Title of project: Prevalence and Willingness of Mothers in a Local Support Group to Ask Pharmacists for Pediatric Dosing of Over-the-Counter (OTC) Products: A Descriptive Study

Class of 2014 Abstract / Specific Aims: To assess the prevalence of mothers asking pharmacists for pediatric dosing and mothers’ knowledge of pediatric OTC use. Subjects: Mothers with ≥ one child under 6 years old in a local mothers’ support group in Tucson. Methods: Questionnaires were sent out weekly for a month by the group’s listserv and social media website to determine the prevalence of mothers that ask pharmacists questions and assess their knowledge of OTC medications and what medical sources they use. Data on ages, ages of children, number of children, race/ethnicity, pharmacy visited, education, insurance coverage, and children’s chronic diseases were collected. Main Results: Twenty-six people responded. About 46% of the participants were 30 - 34 years old. Forty-six percent of mothers had 2 children; 42% had 1 child. About 54% of mothers have asked a pharmacist for pediatric OTC dosing. The reasons mothers gave for not asking pharmacists dosing information were that they hadn’t needed to ask (25%), they asked a doctor (16%), they used an online resource (8%), and they didn’t think about asking (8%). Three questions assessed OTC knowledge; 50% of participants got all questions correct, 38.5% got two questions correct, and 11.5% got one question correct. There wasn’t a difference in OTC knowledge and whether they asked a pharmacist questions (p=0.373). Conclusion: More than half of mothers asked pharmacists dosing information, but this percentage could still be higher. Fifty percent got all three questions right, 38.5% got two questions correct, and 11.5% of mothers got only 1 question correct.

An Interactive Approach to Educate Older Adults on the Safe Use of Over-the-Counter Medications

Class of 2011 Abstract / OBJECTIVES: The objective of this study was to create, implement, and evaluate an interactive educational program for older adults on the safe use of over-the-counter (OTC) medications. METHODS: This was a prospective, interventional study in which all subjects received a 40-minute session covering 12 OTC topics tailored to older adults and delivered by pharmacy students. Convenience sampling was used to recruit participants from five independent senior living communities in Arizona. Following the program, participants completed a questionnaire to address the helpfulness and share their intentions of behavior change. RESULTS: Of 88 older adults who attended the program, 64 met the inclusion criteria and voluntarily completed the anonymous survey. 91.8% of the participants indicated the intervention was either “very helpful” or “moderately helpful”. Upon score conversion, the average score for helpfulness among men and women were 2.35 and 2.37, respectively (p=0.922). 95.2% of the participants agreed they have better understanding about OTCs, and 98.3% reported they would recommend this program to a friend. The majority of the subjects stated they will make changes to the way they use OTCs (79.3%), talk with their pharmacist before purchasing OTCs (86.8%), and discuss OTC use with their providers (88.3%). The three most beneficial topics identified included vitamins/minerals, reading a drug label, and sleep medications. CONCLUSION: An interactive educational program on the safe use of OTC medications, tailored to older adults and delivered by pharmacy students, was helpful and generated positive intentions regarding behavior change in OTC use among participants.

Photonic Integrated Circuit Architecture for Radio-over-Fibre Applications

The aim of the research presented in this thesis is to develop photonic integrated circuit (PIC) for Radio-over-Fiber (RoF) application. As such, at the beginning of the thesis, a dual-function photonic integrated circuit for microwave photonic applications is proposed. The photonic circuit is arranged to have two separate output ports, and depending upon the RF input signal strength, it provides either tunable millimeter wave carriers by frequency octo-tupling of the RF signal or frequency up-conversion of a microwave signal from the electrical to the optical domain. The circuit exploits the intrinsic relative phases between the ports of multi-mode interference couplers (MMI) to provide all the static optical phases needed, hence drift free. In the middle of thesis, a generalized architecture having N parallel phase modulators driven electrically with a progressive 2π⁄N phase shift is analyzed. The proposed design is justified by computer simulation for N=8 architecture with properly determined optical phase shifts to generate frequency multiplication of an electrical signal. The front- and back-end of the circuit comprises 4×4 MMI couplers enclosing an array of four pairs of phase modulators and 2×2 MMI couplers. The proposed design for frequency multiplication requires no optical or electrical filters; the operation is not limited to carefully adjusted modulation indexes. Later on, a generalized approach for achieving frequency multiplication using two cascades MZM is presented. The proposed design consists of a Mach-Zehnder interferometer with each arm containing a pair of Mach-Zehnder modulators (MZM) in series as a means of optoelectronic frequency multiplication (octo-tupling and quattourviginti-tupling). The circuit requires no electrical or optical filters. There is no requirement to carefully adjust the modulation index to achieve correct operation of the octo-tupler. A comparison is made with an alternative functionally equivalent single-stage parallel MZM circuit discussed herein the thesis. Finally, the thesis describes the generation of the same magnitude but opposite sign high order single optical side band from its output ports by using a RF source. A single stage parallel Mach-Zehnder Modulator (MZM) and a two-stage series parallel MZM architecture is described and their relative merits and demerits discussed. As an illustration of a prospective application it is shown how the circuit may be used to transport radio signals over fibre for wireless access; generating remotely a mm-wave carrier modulated by digital IQ data. A detail calculation of symbol error rate is presented to characterise the system performance. A mathematical analysis is provided to describe the principle of operation for all the proposed design and validated by commercially available industrial standard simulation tool.

The effects of aberrant chromosomes on variegation and crossing over in Drosophila melanogaster

It has been suggested (Suzuki, 1965a) that the extrinsic and intrinsic factors which increase crossing over in the centromere regions of Drosophila do so by inactivating these regions at the time of crossing over; the inactivation resulting in altered chromosome structure which permits the intimate synapsis necessary for crossing over. This hypothesis predicts that the 3L heterochromatic marker w⁺ carried in Dp(wm)264.58a, which exhibits position effect variegation, would tend to be inactivated by chromosome aberrations, known to increase crossing over. The reversed acrocentric (RA) compound X chromosome, the X chromosome inversions sc⁴sc⁸ and sc⁸, the autosomal inversions Cy, Sb, and Ubx, and the Minute mutants M(2) and M(3) were tested for their effects on the expression of w⁺ in various coisogenic stocks. The amount of pigment in each eye was visually scored into twelve classes. Crossover analyses of the centromere regions of chromosomes 2 and 3 were performed to confirm the effects of sc⁴sc⁸, Cy, Ubx, and Sb on crossing over. The RA, sc⁴sc⁸, Cy, and Ubx all increase crossing over and significantly depress the activity of w⁺. The degree of pigment reduction is correlated with the amount of increase in crossing over. Combination of these aberrations produces an effect on pigmentation and crossing over greater than that produced by either considered singly. Sb has a slight effect in increasing crossing over and decreasing pigmentation. M(3), known to increase crossing over, significantly decreases pigmentation whereas M(2) does not. The effect of the X chromosome inversion sc⁸ is doubtful. Expression of w⁺ is affected similarly in males and females, an observation suggesting that the chromosome physiology of both sexes is similar although the actual factor(s) mediating crossing over are absent in the males. These results lend support to the proposed hypothesis but, with fluctuating parental source effects and variations found within a stock testifying to the general lability of the system, further tests under stringently controlled conditions are necessary before such experiments can be considered critical. / Science, Faculty of / Zoology, Department of / Graduate

Freedom is human’s natural instinct, which was limited by Ethernet and Fixed Telephony Era. With the emergence of new technologies like wireless fidelity (WiFi) and voice over IP (VoIP) humans once again have freedom of movement; which at the very same time provides enough reasons to change the market dynamics of communication industry. The buzz of Voice over WiFi (VoWiFi) in recent years indicates that VoWiFi is shaping up as the next big challenge to traditional telephony, not only due to cost, but also due to range of services and amount of freedom it can offer. However, at the very same time these technologies have evolved to threaten the well-established telephony markets. Enterprise solutions for VoWiFi require enhanced security mechanism and seamless handovers. To address security related issues Wi-Fi Alliance in conjunction with IEEE introduced an enhanced and interoperable security scheme called WiFi Protected Access (WPA). Real time services are sensitive to latency, hence requiring bounded delay time throughout an ongoing session. Handovers in WiFi networks can take fairly long time which real time services cannot tolerate. The problem is further elevated when WiFi networks are secured by using WPA Enterprise. In this thesis we will examine the complete handoff process in WiFi networks. The impact of handovers on VoIP traffic will also be observed. Following the detailed analysis some suggestions will be presented concerning how to reduce this handoff latency. / Friheten som ligger i människans natur begränsades av Ethernet och den fasta telefonin. Med uppkomsten av nya teknologier så som Wireless Fidelity (WiFi) och Voice over IP (VoIP) återfår människan den en gång förlorade friheten. Samtidigt kommer telekommunikationsindustrin att kunna ändras till sin struktur genom WiFi och VoIP. Integreringen av Voice over IP och WiFi , även mer känd som Voice over WiFi, (VoWiFi) har under senare år indikerat att det är en potentiell utmanare till traditionell telefoni inte bara ur ett kostnadsperspektiv utan också för att denna teknologi medför ökade möjligheter när det gäller nya tjänster. Dock återstår en del arbete för VoWiFi för att kunna rubba den fasta telefonin. Företagslösningar av denna teknologi kräver att säkerhetsaspekterna ses över dessutom måste seamless handover fungera på ett tillfredställande sätt. För att se över säkerhetsaspekterna har Wi-Fi Alliance i samarbete med IEEE introducerat säkehetsmekanismen WiFi Protected Access (WPA). Realtidstjänster är känsliga mot fördröjningar. Handover i ett WiFi nätverk kan ta relativt lång tid vilket är oacceptabelt för realtidstjänster. Problemet blir än mer påtagligt när WiFi-nätet är säkrat med hjälp av WPA. I denna exjobbsrapport kommer handoff processen för WiFi nätverk att behandlas. Effekten av handover för VoIP trafik kommer också att beskrivas. Resultat och analyser kommer att föreslås för hur man kan reducera handoff-fördröjningar.

Sensory over-responsivity in children of 3-5 years: A descriptive, analytical study

BACKGROUND: Sensory over-responsivity (SOR) is a type of Sensory Modulation Disorder (SMD), where the individual has an over-responsive behavioural reaction to non-harmful or non-threatening sensory stimulation, which is out of proportion to the stimulus. SOR can negatively impact a child’s engagement and performance in their daily life. SOR is frequently diagnosed by occupational therapists, and deep pressure is an important facet of the treatment of SOR by occupational therapists. Prior research (Alberts &amp; Ronca, 2012) indicates that the component of pressure in the vaginal birth process aids the infant’s neurophysiological adaption to extra-uterine life. This component is absent in elective caesarean section births. This study therefore set out to determine whether method of birth could be associated with SOR, as well as investigating demographic and other variables linked to SOR. It was hypothesised that there would be a higher prevalence of SOR in children aged 3-5 years born by elective caesarean section compared to those born by vaginal birth. The study objectives were: • To establish a profile (demographic and variables linked to SOR) of participants (mother-child dyads) by birth method group (CS or vaginal delivery) • To determine the prevalence of SOR by birth method • To establish if there is a statistically significant difference in SOR and birth method • To establish which variables (for example birth weight, jaundice, birth complications) are associated with SOR METHOD: A quantitative, descriptive, analytical study was conducted with a sample of 91 children between the ages of 3 years 0 months and 4 years 11 months. Children across various language, cultural and socio-economic groups were recruited and allocated to two groups based on their method of birth - vaginal delivery and elective caesarean section. Caregivers of each child completed the Short Sensory Profile 2 (SSP2) questionnaire as well as a demographic information questionnaire. The scores for SOR were calculated for each participant, and prevalence of SOR between the two birth method groups was compared. Demographic variables were tested for significance between the two groups. The variables showing a significant difference were further analysed to determine any association with SOR. RESULTS: There were 91 participants, 58 in the VB group and 33 in the CS group. Mothers in the VB group gave birth at a younger age (U = 499.0, p < .001), were of a lower income level (chi-square = 11.49, df = 2, p = .003) and more likely to be single (Fishers exact p (2-tailed) = .037). The children in the VB group were of a greater gestational age (U = 472.5, p = .001), had a shorter time period before the first breastfeed (U = 478.0, p = .006), and had fewer sleeping difficulties (Fishers exact p (2-tailed) = .003). The prevalence of SOR for the total sample was 22%. There was a significant association in SOR prevalence and birth method (Fishers exact p (2-tailed) = .034), with greater prevalence in the VB group (29%) as opposed to the elective CS group (9%). There were statistically significant associations between SOR and maternal age (U = 380.5, p = .004), marital status (Fishers exact p (2-tailed) = .003) and time after birth to the first breastfeed (U = 394.5, p = .049). CONCLUSION: There was a statistically significant difference in SOR between the two birth method groups, with higher prevalence in the VB group. This was thought to be linked to cultural and language challenges associated with the use of the SSP2, and the impact of low socio-economic circumstances on child development and the ability to regulate sensory input. Recommendations include developing and validating a culturally appropriate sensory profile questionnaire, available in the most common official languages to facilitate the accurate assessment of sensory modulation of all children living in South Africa. In addition, there is a need to test the birth method hypothesis in demographically balanced groups.
