study, especially in the areas of mindfulness and defusion. A larger sample size should be used to identify the benefits more closely from each phase and to identify the impact of a less homogeneous group of people. It can then be generalized to other parents of children with special needs.

A Bunker Garden: Mindfulness-Based Landscape Design to Restore Physicians from Burnout

Landscape architects design healing gardens at healthcare facilities to support patients, visitors, caregivers, and staff. Many acknowledge that medical staff regularly visit healing gardens to escape work-related stress (Marcus and Sachs, 2014). Rarely, however, are healing gardens on medical campuses designed specifically to support physicians' well-being. There is a void in healing garden design theory. Reports on the prevalence of physician burnout, warn of a widespread crisis and dismal reality within the medical community (T. D. Shanafelt et al., 2015). Researchers pronounce an urgent need for evidence-based interventions, which address individual contributing factors to burnout (Christina Maslach, Jackson, and Leiter, 1986). By investigating the Mindfulness-Based Stress Reduction program, an evidence-based therapy, clinically proven to cultivate emotional healing, for physicians suffering burnout, this research reveals how a therapeutic garden could meld mindfulness-based practices with environmental theory; healing garden design precedents; and healthcare design typologies. Finally, mindfulness-based landscape design guidelines describe how a private, restorative, healing garden could help maintain physicians' well-being and rehabilitate physicians experiencing burnout due to emotional exhaustion within the workplace. / Master of Landscape Architecture
