PREDICTING UNSCHEDULED DENTAL VISITS BY MEMBERS OF THE AUSTRALIAN DEFENCE FORCE

Background. Whether or not the Australian serviceman is dentally fit for deployment is determined by the dental fitness standard based on NATO’s dental fitness standard for military personnel. Recent operations by Australian, European, and USA defence forces has found that being dentally fit to deployment is not predictive of whether or not a member is likely to have a dental problem in the twelve months between their dental assessments. In order to maintain an effective member in the field, a suitable risk management assessment pre-deployment is essential so that dental casualties are better managed either through having fewer casualties and/or having those casualties better managed in the field. Aim. The aim of this thesis is to establish a Dental Fitness Classification which more accurately predicts a member’s risk of an Unscheduled Dental Visit (UDV) than the existing Denclas system of classification. Methodology. A prospective cohort study was conducted on 875 Australian service men and women in all 3 services in 7 operational bases across Australia with ages ranging from 17-56. Participants were enrolled during their annual dental examination where clinical details were recorded and a questionnaire completed on demographics and lifestyles of participants. UDVs over the next twelve months were enumerated as any visit to the dental centre which did not form part of their planned treatment following an annual dental examination. The data were first analyzed for associations between each putative risk factor and a binary variable indicating whether or not participants had a UDV in the 12 months since enrolment into the study. Variables for the modeling were then selected that yielded moderately significant associations with the odds of a UDV. From the prospective modeling, the parameter estimates from the logistic regression model (including the intercept) were then used to create an Excel spreadsheet for use in dental clinics. The spreadsheet contained the algorithm that calculated a patient’s probability of having a UDV, based on the reduced set of clinical findings and questionnaire responses found to optimize sensitivity and specificity. Results. Prediction models were most accurate when created for the two cohorts; the dentally fit and dentally unfit. The dentally fit model has 8 variables; Denclas 2 (OR=1.50); Unsupported enamel on a an endodontically treated tooth (OR=22.87); Deep periodontal pocketing (OR=13.97); Healthy Diet (OR= 0.07); Oral Hygiene Score (OR=0.27); Years of Service (OR= 3.96 & 3.49); Prior Visits to the Dentist only for the Relief of Pain (OR=0.35) and the interaction term of Denclas and Unsupported enamel on an endodontically treated tooth (OR= 0.09). The model has a sensitivity =82.61% and a specificity =70.23%. The dentally unfit model has 3 variables; Denclas 4(OR=7.08); Large Fillings (OR=7.06) and Toothbrushing times per week (OR=1.95) with a sensitivity =62.86% and specificity = 77.71%. The dentally fit model has excellent discrimination (Area Under the Receiver Operator Characteristic (AUROC) =0.83) and the dentally unfit has a reasonable discrimination (AUROC=0.74) and both are better than the Denclas system in practice in Defence Force Dental Services around the world (OR=1.80, sensitivity= 41.67% and specificity =72.22%). These models can be used to determine a member’s risk of becoming a UDV from a simple chairside tool embedded in a Microsoft Excel® file. Conclusion. There is a set of easily obtainable variables that can more accurately predict an ADF member’s likelihood of becoming a UDV than the present Denclas system which can be adapted to determine a member’s risk at the chairside.

O conhecimento da situação de saúde bucal de diferentes grupos populacionais, por meio de levantamentos epidemiológicos, é fundamental para o planejamento e desenvolvimento de ações em saúde adequadas às necessidades e aos riscos específicos de cada grupo, bem como possibilita monitorar os agravos e avaliar as estratégias implementadas ao longo do tempo. Este estudo buscou delinear o perfil epidemiológico de crianças de 12 anos de idade matriculadas nas escolas do município de Rio Grande da Serra, Estado de São Paulo, quanto à prevalência da fluorose dentária e a prevalência e necessidade de tratamento para cárie dentária. Buscou ainda, caracterizar a população, verificando as associações existentes entre as condições de estudo e as variáveis: socioeconômicas, demográficas, de acesso aos serviços odontológicos, percepção em saúde bucal e hábitos de higiene bucal. A amostra foi composta por 393 escolares. Apenas uma cirurgiã-dentista, previamente calibrada (Kappa=0,84), realizou todos os exames clínicos utilizando a metodologia recomendada pela Organização Mundial da Saúde (4a edição). A prevalência da cárie dentária aos 12 anos de idade foi de 55,7% e o índice CPOD médio encontrado foi de 1,5. O maior percentual encontrado quanto à necessidade de tratamento em relação cárie dentária foi para restauração de uma superfície em 30,6% dos avaliados, seguido por restauração de duas ou mais faces em 14,8%. A prevalência da fluorose dentária aos 12 anos de idade, segundo índice Dean, foi de 57%, praticamente restrita aos graus muito leve (48,3%) e leve (8,4%). Encontrou-se associação estatisticamente significante entre fluorose dentária e: número de cômodos na casa; ingestão de dentifrício e tamanho da escova de dente. Também se revelou estatisticamente significante a associação entre cárie dentária e: número de pessoas na casa; visita ao cirurgião-dentista e freqüência de escovação. Os resultados encontrados indicam a necessidade de implementação de medidas de atenção à saúde bucal, em especial aquelas voltadas à educação em saúde, ao controle dos fluoretos e ao monitoramento da fluorose dentária. / Knowledge of oral health conditions of different population groups, through epidemiological surveys, is essential for planning and developing actions on health appropriated for the needs and specific risks of each group, as well as it enables the monitoring of damages and the evaluation of implemented strategies over time.This study aimed at delineating the epidemiological profile of 12-year-old children who were enrolled in schools of the Municipality of Rio Grande da Serra, in the State of São Paulo, regarding the prevalence of dental fluorosis, as well as the prevalence and need of treatment for dental caries. The study has also sought to characterize the population, checking the associations between the study conditions and variables: socioeconomic, demographic, access to dental services, perception of oral health and oral hygiene habits. The sample consisted of 393 students. Only one dental surgeon, previously calibrated (Kappa = 0.84), performed all clinical examinations using the methodology recommended by the World Health Organization (4th edition). Regarding the need for treatment of dental caries, the highest percentage found was for the restoration of a surface in 30.6% of the assessed, followed by the restoration of two or more faces in 14.8% of them. The prevalence of dental fluorosis amongst 12-year-old children, according to Dean index, was 57%, largely confined to very mild (48.3%) and mild degrees (8.4%). It was found a statistically significant association between dental fluorosis and: the number of rooms inside the house; the ingestion of toothpaste; the toothbrush size. It has also revealed a statistically significant association between dental caries and: the number of people in the house; visit to the dentist; and brushing frequency. The findings denote the need for the implementation of oral health care measures, in particular those focused on health education, control of fluorides, and monitoring of dental fluorosis.

Material and Methods: The sample was composed by 170 patients (261 avulsed teeth) treated in Dental Urgency Service of the Dental School of the Federal University of Goiás, Brazil, between 2000 and 2008. The following informations were collected from the patients´ files: gender, age, cause of tooth avulsion, seasonal distribution, type and number of avulsed teeth, replantation, stage of root development, transport media, extra-oral period, presence of additional damage, treatment provided and post-traumatic complications. The statistical treatment analyzed data from frequency distribution and chi-square test. The level of significance was set at 5% for all analyses. Results: Higher occurrence of tooth avulsion was observed in males (71.18%) and patients with 6-10 and 11-15 years old (30.59% each one). The main etiologic factors involved were falls (51.76%), traffic accidents (29.41%) and violence (6.47%). The seasonal distribution showed that most cases occurred in Autumn (March to June; 31.18%), followed by Winter (June to September; 27.65%). The most affected tooth was the upper central incisor (62.45%), followed by the upper lateral incisor (21.46%). Significant proportion (67.23%) of traumatized teeth presented root apex completely formed. One hundred and nineteen teeth (45.59%) were replanted. The majority of the replanted teeth (89.08%) received delayed replantation. Thirty eight teeth (31.93%) were stored in dry media. Periodontal healing was observed in 41 teeth (34.45%), inflammatory root resorption in 44 teeth (36.97%) and replacement root resorption in 22 teeth (18.49%). The most frequent modalities of treatment for replanted teeth were endodontic treatment and the temporary filling of the root canal with calcium hydroxide (58.82%) and endodontic treatment and the definitive root canal filling (26.89%). Conclusion: There was a high number of dental avulsions in males, aged under 15 years, from falls and mainly involving maxillary anterior teeth, and low rate of replantation accompanied by a significant number of teeth stored in non-physiological over a long period time. / Objetivo: Avaliar os aspectos epidemiológicos e fatores clínicos da avulsão de dentes permanentes. Material e Método: A amostra do estudo era composta por 170 pacientes (261 dentes avulsionados) atendidos no Serviço de Urgência Odontológica da Faculdade de Odontologia da Universidade Federal de Goiás, entre os anos de 2000 e 2008. As seguintes informações foram retiradas dos registros odontológicos de cada paciente: gênero, idade, fator etiológico, distribuição sazonal, grupo dentário, número de dentes avulsionados, reimplante, estágio de desenvolvimento radicular, meio de transporte, período extra-bucal, presença de dano adicional ao dente, tipo de tratamento e o tipo de complicação pós-traumática. O tratamento estatístico analisou os dados frente à distribuição de frequência e qui-quadrado. O nível de significância foi de p<0,05. Resultados: Observou-se elevada ocorrência de avulsões em indivíduos do gênero masculino (71,18%) e com idade entre 6-10 e 11-15 anos (30,59% cada um). Os principais fatores etiológicos foram quedas (51,76%), acidentes de trânsito (29,41%) e violência (6,47%). A distribuição sazonal evidenciou elevado número de traumatismos no outono (março a junho; 31,18%) e no inverno (junho a setembro; 27,65%). O dente mais comumente afetado foi o incisivo central superior (62,45%), seguido pelo incisivo lateral superior (21,46%). Significativa proporção (67,23%) dos dentes traumatizados apresentava os ápices radiculares completamente formados. Cento e dezenove dentes avulsionados (45,59%) foram reimplantados. A maioria dos dentes (89,08%) recebeu reimplante tardio. Trinta e oito dentes (31,93%) foram conservados à seco. Reparo do periodonto foi observado em 41 dentes (34,45%), reabsorção radicular inflamatória em 44 dentes (36,97%) e reabsorção radicular por substituição em 22 dentes (18,49%). As modalidades terapêuticas mais comumente realizadas nos dentes reimplantados foram o tratamento endodôntico com a obturação temporária do canal radicular com hidróxido de cálcio (58,82%) e o tratamento endodôntico com a obturação definitiva do canal radicular (26,89%). Conclusão: Verificou-se elevado número de avulsões dentárias em indivíduos do gênero masculino, com idade inferior a 15 anos, decorrentes de quedas e envolvendo principalmente dentes superiores anteriores; e baixo índice de reimplantes acompanhado de significativo número de dentes acondicionados em meios não fisiológicos por longo período de tempo.
