The influence of market structure, collaboration and price competition on supply network disruptions in open and closed markets

The relaxation of international boundaries has enabled the globalisation of markets making available an ever increasing number of specialised suppliers and markets. Inevitably this results in supply chains sharing suppliers and customers reflected in a network of relationships. Within this context firms buyers configure their supply relationships based on their perception of supply risk. Risk is managed by either increasing trust or commitment or by increasing the number of suppliers. Increasing trust and commitment facilitates collaboration and reduces the propensity for a supplier to exit the relationship. Conversely, increasing the number of suppliers reduces dependency and increases the ease of making alternative supply arrangements. The emergent network of relationships is dynamic and complex, and due in no small part to the influence of inventory management practices, tightly coupled. This critical organization of the network describes a system that contrary to existing supply chain conceptualisation exists far from equilibrium, requiring a different more appropriate theoretical lens through which to view them. This thesis adopts a Complex Adaptive Systems (CAS) perspective to position supply networks as tightly coupled complex systems which according to Normal Accident Theory (NAT) are vulnerable to disruptions as a consequence of normal operations. The consequential boundless and emergent nature of supply networks makes them difficult to research using traditional empirical methods, instead this research builds a generalised supply network agent based computer model, allowing network constituents (agents) to take autonomous parallel action reflecting the true emergent nature of supply networks. This thesis uses the results from a series of carefully designed computer experiments to elucidate how supply networks respond to a variety of market structures and permitted agent behaviours. Market structures define the vertical (between tier) and horizontal (within tier) levels of price differentiation. Within each structure agents are permitted to autonomously modify their prices (constrained by market structure) and collaborate by sharing demand information. By examining how supply networks respond to different permitted agent behaviours in a range of market structures this thesis makes 4 contributions. Firstly, it extends NAT by incorporating the adaptive nature of supply network constituents. Secondly it extends supply chain management by specifying supply networks as dynamic not static phenomena. Thirdly it extends supply chain risk management through developing an understanding of the impact different permitted behaviour combinations on the networks vulnerability to disruptions in the context of normal operations. Finally by developing the understanding how normal operations impact a supply networks vulnerability to disruptions it informs the practice of supply chain risk management.

Three Mile Island, Chernobyl, and the Fukushima Daiichi Nuclear Crises: An Argument for Normal Accident Theory

Thesis advisor: Donald Fishman / This paper will study three particular accidents in the nuclear industry: Three Mile Island, Chernobyl, and the Fukushima Daiichi plant. These crises will be evaluated through a crisis management framework, using two main accident theories: Normal Accident Theory, and High Reliability Theory. The examination of the crises and the organizations involved will show that no matter how reliable the complex systems are, accidents are inevitable in the nuclear industry. High reliability theory expresses an ideal for complex organizations. While following the theory’s suggestions can limit some problems from occurring, acting as a mindful and reliable organization cannot prevent all disasters. The three cases presented in this paper will show that Normal Accident Theory must be accepted by the nuclear industry. Thus, governments and nuclear power plant operators must be prepared with crisis management plans in order to successfully handle emergency situations and limit damages. The first part of this paper will introduce Normal Accident Theory and High Reliability Theory. Evaluating the Normal Accident Theory in Complex Systems as a Predictive Approach to Mining Haulage Operations Safety

The Normal Accident Theory (NAT) attempts to understand why accidents occur in systems with high-risk technologies. NAT is characterized by two attributes: complexity and coupling. The combination of these attributes results in unplanned and unintended catastrophic consequences. High-risk technology systems that are complex and tightly coupled have a high probability of experiencing system failures. The mining industry has experienced significant incidents involving haulage operations up to and including severe injuries and fatalities. Although the mining industry has dramatically reduced fatalities and lost time accidents over the last three decades or more, accidents still continue to persist. For example, for the years 1998 - 2002, haulage operations in surface mines alone have accounted for over 40% of all accidents in the mining industry. The systems thinking was applied as an approach to qualitatively and quantitatively evaluate NAT in mining haulage operations. A measurement index was developed to measure this complexity. The results from the index measurements indicated a high degree of complexity that exists in haulage transfer systems than compared to loading and unloading systems. Additionally, several lines of evidence also point to the applicability of NAT in mining systems. They include strong organizational management or safety system does not guarantee zero accidents, complexity is exhibited in mining systems, and they are interactive and tightly coupled systems. Finally, the complexity of these systems were assessed with results indicating that a large number of accidents occur when there are between 4 or 5 causal factors. These factors indicate the degree of complexity necessary before accidents begin to occur.

This thesis examines the impact of urban water infrastructure on the Ottawa River through an exploration of the City of Ottawa's historical development from the early modern period to the present. The primary aim is to explain how the Ottawa River came to be removed or ignored from the City of Ottawa's urban development strategy. The thesis focuses on the periods of 1910-1920 (early modernity) and 1999-2012 (present). The theories applied are risk, risk management, normal accident theory and the politics of infrastructure. The data and information for this thesis were primarily retrieved from the City of Ottawa website as well as from the archives of the City of Ottawa. The thesis identifies several factors explaining why the Ottawa River has been mistreated over time, as well as the challenges involved in reforming present-day practices and infrastructure. Several recommendations to fix the situation are advanced.

When Sugar Turns to Sh%&: Immediate Action Decision Making and Resilience in High Reliability Teams

Organizational scholars have long been interested in organizations which exemplify high reliability. While such organizational studies have provided valuable clues to the ways in which such organizations form and function, this paper argues that a more nuanced study of high reliability processes within team contexts is warranted. This study focuses on organizational teams which are faced with the challenges of maintaining high levels of reliability. Of particular interest is how teams manage adverse events which disrupt the team's process and how they make adaptations immediately to restore their functionality. In my dissertation, I: (1) explore the existing literature surrounding high reliability organization and resilience, (2) present a qualitative analysis of Special Weapons and Tactics (SWAT) teams to explore and identify factors surrounding adaptation within the critical moment, and (3) discuss the implications of these factors in the theory and research surrounding high-reliability teams. The findings of this study find strong connection with the work of Weick and serve to advance and clarify previous characteristics associated with high reliability organizing; however, by using the small group as the unit of analysis for the study additions to concepts traditionally associated with high reliability organizing can be noted: (1) controlling variability during team function, (2) accepting the value of the unexpected, (3) continuous forward motion, and (4) the role of tacit and explicit knowledge.
