The Relationship Between Teaching and Attainment of Knowledge and Skill Performance by Nurse Aides in a Rural Area

The problem of the study was to determine the relationship between teaching methods and attainment of knowledge and skill performance of nurse aides in rural area nursing homes. The purpose of this study was to determine the improvement in skills performance and knowledge gain in the population cited above. The skill to be learned was operation of an electronic thermometer. The analysis of variance used on the pretest for written performance showed a significant difference at the .01 level; however, the analysis of covariance done on the posttest results showed no significant difference, indicating that the initial difference was of no consequence to the results obtained on the posttests. The paired comparison t test of the group means obtained from written tests showed that the programmed instruction group was superior to either the control or teacher-taught groups at the .001 level. Overall the programmed instruction tool was more effective in teaching skill and learning of cognitive knowledge. This report concluded that the evidence seems to support the use of programmed instruction as a means of economically and effectively teaching nurse aides who work in a rural area nursing home.

The perceptions and experiences of auxiliary nurses regarding breastfeeding in a pediatric setting of an academic hospital in the Western Cape

Thesis (MCur)--University of Stellenbosch, 2011. / ENGLISH ABSTRACT: Breastfeeding is considered as the most preferable method of infant feeding to fulfill babies’ nutritional needs important to the growth and development of babies. Unfortunately, there were babies not breastfed due to numerous reasons including infant illness and hospitalization. The purpose of this research was to describe the experiences and perceptions of nurses regarding breastfeeding in a pediatric setting of an academic hospital in the Province of the Western Cape. A descriptive study design with a qualitative approach was used to explore the experiences and perceptions of auxiliary nurses regarding breastfeeding. Purposive sampling was utilized to consciously select thirteen auxiliary nurses to participate in the study. Semi-structured interviews were used to collect data. Data analysis involved the transcribing of tape recorded interviews, the generating of themes and sub-themes, coding of the data, interpretation and organization of data and the drawing of conclusions. According to the auxiliary nurses’ perceptions, most of the babies in the pediatric setting were not being breastfed. Breastfeeding was being supplemented with formula milk when mothers chose not to breastfeed, experienced breastfeeding problems, were not with the baby or the baby was too sick to breastfeed. The babies’ illness, the hospital environment and lack of resources were challenging auxiliary nurses when supporting breastfeeding mothers. Not all health professionals were supportive of breastfeeding. Interrelated factors including, shortage of staff, time constraints, heavy work-loads, auxiliary nurses’ breastfeeding knowledge and experience, their confidence to support breastfeeding and communication regarding breastfeeding, influenced auxiliary nurses’ ability to support breastfeeding babies and mothers. The research findings indicate that there was a need for breastfeeding promotion in the pediatric setting. Recommendations included a written breastfeeding policy, breastfeeding training for all health care professionals, better breastfeeding education and support for mothers, the maintenance and of breastfeeding during the babies’ illness, adequate accommodation for breastfeeding mothers and the support of breastfeeding mothers who are HIV positive. / AFRIKAANSE OPSOMMING: Borsvoeding is beskou as die mees verkieslike voeding metode vir babas om hul te voorsien aan die nodige voedingsbehoeftes belangrik vir die groei en ontwikkeling van babas. Die doel van hierdie studie was om die ervarings en persepsies van verpleeg assistente met betrekking tot borsvoeding in ‘n pediatriese instelling in ‘n akademiese hospitaal in die Provinsie van die Weskaap te beskryf. ‘n Beskrywende studie ontwerp met ‘n kwalitatiewe benadering was gebruik ver die doel van die studie. Streekproeftrekking was gebruik om doelbewus drie assistant verpleegkundiges te selekteer om deel te neem aan die studie. Semi-gestruktureerde onderhoude was gebruik om data in te samel. Die data-analise behels die transkibering van band opgeneemde onderhoude, die opwekking van temas en subtemas onderverdeel, kodering van die data, interpretasie an organisasie van die data en die opstel van gevolgtrekkings. Dit was bevind na gelang van die assistant verpleegkundiges se persepsies, dat meeste van die babas in die hospitaal was nie geborsvoed. Borsvoeding was aangevul met formule melk wanneer moeders verkies om nie te borsvoed, ervaar borsvoeding probleme, was nie met die babas, of die baba was te siek om the voed aan die bors. Baba siektes, die hospitaal omgewing en die gebrek aan hulpbronne was uitdagend vir assistant verpleegkundiges om borsvoeding moeders te ondersteun. Interafhanklike faktore insluitend, ‘n tekort aan personeel, tyd beperkinge, swaar werk-vragte, borsvoeding kennis en ondervinding, die vertroue om borsvoeding te ondersteun en kommunikasie met betrekkking tot borsvoeding, het die borsvoeding ondersteuning van assistant verpleegkundiges beinvloed. Die navorsingsbevindinge dui daarop dat daar ‘n behoefte was aan borsvoeding bevordering in die hospitaal. Aanbevelings sluit in ‘n skriftelike borsvoeding beleid, borsvoeding opleiding vir alle gesondheidspersoneel, beter borsvoeding inligting en ondersteuning vir moeders, die instandhouding van borsvoeding tydens hospitalisasie, voldoende akkomodasie vir borsvoeding moeders and die ondersteuning van borsvoeding moeders wat MIV-positief is.

A Study of the Omnibus Budget Reconciliation Act of 1987 and the Amendments of 1989 and 1990. Mandatory Education for Nursing Assistants and Their Effect on Job Performance in Two Counties in Florida

The purpose of this study was to focus on the Omnibus Budget Reconciliation Act of 1987 and the amendments of 1989 and 1990. Nursing assistants were placed in cluster groups of 300-hours, 120-hours, and 0-hours. Each subject's job performance of nursing care was observed using the Francis's Task Performance Rating Scale. The purpose of the analysis was to determine (1) if there was a significant difference in job performance of patient care between program completers and the challengers, (2) if there was a significant difference in the job performance of patient care between 300-hour and 120-hour, and (3) if there was a significant difference between the content required in OBRA mandated nursing assistant programs and actual job performance skills needed in patient care. It was determined that program completers' job performance of patient care was significantly different from the challengers.

Exploring the role of patient care workers in private hospitals in the Cape Metropole

Thesis (MCur)--Stellenbosch University, 2015. / ENGLISH ABSTRACT: Controversy was observed regarding the opinions of nursing managers on the role of patient care workers (PCWs) in private hospitals. These opinions ranged from praise for their contribution towards patient care to serious concerns about the impact of their role on patient safety. The aim of this study was therefore to explore the role of PCWs in private hospitals in the Cape Metropole, South Africa. A qualitative approach with a descriptive design was applied to explore the role of PCWs as perceived by unit managers, nurses and patient care workers. Purposive sampling was used to select participants from medical and surgical wards from three different private hospitals, one each from the three major private hospital groups in South Africa (n=15). Permission to conduct the study was obtained from the Health Research Ethics Committee of the Stellenbosch University, as well as from the private hospital organisations. Fifteen semi-structured interviews were conducted, transcribed and analysed. Six themes emerged from the data. These included PCW activities, care organisation, position in the patient care team, training, reasons for employment and concerns about the PCW role. The findings indicated strong similarities with the health care asistant role as described in the literature study. The activities of PCWs are focused on direct patient care and they spend much time with patients. They are close observers of the patient’s condition and report to nurses. PCWs seem to be well integrated into the patient care team and are mostly seen as nurses. Yet, there are concerns about their evolving role despite their limited training programmes and the lack of direct supervision. The researcher recommends that the work of PCWs should be regulated, but that the nursing profession should critically evaluate the need for another nursing category in addition to that of the enrolled nurse auxiliary. / AFRIKAANSE OPSOMMING: Teenstrydigheid is waargeneem met betrekking tot die opinies van verpleegbestuurders oor die rol van pasiёntsorgwerkers (PSWs) in privaat hospitale. Hierdie opinies het variëer van waardering vir hul bydrae tot pasiёntsorg tot ernstige besorgdheid oor die impak van hulle rol op pasiënt veiligheid. Die doel van hierdie studie was dus om die rol van PSWs in privaat hospitale in die Kaapse Metropool in Suid Afrika te ondersoek. ‘n Kwalitatiewe benadering met ‘n beskrywende ontwerp is gevolg om die rol van PSWs, soos waargeneem deur eenheidsbestuurders, verpleegsters en PSWs self, te ondersoek. Doelgerigte steekproeftrekking is gebruik om deelnemers van mediese en chirurgiese sale uit drie verskillende privaat hospitale, een uit elk van die drie grootste privaat hospitaal organisasies in Suid Afrika, te kies (n=15). Toestemming om die studie te doen is verkry van die Etiek Komitee vir Gesondheidsorgnavorsing van die Universiteit van Stellenbosch sowel as van die privaat hospitaal organisasies. Vyftien semi-gestruktureerde onderhoude is gevoer, woordeliks getik en ge-analiseer. Ses temas het uit die data na vore gekom. Dit sluit die aktiwiteite van PSWs, die organisering van sorg, plek in die pasiёntsorg span, opleiding, redes vir indiensneming en besorgdheid oor die rol van PSWs. Die bevindinge toon ‘n sterk ooreenkoms met die rol van die gesondheidsorg assistent soos beskryf in die literatuur. PSWs fokus op direkte pasiёntsorg en spandeer baie tyd met pasiёnte. Weens hulle nabyheid aan die pasiёnt, kan hulle die pasiёnt se toestand waarneem en bevindings rapporteer aan verpleegsters. PSWs is oёnskynlik goed geїntegreer in die pasiёntsorgspan en word meesal as verpleegsters beskou. Tog is daar besorgdheid oor die uitbreiding van hulle rol ten spyte van beperkte opleidingsprogramme en ‘n gebrek aan toesighouding. Die navorser stel voor dat die werk van PSWs gereguleer behoort te word, maar ook dat die verpleegprofessie die nodigheid van ‘n addisionele kategorie tot die assistent verpleegster, krities moet evalueer.

Nurse satisfaction with delegation to assistive personnel : a descriptive study

Restructuring of health care has become widespread in hospitals across the United States. The emphasis on reducing healthcare costs has forced many organizations to redesign the roles and responsibilities of care givers. The American Hospital Association (1990) reported that 97% of hospitals were using unlicensed assistive personnel for care.The purpose of the study was to determine if registered nurses in central Indiana were utilizing delegation in the workplace and to determine if the use of unlicensed personnel affected nurse satisfaction with work. The theoretical framework was General Systems Theory.The population (N=292) was registered nurses working with unlicensed assistive personnel in a four-hospital network. The number of participants was 66 (23%). The questionnaire, who Helps You with Your Work?, was utilized. Participation was strictly voluntary and the identity of the participants was kept confidential.Data revealed that 95% of nurses who responded were utilizing unlicensed assistive personnel in the workplace. Delegation of certain tasks, such as bed making, weighing of delegation and the personnel should be broadened patients, and measuring and recording intake and output, was occurring. Nurses were not delegating more complex tasks, such as monitoring IV infusions or assisting physicians with examinations.Nurses, overall, were only slightly satisfied with work, as evidenced by a mean of 3.3 (3=neither satisfied or dissatisfied). Job satisfaction was not significantly related to utilization of unlicensed assistive personnel.Unlicensed assistive personnel were being utilized in hospitals in Central Indiana. Nurses were responsible for work delegated to unlicensed assistive personnel. Nurses should realize the importance of determining capabilities and limitations of unlicensed assistive personnel. Job satisfaction was not significantly related to utilization of unlicensed assistive personnel.Future research on the subject utilization of unlicensed assistive to include more than four hospitals. Job satisfaction needs to be measured using different staffing patterns and workload as well as the utilization of unlicensed assistive personnel. The data supported the need for further research on delegation, job satisfaction and the utilization of unlicensed assistive personnel. / School of Nursing
