Antibiotic Use in Home Health: A Primer

Cost containment measures within hospital systems push for earlier discharges on stable patients. Due to patient placement difficulties and costs associated with skilled facilities, antibiotic use in home health care settings is becoming a common occurrence. This trend will likely increase as care continues to shift to outpatient areas. Lack of sufficient serum drug concentrations needed in severe infections and increasing resistance to many of the oral options often necessitates the use of the intravenous (IV) route. Home health care practitioners may have minimal information on patients or limited experience with IV antibiotics that may impact quality of care. This review summarizes key points relevant to IV antibiotics routinely used by home health prescribers, nurses, technicians, and care managers. This review will focus on antibacterial agents including vancomycin, aminoglycosides, beta-lactams, daptomycin, tigecycline, and telavancin. Appropriate dosing, indications, adverse events, monitoring parameters, and feasibility of using IV antibiotics are discussed.

When the etiological agent is Streptococcus, progression is generally less aggressive. Thus, the possibility of outpatient treatment becomes an attractive option from the social and economic standpoint. In the literature, this type of treatment is known as outpatient parenteral antimicrobial therapy (OPAT). Objective: The aim of the present study was to demonstrate that outpatient treatment is safe and effective in cases of streptococcic endocarditis. Patients and Methods: Six patients with bacterial endocarditis, diagnosed using the modified Duke University criteria, were followed up between January 2006 and November 2008. Four patients were female (66.6%) and two were male (33.4%). Mean age was 47.8 years, ranging from 32 to 65 years. All patients were eligible for outpatient treatment and agreed to take part in the study, having been either partially or wholly treated in an outpatient regimen. xiii Imaging and laboratory exams were performed at the beginning and end of treatment and when otherwise deemed necessary. Results: All patients progressed with no complications and with the complete resolution of the infection. The results of the laboratory and imaging exams are presented and discussed. Conclusion: Streptococcic bacterial endocarditis may be safely and effectively treated in an outpatient regimen for selected patients with no concomitant aggravating factors. / A Endocardite bacteriana é uma grave doença infecciosa cujo tratamento é tradicionalmente feito com o paciente internado, recebendo medicação intravenosa. As endocardites cujo agente etiológico é o Streptococcus costumam ter evolução menos agressiva. Assim, a possibilidade de tratamento ambulatorial passa a ser atraente tanto do ponto de vista social como econômico. Esse tipo de tratamento é conhecido na literatura pela sigla OPAT (Outpatient Parenteral Antimicrobial Therapy) Objetivo: O objetivo deste estudo foi demonstrar que, em casos de endocardite estreptocócica bem selecionados, o tratamento ambulatorial é seguro e eficaz. Casuística e Método: Foram acompanhados seis pacientes com endocardite bacteriana por Streptococcus, diagnosticados pelos critérios modificados da Universidade de Duke, no período de janeiro de 2006 a novembro de 2008. Quatro pacientes eram do sexo feminino (66,6%) e dois eram do sexo masculino (33.4%). A idade média foi de 47,8 anos, variando de 32 a 65 anos. xi Todos os pacientes que eram elegíveis para o tratamento ambulatorial concordaram em fazer parte do estudo tendo sido tratados parcialmente ou integralmente em regime ambulatorial. Os exames de imagem e laboratoriais eram feitos no início e ao final do tratamento ou de acordo com a necessidade Resultado: Todos evoluíram sem complicações e com resolução completa do quadro infeccioso. São apresentados e comentados os resultados evolutivos dos exames laboratoriais e de imagem realizados. Conclusão: A endocardite bacteriana Estreptocócica pode ser tratada de forma segura em regime ambulatorial em pacientes selecionados, nos quais não existe concomitância de fatores agravantes, com conseqüente benefício para o paciente.

The subject of the thesis are the documents of the canony of Teplá, as well as some aspect of the administration of the monastery in the Middle Ages, namely under the abbot Sigismund Hausmann (1459-1506). It contains, besides the introductory chapter, three main parts, treating the history of the monastery in the Middle Ages, its documents and its administration, respectively. In the introduction, the basic secondary literature and primary sources for the topic are introduced. In addition, the structure and the methodology of the thesis are outlined. The history of the canony from its foundation up to the year 1526 is given in the first part. The following turning points are chosen for the periodization: 1420, 1459, and 1507. The presentation is based mostly on primary sources and it is focused mainly on the later Middle Ages. The second section, devoted to the medieval diplomatics of the monastery, begins with the typology of the diplomatic material of the Premonstratensian canonies in Bohemia and Moravia. Summary: The thesis focuses on a piece of work in the form of a monastic rule which considerably influenced the whole Christian world, significantly contributed to the formation of spirituality of monastic communities, shaped the European culture and has much to say even today. The rule derives its name from its author, St. Benedict of Nursia. Given the considerable impact of his work on shaping the subsequent history of monasticism, St. Benedict is rightly called the Father of Western Monasticism. The first part introduces the Rule of Saint Benedict (Regula Benedicti). It refers to the period of its creation when certain common foundations of monastic life existed but lacked any solid anchoring. Despite the existence of other monastic rules, the Rule of Saint Benedict became the leading one thanks to its clarity and brevity. The second part deals with the Rule of the Master (Regula Magistri), an anonymous collection of monastic precepts which played a significant role as a source material for the Rule of Saint Benedict. The goal of this thesis was to analyse the development of abbey in Waldsassen since the end of 14th century to the finish of George's of Podiebrad reign. Author struggled to describe on the basis of analysis of above all unpublished monastic sources, which are deposited in State archive in Amberg, various aspects of life in Waldsassen convent. Apart from the economic base and its changes, which are well covered by amount of sources, also social a political relations to secular power in the Holy Roman Empire - roman rulers and princes are studied. Then the basis deals with contacts of monastery with the Crown of Bohemia and its aristocracy and also with the position of Waldsassen in the Order and its cooperation with other Cistercian monasteries. Matters dealt with the papal throne are not forgetten, too. Detailed analysis was dedicated to parish network and the land register of Waldsassen, which presents the valuable source for former economic relationships and toponyms of "Stiftland" and Upper Palatine. The fourth chapter is also comprehensive, describing Waldsassen during the Hussite wars and eliminates some myths about this period in the region. The thesis focuses on the economy of the Cistercian Order, from its inception until the end of the 13th century. There are two parallel analysis of monasteries Obazine (France) and Plasy (Bohemia), which are compared in the final part of the research. First the study present foundation of monasteries in the historical context of the country and the region. Then he focuses on the process of acquiring property and attempts to reveal common and different features in the economic development of the courts and farming. The work may present the real situation only to the extent that it allows the historical sources. The main historical source is cartulary of Obazine and documents of Plasy from Codex diplomaticus et epistolaris regni Bohemiae. In addition, the study draws information from various foreign and czech literature.

The significant, cultural heritage, Kladruby's monastery lies n Tachovon in the Western part of the Czech republic. It lies between Pilsen and Tachov, specifically, Thirty kilometres west from Pilsen and five kilometres south from Stříbro. The history of the township and monastery is closely bound. The middle ages brought prosperity and rise to both of them.To this rise of Kladruby contributed it's great place by the Norimberská stezka and the discovery of silver ore nearby. Thanks to this, in 1230, it was raised by Václav I. to township. With this Kladruby gained the rights of a town ( e.g. The right to organize markets,the right to perform Criminal Law and the right to brew) as one of the first seven town in the Czech republic. Because of the wealth from silver mining, a mining settlement Stříbro was establishes, in the late twelfth century. The Benedictine monastery was established, by Vladislav I. of Přemyslid dynasty, in 1115. He himself is buried there as one of few Czech princes, who are not buried in Prague. Between the 12. And 13.
