Organização da demanda no serviço Odontológico do Sistema Único de Saúde /

O estudo teve como objetivo analisar e descrever a organização da demanda de pacientes, do serviço de saúde bucal de um município do estado de São Paulo, com enfoque na organização e na resolubilidade do serviço. Trata-se de um estudo exploratório, quanti-qualitativo descritivo dos serviços de saúde bucal, ofertados à população no município de Penápolis - SP. Foram utilizadas diversas estratégias metodológicas: análise documental, análise de dados secundários e entrevista semiestruturada com a Coordenadora de Saúde Bucal do município para obtenção de informações sobre o funcionamento e organização específicas do sistema de saúde do município. Foram analisados prontuários dos pacientes atendidos no período de dezembro a janeiro de 2010 no Centro de Especialidades Odontológicas e descrito o fluxo de pacientes, referenciados e contra-referênciados entre a Atenção Básica e Atenção Secundária. Categorias foram pré-definidas e ajustadas para análise: Descrição da rede de serviços odontológicos; Organização e Integração da Atenção Básica e Atenção Especializada; Procedimentos referenciados ao Centro de Especialidades Odontológicas, que tiveram no mínimo a primeira consulta; Procedimentos referenciados ao Centro de Especialidades Odontológicas, que tiveram tratamento especializado realizado; Tempo necessário para a conclusão do tratamento no Centro de Especialidade Odontológica; Número de consultas para conclusão do tratamento. O encaminhamento entre os níveis... / Abstract: The Brazilian Health System (SUS) brought reformulations on policies and reorganization of dental services, based on doctrine principles: universality, equality and integrality. The study had the aim to analyze and describe the organization of patients' demand, on dental services from a city of São Paulo State, focusing on organization, resolution of service. It's an exploratory and descriptive study with quantitative and qualitative characteristic of dental services offered to population from Penápolis-SP. It was used many methodological strategies: documental analysis, analysis of secondary data and semi-structured interview with the Coordinator of Dental Health from the studied city about the functioning and specific organization of city. It was also analyzed the recorders of patients attended on Centre of Dental Specialties and it was described the flow of patients referred and back-referred between Primary and Secondary Attention. Categories were predefined and adjusted: description of dental services net; Organization and Integration of Primary and Specialized Attention; Proceedings referred to Centre of Dental Specialties that had at least the first consult; Proceedings referred to the Centre of Dental Specialties that had completed the specialized treatment; The time necessary for conclusion in the Centre of Dental Specialties, Number of consults to conclude the specialized treatment. The Primary Dental Attention has 10 dentists, 5 teams of Oral Health and one Centre of Dental Specialties with 5 experts. Topic of the thesis is to clarify the aims of living and values of the handicapped people. In the theoretical part the work deals with definitions of the basic terms related to a corporal defect, course and model of crisis as a radical change regarding physical image of human being, it highlights the main crisis starter, thus stress, and emphasizes the factors which have the greatest effect on mental state of the handicapped patient. Also, this work refers to sources of one{\crq}s reason for living, right to life and possibilities of social assistance for the handicapped. The empirical part is focused to find out in what extent the handicapped people experience their life as a meaningful, in what extent they are satisfied with their life and what values they prefer.

Este trabalho consiste no estudo de propostas de atividades didáticas para alunos com necessidades educacionais especiais, particularmente daquelas que são recomendadas para as salas de aula das escolas públicas estaduais de São Paulo. Teve por objetivos, construir ou adaptar, conforme o caso, aplicar e analisar uma sequência didática, para promover uma aprendizagem adequada a dois alunos com necessidades educacionais especiais, inseridos em uma sala de aula comum, sendo ambos classificados como portadores de paralisia cerebral. Caracterizaremos algumas deficiências que aparecem em escolas comuns ou regulares. Baseando-nos na Proposta Curricular do Estado de São Paulo e também nas Diretrizes Curriculares da Prefeitura de Campinas, como produto deste trabalho, propusemos dois planos de ensino a serem aplicados no sexto e oitavo anos do ensino fundamental e estudamos quais deveriam ser as adaptações desses planos, para adequá-los a esses alunos, sendo um do oitavo e outro do sexto ano do ensino fundamental. Ambas as sequências didáticas foram aplicadas e analisadas, donde concluímos que foram adequadas à aprendizagem de todos os alunos presentes em classe. Porém, também constatamos que as deficiências que aparecem nas escolas, mesmo tendo a mesma classificação, não podem ser tratadas com igualdade, no sentido de que os planos de ensino devem ser adaptados para as necessidades específicas de cada aluno / This research studies proposals of didactic activities to students with special educational needs, particularly those recommended to public schools in the State of São Paulo, Brazil. We characterize some technical deficiencies which arise in regular schools, and specifically some of our case study. Our main aim was to propose, apply, and analyze a didactical sequence, in order to promote significant learning to students with special needs, inserted in regular classroom, both classified as cerebral palsy. Based on the Curricular Proposals of the State of São Paulo, and also in the curricular directions of the city of Campinas, as a production of this study, we have proposed two teaching plans to be applied to the sixth and eighth grades of basic school. We have studied which adaptations should be done in such plans to fit them to those students. We have applied both the didactical sequences, and concluded that they were appropriate to the learning of all students present in class. However, we also have seen that deficiencies that appear in regular schools, even when they have same classification, should not be treated as equals, in the sense that the educational plans need to be adapted to each student´s specific needs

2016 Arizona Statewide Emergency Medical Services Needs Assessment (ASENA)

Emergency Medical Services (EMS) is an institution and product of public health, health care, and public safety that is chopped and scattered across multiple jurisdictional deployment methodologies throughout Arizona. To fully-asses the EMS needs of the state, those jurisdictions are considered as a whole; for it is the whole that makes a system, and a system is what truly impacts patient outcomes. Evaluating the ""whole"" is the genesis and driver of the 2016 Arizona Statewide EMS Needs Assessment (ASENA). The primary objective of ASENA is to establish a current ""snap-shot"" of EMS in the state while simultaneously identifying needs and/or areas that can be targeted for further analysis and/or improvement as part of Population Health Management and Emergency Medical Services Integration under the AZ Flex Grant funded by the U.S. Health Resources and Services Administration (HRSA). In addition, the secondary objective of ASENA is to compare and contrast this current ""snap-shot"" with data obtained in a more narrow needs assessment conducted in 2001, allowing comparison of changes in Arizona's critical access EMS system over 15 years. To accomplish this, a 105-question needs assessment survey tool was developed and distributed to EMS agencies throughout the state. The fully-vetted survey tool collected information pertaining to sixteen core functional sections. Eighty-six agencies fully-completed the needs assessment survey tool, with respondents evenly distributed across the state's four EMS coordinating regions and representative of the various service-delivery methodologies. The combined service areas of the respondents cover over 85% of the state's population. Arizona's statewide EMS system is well organized and positioned to deliver advanced levels of prehospital care for the vast majority of its citizens and visitors, with some variation between urban and rural regions. Key needs identified relate to: patient care reporting between EMS providers, emergency departments and receiving hospitals; quality assurance activities; education and skills training programs; dispatch system capabilities; mass casualty and public health preparedness; equipment and supplies; and more robust use of data and analyses to inform continuous EMS system improvement.

Political Media Bias in the United States: Immigration and the Trump Administration

This research is aimed at identifying political bias in mainstream media news channels. Specifically, this thesis focuses on political bias portrayed through the media following the inauguration of President Donald Trump. This analysis explores the media’s coverage of the initial travel ban (enforced by executive order) during the first month of the Trump presidency. The content in this research explores specific frames, facts, statistics, wording, phrasing, and overall presentations of two primetime media hosts, Rachel Maddow and Sean Hannity. This research explores several presentations from each host, all pertaining to the Trump Administration’s initial travel ban. Framing theory is used throughout the manuscript to emphasize the manner in which each host presents news and information, while also distinguishing the differences between each host due to their partisan agendas. Through this research, it was found that both media hosts presented news and information on the same issue (the travel ban) through completely different perspectives. Each host varied drastically in tone, phrasing, and facts of emphasis, while also presenting each issue in a manner that aligns with a specific partisan agenda. This research demonstrates that partisan agendas and perceived audience needs take priority over the reporting of objective facts and straightforward coverage on the issue of immigration in the United States.

Perceptions of patients on the fulfilment of their basic needs while receiving surgical emergency care

An academic hospital in the Western Cape is dedicated to living up to its vision to deliver excellent nursing services, which include the fulfilment of the basic needs of patients who are making use of surgical emergency department. Emergency surgical departments should be created in a manner that provides safety and comfort to patients. Patient satisfaction is influenced by the manner in which their expectations about the successful addressing of their basic needs are met. At the moment, the extent of meeting these basic needs during the delivery of nursing care in the surgical emergency department of the academic hospital is unclear. The purpose of this study was to explore and describe the perceptions of patients about the fulfilment of their basic needs during nursing care in a surgical emergency department at an academic hospital in the Western Cape. The study was conducted according to a quantitative non-experimental descriptive survey design. A convenience sampling method was used to select patients (n = 150) after they were discharged from a surgical emergency department. A self-administered questionnaire was compiled within the framework of Alderfer's theory, with closed and a few open questions, that was for distribution to respondents. Descriptive statistics were extracted. The responses to items were indicated on a continuum starting at 1 (never) to 7 (always) on a 5-point Likert scale. Respondents signed informed consent before they completed the instrument in a private room that took around 30 minutes. In this study, validity and reliability were maintained during the research process, and ethical principles were adhered to. Descriptive data was presented through mean values and standard deviations and a factor analysis performed. The findings were presented according to six factors that indicated that the responses varied on the basic existence, relatedness and growth needs of patients in a surgical emergency unit. From the findings, recommendation was described for the operational nursing managers in the surgical emergency department to take action during addressing patients' basic needs in the delivery of nursing care. It can be concluded that patients perceive that many of their basic needs are not being met while receiving surgical emergency care.

Identification & visualization of patient information elements to support chronic iIlness care: a scoping review and pilot study

Purpose: The purpose of this thesis is to determine what is known from the literature about the use of Clinical Information Systems (CIS’s) to support the information needs of individual health care providers (HCP), in particular the nurse case manager, and the inter-professional team providing chronic illness care in the community setting. Methods and Analysis: This is a scoping review with a pilot study for feasibility. MEDLINE, CINAHL, and WEB OF SCIENCE were searched up to April 2017. Reference lists and a citation manager of included studies were searched to identify further studies. Relevant full text papers were obtained and screened against inclusion criteria. Data from eligible articles was extracted using a predefined extraction form. Thematic narrative descriptions and descriptive statistics were used to summarize findings. Nurse case managers were recruited from diabetes and chronic kidney disease clinics for an exploratory questionnaire and follow up interview. Descriptive content analysis and nonparametric statistics were used to summarize findings of the pilot study. Results: 45 articles were identified meeting the inclusion criteria. Three themes emerged (1) patient information elements (2) visualization formats, techniques, and organization and (3) visualization of patient information elements. Diagnostics and observations were the most frequently mentioned information elements. Text was the main representation format. Four participants completed the pilot study initial questionnaire and one completed the follow up interview. There was 100% agreement for 11 elements. Six themes emerged (1) required information can change (2) information is required for different purposes (3) information required for communication is related to nurse case manager concerns (4) required information varies depending on the discipline reviewing it (5) certain types of information need to be grouped together and (6) it is difficult for a HCP to visualize what is necessary in a CIS without first seeing or trying it. Recommendations: The recommendations are a concept-oriented view customizable to the role of the HCP to display: diagnostics, outcomes and comparisons as graphs and colour coded, observations, medications, problem lists, clinical events, guidelines, the care plan, clinician to clinician communication, patient to clinician communication and clinician to patient communication as text, and clinical events as a timeline. Conclusion: This review and accompanying pilot study is a starting point for a framework of guidelines with the recommendations of proposed patient information elements and the visualization formats, techniques and organization. / Graduate

ASSESSMENT OF DESALINATION NEEDS AND APPROPRIATE TECHNOLGIES FOR SRI LANKA

This study investigates the desalination needs and available technologies in Sri Lanka. Lack of rainfall, pollution due to agricultural chemicals, presence of fluoride, increasing demand, exploitation of ground water and brackishness have created scarcity of fresh pure water specially in near costal and dry zones in Sri Lanka. Due to Cronic Kidney Disease (CKD) around 500 people died in dry zones annually which is suspected to cause by Arsenic and Cadmium contain in ground water due to agriculture chemicals.   The available desalination methods are Reverse Osmosis (RO), Solar distillation and conventional methods. The cost for RO is Rs.0.10 cents per liter and solar distillation Rs.2.96 per liter. Although the price shows that the RO is better but due to high initial investment as a third world country it is very difficult to afford huge initial investment without government intervention. The experimental solar desalination units only produce nearly 5liters of potable water per day and directly impacted by availability of solar radiation. The energy availability of Sri Lanka and future potable water demand predicted as 2188.3 Mn liters as maximum demand which will be in 2030, therefore by that time the government should have a proper plan to cater the demand and desalination plants need to be planned and built based on the demand of dry zones and specially agriculture areas.   The applicability of renewable energy for desalination in local arena was also simulated taking the Delf Reverse Osmosis plant for the simulation. Results show that the optimum design is combination of Solar PV and existing 100kW Diesel generator Set with Battery bank and converter.

Oral health status among children with special needs in Khartoum State, Sudan

Magister Scientiae Dentium - MSc(Dent) / Introduction: Children with disabilities and other special needs present unique challenges for oral health professionals in the planning and execution of dental treatment. The oral health of children with special health care needs is influenced by various socio-demographic factors, including their living conditions and severity of the impairment. According to United States (US) Maternal and Child Health Bureau, special health care needs (SHCN) children are defined as “those who have or are at increased risk for a chronic physical, developmental, behavioral, or emotional condition and who also require health and related services of a type or amount beyond that required by children generally”. Aim and Objectives: The aim was to determine the oral health status of children with special needs. The objectives were to determine the prevalence of dental caries, periodontal disease and oral mucosal lesions among children with special needs in Khartoum State aged 5 to 15 years old attending educational and rehabilitation institutions. Methodology: A descriptive cross–sectional study was carried out. Data were collected with a data capture sheet that was a modified WHO Oral Health Assessment Clinical Oral Examination Guideline. Demographic variables, dmft, periodontal disease and oral mucosal lesions were recorded. Results: The mean dmft /DMFT scores was 2.02/1.25. The DMFT score increased with age, girls showed higher score than boys and autistic children had the poorest oral health among other disabilities. More than two third of the examined sextants were healthy, less than quarter had bleeding (18.3%) and 5% calculus accumulation. Fissured tongue was the most frequent oral lesion found. Conclusion: Children with special needs in Khartoum State demonstrated a high prevalence of dental caries and periodontal disease and do not receive adequate dental care. There is an urgent need for both preventive and treatment programmes to improve the oral health of children with special needs.
