Självbestämmande och inflytande vid behovsbedömning inom äldreomsorgen: En vinjettstudie

Bakgrund och problem: Självbestämmanderätt är en grundprincip i socialtjänstlagen och ska genomsyra allt socialt arbete. Socialtjänstlagen gör ingen skillnad på friska eller sjuka människor, vilket kan skapa dilemman för biståndshandläggare vid behovsbedömning. Syfte och frågeställningar: Studiens syfte är att uppnå en förståelse kring hur bistånds-handläggare arbetar med självbestämmanderätt och inflytande vid behovsbedömning. Studiens frågeställningar är följande: Hur ser biståndshandläggare på äldres självbestämmanderätt när det finns en bristande insikt i de egna behoven? Hur hanterar biståndshandläggare anhörigas inflytande? Metod: Studien är influerad av en kvalitativ och kvantitativ metodblandning och består av en vinjett besvarad av 24 biståndshandläggare, samtliga arbetande utifrån socialtjänstlagen inriktat mot äldreomsorg. Det insamlade empirimaterialet från biståndshandläggarna har bearbetats genom en tematisk analys. Resultat och slutsatser: Resultatet visar att biståndshandläggare menar att den äldre har självbestämmanderätt trots bristande insikt i sina egna behov. Biståndshandläggarnas utsagor visar att den moraliska aspekten att göra gott för den äldre väger tyngre än dennes självbestämmanderätt. Resultatet visar att anhöriga har ett stort inflytande och spelar en viktig roll vid behovsbedömning.

Differences between primary worksite health promotion program provider and program decision-maker in the measurement of success of worksite health promotion programs

This study has presented findings on the correlations of the attitudes of the health promotion program provider and the program decision-maker in measuring the success of their worksite health promotion programs. The purpose of this study was to answer the following three questions: 1) Do company decision-makers and primary program providers agree upon which evaluation methods are currently being used in their worksite health promotion program? 2) Do the company decision-makers and primary program providers rank the top five evaluation criteria in the same order of importance? and 3) In rank order, what are the top five preferred evaluation criteria of the program provider? Results indicate a strong correlation of agreement for questions 1 and 2 (0.937 and 0.951 respectively). The data suggests that primary health promotion program providers and company decision-makers share strong agreement on which evaluation criteria should measure program success. Justification of worksite health promotion requires that the program provider be accountable for the criteria on which the decision-maker bases the programs' success. / Fisher Institute for Wellness

Evaluation of Surgical Quality with a Focus on the Standardized Monitoring of Peri-Operative Morbidity and Mortality

Objective: Evaluation of surgical quality ensures consistency of care and facilitates improvements in the quality of care delivered. Methods: An overview of surgical quality measurement is presented. A system for monitoring thoracic morbidity and mortality (TM&M) at the Ottawa Hospital is introduced and evaluated. Results of a needs assessment survey on the involvement in thoracic surgical research and quality improvement initiatives are presented. Results: Structure, process, and outcomes reflect different viewpoints on how to evaluate surgical quality. The feasibility of the TM&M system is evaluated using descriptive and univariate statistics, while its inter-rater reliability is assessed amongst the Canadian Association of Thoracic Surgeons. Conclusions: Outcomes have been fundamental in the evaluation of surgical quality. TM&M classification system advocates for a practice of continuous quality improvement and provides standardized and reliable feedback on surgical outcomes. Results of the needs assessment have built a strong foundation of knowledge on prospective ways to enhance the monitoring of surgical quality.
