Tyrimo tikslas – įvertinti veiksnius, turinčius įtakos bilirubino koncentracijai naujagimio kraujyje, ir nustatyti kriterijų, kuriuo remiantis galima prognozuoti naujagimių hiperbilirubinemiją esant ABO antigeniniam netapatumui. Tyrimo uždaviniai: įvertinti motinos ir naujagimio veiksnius, turinčius įtakos naujagimio bilirubino koncentracijai kraujo serume 6 gyvenimo valandą; ištirti veiksnius, turinčius įtakos bilirubino koncentracijos naujagimio kraujo serume pokyčiui pirmą–trečią gyvenimo parą bei pokyčio prognozinę vertę; nustatyti bilirubino koncentracijos kraujo serume, hematokrito, retikulocitų, sferocitų kiekių ir tiesioginės Kumbso reakcijos vertę prognozuojant naujagimių hiperbilirubinemiją, esant ABO antigenininiam netapatumui; palyginti invazinį ir neinvazinį bilirubino koncentracijos nustatymo metodus esant ABO antigeniniam netapatumui; įvertinti perkutaninės bilirubinometrijos vertę diagnozuojant naujagimių hiperbilirubinemiją esant ABO antigeniniam netapatumui. / The aim of the study was to assess the factors influencing neonatal bilirubin level and to establish predictive criteria of neonatal hyperbilirubinemia in ABO antigenic incompatibility. The tasks to be carried out were as follows: to establish maternal and neonatal factors influencing neonatal total serum bilirubin at the age of 6 hoursTo establish the factors influencing the change of neonatal total serum bilirubin on days 1-3 of life and to identify the predictive value of this change. To establish the value of total serum bilirubin, hematocrit, reticulocyte, spherocyte and direct Coomb’s test in prognosing neonatal hyperbilirubinemia in ABO incompatibility, to compare non-invasive and invasive methods of total serum bilirubin assessment in the case of ABO incompatibility.

The denial of neonatal pain : a Wittgensteinian investigation

This essay presents a Wittgensteinian analysis of the rationales, beliefs, and contributing factors that supported the denial of neonatal pain until the late 80's. It provides an historical review of the denial of neonatal pain and describes the main events leading to its recognition. It explores the link between enduring erroneous conceptions and scientific assumptions about the nature of pain, and the denial of neonatal pain. Wittgenstein's work on the origin of pain-language in the natural tendency of human beings to exhibit pain-behaviour and to react to the pain manifestation of others by engaging in meaningful activities provides the background for this investigation of neonatal pain. The lack of training in pain assessment techniques and the unique pain manifestation of sick and premature infants is considered. The impact of Neonatology's driving concepts and the overreaching scientifical approach of medicine is also discussed. Finally, it is recognized that individual, scientific, and sociopolitical forces have influenced neonatal pain research and clinical practice. The essay concludes with a reflection on the consequence of privileging the biological function over the experiential dimension of life for sick infants presently in the N.I.C.U but also for the quality of their long-lasting memories.

The effects of gender differences in newborns on adult-infant interaction

The current study examined whether female and male newborns could be differentiated using physical and behavioural characteristics. Sixteen female and fifteen male newborns were videotaped interacting briefly with a female and male experimenter. Raters coded neonatal physical characteristics, and both neonatal and adult behaviours. Results showed that raters could discriminate the sex of the newborns based upon the degree of cuddliness and activity level. It was also found that adults responded differently with female and male newborns. Finally, the male and female experimenter were found to differ in their behaviours with the newborns. Discussion focuses on the importance of methodological considerations in assessing sex differences in neonatal behaviours, and on the implications of neonatal behavioural sex differences on subsequent parent-child interaction.

The Effect of Caffeine on the Neurobehavioral and Neuropathological Outcome of the Newborn Rat

Caffeine is used for the treatment of apnea of prematurity. The objective of this study was to determine the long term neuropathological and neurobehavioral effects of caffeine on the immature rat brain. Newborn rats were injected with either caffeine, or normal saline from postnatal days 3 to 7, equivalent to the human premature infant of 28-36 weeks. Behavioral tests revealed no abnormality in caffeine treated animals compared to controls. Fluro-Jade B stain of P4 rat brains showed that caffeine caused significant neuronal cell death in some areas of the brain, compared to controls, but this alteration was transient and not present at P8. Anti-NeuN stain at P21 showed significant neuronal cell loss in CA1 and hypothalamus regions in the caffeine group, but not at P160. Anti-Neurofilament M stain at P8, P21 and P160 showed no differences between the control and caffeine groups. We conclude that use of caffeine has no significant effect on the behavioral tests measured in our newborn rat pups. While caffeine caused neuronal cell death at P4, and neuronal cell loss in CA1 and hypothalamus regions at P21, there was no long-lasting effect on neuropathological outcome. However, given these latter findings, the use of caffeine in the premature infant must still be done with caution. / Medical Sciences

The markers of interplay between the music therapist and the medically fragile newborn infant

An emergent qualitative design provided the scaffolding for the micro-analysis of video footage and subsequent video-cued discussion with four reviewers. The collated material was used to prepare thick descriptions which were annotated for infant and therapist behaviours. A further thematic analysis of these behaviours provided 14 sets of behaviours used by the medically fragile newborn infants to indicate availability for interplay and 20 sets of behaviours used by the therapist in response to the infant. The interaction of these categorised behaviours provided seven markers of interplay between the music therapist and the medically fragile newborn infant.
